[\[1\]](<https://clinicsjo.com/ar/articles/%D8%A7%D9%84%D8%A3%D8%AF%D9%88%D9%8A%D8%A9-%D9%81%D9%8A-%D8%B1%D9%85%D8%B6%D8%A7%D9%86-%D8%AA%D9%86%D8%B8%D9%8A%D9%85-%D9%85%D9%88%D8%A7%D8%B9%D9%8A%D8%AF-%D8%A7%D9%84%D8%AF%D9%88%D8%A7%D8%A1#ref-S1>)

Medications during Ramadan need a written plan made before fasting begins, not personal guesswork. Many medications taken once or twice a day can be moved to the iftar and suhoor meals, but only after consulting your doctor or pharmacist. Medications taken three times a day need review, and medications for chronic conditions such as epilepsy, heart disease, and organ transplants must not be stopped on your own decision.

## Medications in Ramadan: Why Does the Risk of Error Increase?

Sleep, eating, and drinking schedules change during Ramadan, which disrupts the medication habits you have followed for months or years. You may forget a dose at suhoor because you are drowsy, take two doses close together at iftar, or delay an essential medication until the time has passed.

MedlinePlus, the US National Library of Medicine's health information resource, states that medication errors can happen in hospitals, doctors' offices, pharmacies, or at home [\[1\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S1>). It recommends that you know the name of your medication, how long you have been using it, and how many times a day you take it, and that you read the label every time instead of relying on memory alone [\[1\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S1>). This is the most important thing you need when your daily schedule changes.

Our religion, along with medicine, recognizes that a person who is sick or has a particular health condition may be exempt from fasting. A British diabetes organization notes that patients, elderly people, travelers, pregnant and breastfeeding women, menstruating women, and children below puberty are among the groups who may be exempt from fasting [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>). The final decision is personal, but it should be based on your doctor's advice and reliable religious knowledge.

## What Should You Do Well Before Ramadan?

Sources indicate that the coming Ramadan begins on approximately 7 February 2027 and lasts 29 or 30 days [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>). It is best not to wait until the last week. Medication adjustments need time for trial and follow-up, especially for patients with diabetes, high blood pressure, and heart disease.

### Preparation Steps

1. **Write a list of all your medications:** the name, dose, and timing, including painkillers, vitamins, supplements, and herbs. MedlinePlus recommends bringing this list to every medical visit [\[1\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S1>).

2. **Record allergies and past experiences:** any medication that has previously caused you a problem. You can read more about [drug allergy](<https://clinicsjo.com/en/condition/drug-allergy>).

3. **Book a review appointment:** well in advance of the month, and discuss your test results, medications, and any existing complications, as diabetes organizations advise [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

4. **Ask the essential questions:** Why am I taking this medication? What are its side effects? What should I do if they appear? Does it interact with my other medications? Is there any food I should avoid with it? [\[1\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S1>)

5. **Get a written schedule:** one that specifies each medication and its timing after iftar and before sleep, and post it in a visible place.

The guide to Avoiding Medication Errors at Home can help you organize your family's medications, and you will find the first steps summarized in [this common question](<https://clinicsjo.com/en/faq/avoiding-medication-errors-at-home-first-steps>).

## Can Medications Be Moved to Iftar and Suhoor During Ramadan?

In many cases this is possible, but you should not decide alone. Some medications must be taken with food, some on an empty stomach, and some depend on the timing between doses for their effect. Therefore, your doctor or pharmacist must determine the appropriate adjustment for your specific medication.

Dosing Pattern | General Idea | What to Do |

Once daily | Often can be moved to iftar or suhoor | Ask about the most suitable timing and its relation to food |

Twice daily | Often split between iftar and suhoor | Ask whether the interval between the two doses is suitable for your medication |

Three times daily or more | Difficult to split within the iftar hours alone | Needs review; your doctor may change the medication or its pharmaceutical form |

As needed (painkillers, allergy medications) | No fixed schedule | Ask about safe limits and alternatives |

Extended-release or coated medications | The way they are taken is part of their effect | Do not split, crush, or open the capsule without guidance |

### Why Are Three Doses in a Short Time Not Enough?

If your medication is taken three times a day, do not cram all three doses between iftar and suhoor on your own. This may raise the dose within a short period and leave a long gap afterward. Consult your doctor to determine alternatives or long-acting forms if they are suitable for your condition.

As for antibiotics and anti-infective medications, ask your doctor whether your medical condition allows fasting at all. Illness is a well-known reason for exemption, and diabetes organizations advise not fasting when symptoms of influenza or another illness appear [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

## Which Medications Must Not Be Stopped on Your Own Decision?

Do not stop any chronic medication or change its timing on your own simply because you are fasting. This applies particularly to medications for:

- **Epilepsy and seizures:** delaying or stopping them may increase the risk of seizures.

- **Heart and blood vessels:** such as blood pressure medications, blood thinners, and angina medications.

- **Organ transplants:** they prevent organ rejection, and any adjustment requires the expertise of the transplant team.

- **Thyroid and hormones:** these may have special instructions related to food, so review them with your doctor. You can find general information on [description of thyroid medication](<https://clinicsjo.com/en/service/description-of-thyroid-medication>).

- **Psychiatric and neurological conditions:** sudden discontinuation may cause withdrawal symptoms or relapse.

- **Insulin and diabetes medications:** these are detailed below.

Some of these medications may indeed need adjustment, but adjustment should be made through your doctor, not through personal judgment. If you find it difficult to stick to your plan, tell your doctor clearly, because the goal is to solve the problem, not hide it.

## How Do You Manage Diabetes Medications During Ramadan?

A person with diabetes is the one who most needs a precise medication plan during Ramadan. A British diabetes organization advises that a person with diabetes should not fast unless they have discussed it with their healthcare team, because fasting can harm those with diabetes [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>). It stresses that the level of risk depends on the type of diabetes, the type of medication, how well blood sugar is controlled as measured by HbA1c, and whether there are complications in the eyes, nerves, heart, or kidneys [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

### Key Points from Diabetes Guidelines

- Medications that can cause low blood sugar, such as sulfonylureas and insulin, increase the risk during fasting [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

- Diabetes tablets may need to be changed in type, dose, or timing, and the insulin dose may need to be reduced before fasting begins and its type sometimes changed [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

- Pre-mixed insulin is not recommended during fasting [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

- According to these guidelines, checking blood sugar does not break the fast, but the fast must be broken if blood sugar is very high or very low [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

- If blood sugar drops below 4 mmol/l, the fast must be broken and the low treated with sugar, followed by starches [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

- Forgetting your usual medication, eating too many starches or sugars, or reduced physical activity may raise blood sugar, and high blood sugar increases the risk of dehydration [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

- Do not neglect suhoor, and it is preferable that it contains fiber-rich starches that are absorbed slowly [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

To learn about types of diabetes treatments and how they work, read Diabetes Medications: Understanding Their Types and How to Use Them.

## Diuretics and Blood Pressure Medications: How Do You Avoid Dehydration?

Diuretics, such as furosemide, which is listed in drug reference guides [\[2\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S2>), increase the loss of water and salts. During long hours of fasting, this may lead to dehydration, dizziness, and low blood pressure, especially in hot weather or during physical exertion.

Do not stop your diuretic on your own or double it. Ask your doctor to determine a suitable time, and whether you need to have your electrolytes and kidney function checked during the month. The same note applies to multiple blood pressure medications, as your doctor may need to adjust the dose or timing.

Watch for signs of dehydration: intense thirst, dark and scanty urine, headache, and dizziness on standing. For more, read Orthostatic Hypotension: Standing Dizziness, Medications and Dehydration, and learn about the service [managing blood pressure medications](<https://clinicsjo.com/en/service/management-of-antihypertensive-drugs>). At iftar, drink water at intervals between iftar and sleep, and also at suhoor. If you have diabetes, choose sugar-free fluids, as water is the best option [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

## What About Inhalers, Drops, and Injections?

Here the medical aspect intersects with the religious aspect, and I cannot settle the answer for you. Does an asthma inhaler, eye drops, or an injection affect the fast? This is a question to direct to a reliable religious authority or the imam. Diabetes organizations advise speaking with the imam about alternatives to fasting if you are unable to fast [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

The medical aspect is answered by your doctor or pharmacist: Can I move this medication to after iftar? Is there a more suitable alternative? The list of medications at the NHS includes examples of these forms, such as salbutamol inhalers, timolol eye drops, and insulin [\[2\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S2>), and explains how each medication works, how it is taken, and its side effects [\[2\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S2>).

It is best to ask your doctor first about possible medication alternatives, then ask a religious authority about the ruling, and do not stop the medication until you have both answers. Do not break your fast or stop fasting out of anxiety alone if you have a medical need, as urgent health needs have their well-known ruling among scholars.

## How Do You Avoid Dosing Errors on Ordinary Days of Ramadan?

- **Use a weekly pill organizer:** divide doses by iftar, suhoor, and after Taraweeh.

- **Set alarms:** on your phone, especially for the suhoor dose, and do not rely on memory alone [\[1\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S1>).

- **Establish a fixed routine:** such as linking the medication to the first date you eat or the call to Maghrib prayer, as medical sources confirm the value of a household routine [\[1\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S1>).

- **Do not double a dose you forgot:** ask your pharmacist what to do if you forget a dose of this specific medication.

- **Be wary of painkillers and supplements:** review over-the-counter medicines and [safe-use precautions](<https://clinicsjo.com/en/faq/what-are-otc-medicines-and-safe-use-precautions>).

- **Pay attention to herbs and Ramadan drinks:** they may interact with medications, as in Herbal Medicine: Potential Benefits and Hidden Risks.

- **Understand interactions:** read Drug Interactions before adding any new product.

- **Buy with knowledge:** the Drug Safety Consumer Guide summarizes what to check when purchasing. In Jordan, medications and their advertised prices are regulated through the Jordan Food and Drug Administration (JFDA).

If children's or elderly people's medications are in your care, always ask about the dose; children's doses are "according to weight and as prescribed by a doctor or as directed by a pharmacist." Since children below puberty are exempt from fasting [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>), there is no need to change their medication schedule without advice.

## When Should You Consult a Doctor, and When Should You Go to the Emergency Room?

### Consult your doctor or pharmacist in the following cases

- You have missed repeated doses or do not know how to make up a missed dose.

- New side effects appear after changing the timing of a medication.

- Intense thirst, scanty urine, and dizziness on standing, especially with diuretics or blood pressure medications.

- Blood sugar or blood pressure readings fluctuate, or you need dose adjustments.

- You cannot reach your medical team and do not know what to do with your diabetes medications; diabetes organizations advise not fasting in this case [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

### Go immediately to the nearest emergency department or call 911

- Loss of consciousness or fainting, or severe mental confusion.

- A seizure, chest pain, or severe shortness of breath.

- Persistent vomiting that prevents you from taking medication or fluids.

- Signs of severe allergy such as facial swelling or difficulty swallowing.

- Severe low blood sugar that does not improve, or high blood sugar with vomiting, abdominal pain, and rapid breathing, which may indicate diabetic ketoacidosis (DKA), a condition that requires treatment in hospital [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

If blood sugar drops below 4 mmol/l during fasting, break the fast and treat the low, then seek medical care if it does not improve [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

## How Do You Find the Right Doctor or Clinic for Managing Medications During Ramadan?

Look for the specialty that follows your condition: internal medicine, diabetes and endocrinology, cardiology, neurology, chest medicine, or family medicine. You can search by the governorate where you live to shorten the distance, especially if you need close follow-up during the month. Health centers run by the Ministry of Health in the governorates are also available for initial consultations.

When browsing a doctor's or clinic's profile in the directory:

- Check the specialty, the declared experience, and working hours, and whether they suit Ramadan schedules.

- Read the available contact methods, and check whether you can book or call directly as shown in the profile.

- Do not rely on a single review; comparing several profiles gives a clearer picture.

For blood pressure-related follow-up, you can browse the Directory of Blood Pressure Medication Management Clinics in Jordan, which is a directory for searching and comparison and not a recommendation for any specific provider.

### Questions to Ask at Your Appointment

1. Does my condition allow fasting, or do I need further evaluation?

2. Which of my medications can be moved, and at what time? Which should not be adjusted?

3. What signs require me to break my fast or seek emergency care?

4. When should I come back for a review during the month, and what tests are needed?

5. Will you give me the plan in writing so I can show it to my pharmacist?

Finally, medications during Ramadan can usually be managed safely, but the key to safety is a written plan and early consultation. If fasting puts your health at risk, there are lawful alternatives to ask about from the imam or a religious authority, such as charity and feeding the needy [\[3\]](<https://clinicsjo.com/en/articles/medicines-in-ramadan-timing-doses-while-fasting#ref-S3>).

## Frequently asked questions

### Can I change the timing of my medication during Ramadan on my own?

This is not recommended. Some medications depend on food or on the interval between doses, and changing the timing without advice may reduce their effectiveness or increase their side effects. Contact your doctor or pharmacist well before Ramadan and ask for a written schedule that suits your condition.

### What should I do if my medication is taken three times a day?

Do not squeeze the three doses into a short time between iftar and suhoor. This pattern needs a doctor's review, as there may be an alternative of the same medication with fewer daily doses or a different pharmaceutical form, and fasting may not be suitable for your condition. The decision rests with your doctor.

### Can epilepsy, heart, or organ transplant medications be stopped during fasting?

They must not be stopped on your own decision. Interruption may lead to seizures, cardiac complications, or rejection of the transplanted organ. If you fear you will not be able to keep to the schedule, discuss this with your doctor before Ramadan so they can set a safe plan or decide whether fasting is suitable for you.

### Do asthma inhalers, eye drops, and injections break the fast?

This is both a religious and a medical question. Ask a reliable religious authority or the imam about the ruling, and ask your doctor whether the medication can be moved to after iftar or whether an alternative can be chosen. Do not leave an essential medication or delay it without advice, especially if you have an asthma attack.

### How do I avoid dehydration if I take a diuretic?

Do not stop the diuretic or double it on your own. Ask your doctor about the most suitable timing, drink water at intervals between iftar and suhoor, and watch for intense thirst, scanty dark urine, and dizziness. If these signs appear, see your doctor promptly, and go to the emergency room if you faint or become confused.

### What is the most important thing a person with diabetes should know about their medications during Ramadan?

Discuss fasting with your healthcare team before Ramadan, as you may need to change the type or timing of your medication or your insulin dose. Check your blood sugar more often, and break your fast immediately if it drops below 4 mmol/l or rises above the limit set by your doctor. If you are not confident in your plan, it is safest not to fast.
