(Atypical Hyperplasia)

Endometrial hyperplasia is an overgrowth of the cells lining the inside of the uterus [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>), and it is often linked to high estrogen that is not balanced by progesterone. It is not cancer in itself, but the type with atypical cells (atypia) is the most strongly linked to cancer developing later if it is neglected. Diagnosis relies on taking a tissue sample (biopsy), and treatment often involves progestogen hormones or surgery, depending on the type, the woman's age, and whether she wants to become pregnant.

## What Is Endometrial Hyperplasia?

Endometrial hyperplasia (Endometrial Hyperplasia) is a condition in which the cells of the uterine lining grow excessively [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). This lining is the layer that thickens each month in preparation for pregnancy, and then sheds with the menstrual period if no pregnancy occurs. When this balance is disrupted, the lining becomes thicker than usual.

The condition is one of the uterine disorders that may be benign (non-cancerous) or malignant [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). The overgrowth itself is a benign condition, but it is taken seriously because some types can pave the way for endometrial cancer if they are not diagnosed and monitored. To learn more about the [Uterus](<https://clinicsjo.com/en/glossary/uterus>) and its structure, you can review the glossary entry.

It is important not to confuse it with conditions that have similar names, such as [endometriosis](<https://clinicsjo.com/en/condition/endometriosis>), in which tissue resembling the uterine lining grows in other places in the body [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). In hyperplasia, the overgrowth occurs inside the uterus itself.

## Does Endometrial Hyperplasia Turn Into Cancer?

The short answer: not necessarily. Many cases never turn into cancer, especially when they are diagnosed early and followed up regularly. However, the risk varies considerably depending on the type of hyperplasia, which is why the doctor takes care to determine the exact type from the biopsy result before choosing a treatment.

### Endometrial Hyperplasia Without Atypical Cells

In this type, the proliferating cells look close to normal under the microscope. The risk of it turning into cancer is much lower. Treatment is usually hormonal, along with addressing the underlying trigger, such as excess weight or ovulation disorders, followed by regular monitoring.

### Endometrial Hyperplasia With Atypical Cells (Atypical Hyperplasia)

Here, the cells show changes in shape and organization. This type is considered a precancerous condition. The risk is clearly higher, and a cancerous focus that has not yet been detected may be present alongside it. For this reason, doctors often recommend more definitive options, most notably removal of the uterus (hysterectomy) when there is no desire for pregnancy.

This explains what general guidelines state: a woman who has had her uterus removed cannot develop uterine cancer [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). The precise risk for each woman is determined by her doctor based on the histopathology result and her general health.

To understand what a cancer diagnosis itself means and the steps involved, read the article Endometrial Cancer: Risk Factors, Early Symptoms, and Treatment Options and the page on [endometrial cancer](<https://clinicsjo.com/en/condition/endometrial-cancer>).

## What Are the Causes and Risk Factors of Endometrial Hyperplasia?

The largest common factor is exposure to high estrogen, or estrogen not balanced by progesterone. Sources state that high estrogen is among the most important factors increasing the likelihood of uterine cancer [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>), and it is also a known driver of excessive lining growth. Medical sources note that hormonal imbalances are among the general causes of uterine disorders [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>).

Conditions that may raise estrogen levels, according to the same source [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>), include:

- Excess body weight.

- Use of certain types of hormone replacement therapy (HRT).

- Never having given birth.

- Polycystic ovary syndrome (PCOS).

- Menopause after age 55.

Sources also list other factors associated with an increased likelihood of uterine cancer, including diabetes, a family history of colon, ovarian, or uterine cancer, Lynch syndrome (a hereditary condition), the use of medications such as tamoxifen, and pelvic radiotherapy [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>).

Factor | Why It Matters |

Excess body weight | Linked to high estrogen levels [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>), and it is one of the modifiable factors. |

Polycystic ovary syndrome | Listed among conditions that raise estrogen [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). When ovulation becomes less frequent, the progesterone that normally balances estrogen usually decreases. |

Hormone replacement therapy | Some types raise estrogen [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). Discuss the appropriate type with your doctor, and do not change it on your own. |

Tamoxifen | A medication used to treat breast cancer, and one of the drugs linked to an increased likelihood of uterine cancer [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). Do not stop it on your own, and tell your doctor about any unusual bleeding. |

Diabetes and family history | Both increase the likelihood of uterine cancer [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>), so they are important when assessing risk. |

Having one or more of these factors does not mean you will develop hyperplasia, and some women develop it without any clear factor. The purpose of knowing these factors is to pay attention to symptoms and modify what can be modified.

## Which Symptoms Warrant an Evaluation?

The first sign of a uterine problem is often abnormal bleeding [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). It may appear as:

- Bleeding between menstrual periods.

- Very heavy bleeding.

- A period that lasts longer than usual.

- Bleeding after sexual intercourse.

This may be accompanied by pelvic pain [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>).

These symptoms are not specific to hyperplasia, as there are many causes: thyroid disorders, uterine fibroids, polyps, infections, and pregnancy [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). Therefore, do not guess the cause on your own. Heavy bleeding, for example, may be related to [uterine fibroids, which have specific treatment options](<https://clinicsjo.com/en/service/uterine-fibroids-treatment>), or to a condition such as endometrial adenocarcinoma. Distinguishing between them requires a medical examination.

General health guidance emphasizes the importance of reporting any symptoms related to uterine cancer to a doctor [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). This applies particularly to bleeding after menopause, which is not considered normal and always warrants evaluation.

## How Is Endometrial Hyperplasia Diagnosed?

The doctor starts by asking about your symptoms and medical history, then performs a pelvic examination, and may order blood tests, imaging, or a biopsy [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). Because a clinical examination cannot reveal the type of hyperplasia, the biopsy is the decisive step.

1. **Pelvic ultrasound:** Measures the thickness of the lining and detects other causes such as uterine fibroids [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>).

2. **Endometrial biopsy:** A small sample is taken and examined under the microscope to determine whether hyperplasia is present, its type, and whether atypical or cancerous cells are present.

3. **Hysteroscopy:** Allows direct visualization inside the uterus and targeted sampling [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). Read the explanation of [Hysteroscopy](<https://clinicsjo.com/en/glossary/hysteroscopy>).

4. **Dilation and curettage (D&C):** A procedure included among the diagnostic and treatment methods for uterine disorders [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>), used when a more thorough sample is needed.

The doctor chooses the appropriate procedure based on your age, symptoms, and what imaging has shown. If the biopsy result is inconclusive, or symptoms persist despite a normal result, another procedure may be requested. This is common and does not necessarily mean something serious is present.

## What Are the Treatment Options for Endometrial Hyperplasia?

Treatment depends on the cause [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>), and may include hormonal therapy or surgery [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>). The choice is determined by several factors:

- Whether atypical cells are present or not.

- The woman's age and how close she is to menopause.

- Her wish to become pregnant in the future.

- Her overall health.

Option | When It Is Usually Considered | Notes |

Addressing the cause and modifying lifestyle | With almost all types | Reducing excess weight, reviewing hormone therapy, and controlling diabetes all reduce estrogen stimulation [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). |

Progestogen therapy | Usually for hyperplasia without atypical cells, and may be considered in selected cases with atypical cells when pregnancy is desired | Balances the effect of estrogen on the lining. It is given as tablets or through a hormonal intrauterine device (IUD). The doctor determines the dose and duration, and it should not be started or stopped without supervision. |

Observation with biopsy follow-up | Selected low-risk cases | Biopsy is repeated at intervals set by the doctor to confirm the lining's response. |

Hysterectomy | Atypical cells when fertility is not required, or when hormonal therapy fails or the condition recurs | Eliminates the risk of uterine cancer since the uterus is removed [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). |

### Progestogen Hormone Therapy

Progestogen hormones work to suppress the growth of the lining and compensate for the absence of natural progesterone. They come in different forms, and the choice depends on your condition, preference, and tolerance of side effects. A known side effect is light, intermittent bleeding or changes in the menstrual pattern. Tell your doctor about what you notice rather than stopping the medication.

Do not buy hormonal medications or use leftover prescriptions without consulting a doctor. In Jordan, medications are regulated by the Jordan Food and Drug Administration (JFDA), and the pharmacist can explain how to use the medication as prescribed by your doctor.

### Surgery

When atypical cells are present and there is no desire for pregnancy, hysterectomy is often recommended because it permanently removes the source of risk [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). Surgery may also be proposed if hormonal treatment has not worked or the condition has recurred. The surgical approach and the possibility of preserving the ovaries are discussed with the doctor based on age and condition. For women who wish to become pregnant, a special plan is developed under the supervision of an obstetrics and gynecology specialist, with closer monitoring, because preserving the uterus means accepting a higher risk of recurrence.

## How Is Follow-Up Done, and How Can You Reduce the Risk of Recurrence?

Treatment does not end when the medication does. Women often need follow-up biopsies at intervals set by the doctor to confirm that the lining has returned to normal, followed by regular check-ups. If the lining does not respond as it should, the doctor adjusts the plan.

General guidance to reduce the risk of uterine cancer includes the following [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>):

- Maintaining a healthy weight.

- Regular physical activity.

- Following a balanced, healthy diet.

- Discussing the appropriate type of hormone replacement therapy with your doctor before starting it.

Sources state that uterine cancer cannot always be prevented, but these steps reduce the likelihood [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). If you have Lynch syndrome, your care team will develop a special plan to reduce the risk [\[1\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S1>). Keep follow-up appointments even if symptoms have disappeared, since the lining may still need monitoring even when symptoms improve.

## When Should You See a Doctor, and When Should You Go to the Emergency Room?

See a gynecologist and obstetrician as soon as possible if you notice:

- Bleeding between periods, or a period that is heavier or longer than usual [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>).

- Bleeding after sexual intercourse [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>).

- Any vaginal bleeding after menopause.

- Persistent pelvic pain [\[2\]](<https://clinicsjo.com/en/articles/endometrial-hyperplasia-cancer-risk-treatment#ref-S2>).

- Bleeding that returns during or after progestogen therapy.

Call 911 or go to the nearest emergency department immediately in these cases:

- Heavy bleeding that soaks one or more pads within an hour, repeatedly.

- Dizziness, fainting, or severe palpitations along with bleeding.

- Severe, sudden pelvic pain.

- Fever with bleeding or pain.

These signs require urgent evaluation regardless of any previous diagnosis.

## How Can You Find the Right Doctor or Clinic in Jordan?

The appropriate specialty for evaluating endometrial hyperplasia is obstetrics and gynecology. Start from the [Obstetrics and Gynecology doctors in Jordan](<https://clinicsjo.com/en/specialty/obstetrics-gynecology>) page, then narrow your search by governorate, such as [obstetrics and gynecology in Amman](<https://clinicsjo.com/en/specialty/obstetrics-gynecology/amman>), [Irbid](<https://clinicsjo.com/en/specialty/obstetrics-gynecology/irbid>), or [Zarqa](<https://clinicsjo.com/en/specialty/obstetrics-gynecology/zarqa>), or any other governorates available in the directory.

When browsing doctor profiles, pay attention to:

- The specialty and the scope of practice listed.

- Availability of imaging, biopsy, or hysteroscopy services at the clinic, or referral to a suitable center.

- The clinic's location, working hours, and contact or booking methods as provided in the profile.

The ClinicsJo directory provides information on doctors and clinics to facilitate searching and contact. It does not provide online consultations or prescriptions, and it does not rank doctors by medical quality. Treatment decisions are made between the patient and her doctor.

Useful questions to ask at your appointment:

- What type of hyperplasia do I have, and are atypical cells present?

- What options are available for my condition, and why do you prefer one over the others?

- How often will I need follow-up biopsies, and which signs should prompt me to contact you before the appointment?

- How will the treatment affect my chances of becoming pregnant later?

If you have general questions before your appointment, the short answer to [what endometrial hyperplasia means and whether it is cancer](<https://clinicsjo.com/en/faq/ma-dha-taeni-halat-tadakhum-bitanat-al-rahem-wa-hal-hiya-saratan>) may help. Write down your symptoms, the dates of your periods, and the medications you are taking before your visit, as this helps the doctor assess your condition accurately.

This article is for health education and does not replace a medical examination or a doctor's diagnosis. Do not start, stop, or change any hormonal medication without consulting your doctor or pharmacist.

## Frequently asked questions

### Is endometrial hyperplasia cancer?

No, endometrial hyperplasia is not cancer. It is an overgrowth of the cells lining the inside of the uterus. However, some types, especially those with atypical cells, may develop into cancer if they are not treated and monitored. For this reason, a biopsy determines the type of hyperplasia, and the doctor builds the treatment and follow-up plan on its result.

### What is the difference between hyperplasia with atypical cells and without them?

In the type without atypical cells, the cells look close to normal, the risk of it turning into cancer is lower, and it is usually treated with hormones and monitoring. With atypical cells, the changes are clear under the microscope and it is considered a precancerous condition, so the doctor may recommend more definitive options, such as hysterectomy, when there is no desire for pregnancy.

### What are the symptoms of endometrial hyperplasia?

The most prominent symptom is abnormal bleeding, such as bleeding between periods, a very heavy period, or a period longer than usual, or bleeding after sexual intercourse or after menopause. It may be accompanied by pelvic pain. These symptoms have many other causes, so a medical examination and a biopsy are needed to confirm the diagnosis.

### Is a biopsy painful, and is it necessary?

The biopsy is the most accurate way to determine the type of hyperplasia, and a clinical examination or imaging alone is not enough. You may feel cramping or discomfort for a short period, which varies from woman to woman depending on the method used. Ask your doctor about the procedure suitable for you and ways to ease discomfort.

### Can endometrial hyperplasia be treated with medication alone?

In many cases, especially hyperplasia without atypical cells, progestogen hormone therapy along with addressing the cause and biopsy follow-up is sufficient. The doctor determines the type of medication, its dose, and its duration, and it should not be started or stopped without her supervision. Atypical cells may require surgery.

### Does endometrial hyperplasia affect pregnancy?

Hyperplasia may be linked to ovulation disorders, as in polycystic ovary syndrome, which may make pregnancy more difficult. If you wish to become pregnant, tell your doctor early, as she can develop a plan that preserves the uterus with closer monitoring when needed, rather than removal. The decision is based on the type of hyperplasia, your age, and your general health.
