(<https://clinicsjo.com/ar/faq/how-long-does-the-postpartum-period-last>)

Postpartum warning signs are symptoms that require calling 911 or going to the nearest emergency room without delay. The most important are: heavy bleeding that does not slow down, severe headache with blurred vision, fever with pain or foul-smelling discharge, shortness of breath or chest pain, seizures, and thoughts of harming yourself or your baby. Fatigue and light bleeding that gradually decreases are normal signs of recovery.

## What are the postpartum warning signs, and how do I tell them apart from normal recovery?

The first weeks after birth bring many physical and emotional changes, and most of them are normal. However, some complications can develop quickly. Knowing the postpartum warning signs helps you and the people around you act in time.

The World Health Organization (WHO) notes that the major complications responsible for about 75% of maternal deaths are severe bleeding (mostly after birth), infection (usually after birth), and high blood pressure during pregnancy (pre-eclampsia and eclampsia) [\[2\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S2>). The WHO also explains that most of these complications can be prevented or treated when skilled healthcare is available at the right time [\[2\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S2>).

If you are looking for a quick answer, also see our page on [warning signs that require a doctor's review after birth](<https://clinicsjo.com/en/faq/postpartum-warning-signs-after-birth>).

## What is considered normal in the first weeks after birth?

MedlinePlus explains that you may have spotting or bleeding similar to a menstrual period, on and off, for up to six weeks. Swelling in the legs and feet, constipation, cramps similar to menstrual cramps, breast fullness and tenderness, and milk leakage even if you are not breastfeeding may also occur [\[3\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S3>).

You may also feel sad or experience what is called "baby blues" [\[3\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S3>). For more information about how long this period lasts, read [How long does the postpartum period last?](<https://clinicsjo.com/en/faq/how-long-does-the-postpartum-period-last>)

Usually normal | Needs medical evaluation |

Bleeding or spotting that comes and goes and gradually decreases | Bleeding that increases or becomes heavy again after it had lessened |

Mild swelling in the legs and feet | Sudden swelling in the face and hands, or pain and swelling in one leg |

Mild cramps similar to menstrual cramps | Severe or increasing abdominal pain |

Passing sadness and mood swings | Severe sadness or inability to care for yourself or your baby |

## Postpartum warning signs: when should a mother go to the ER immediately?

If any of the following signs appear, do not wait for your scheduled appointment and do not try home remedies. Call 911 or go to the nearest emergency room.

Sign | What it may indicate | Appropriate action |

Heavy bleeding that continues or increases, with dizziness, paleness, fainting, or palpitations | Postpartum hemorrhage | Emergency immediately |

Severe headache that does not go away, blurred vision or flashing lights, upper abdominal pain, seizures | High blood pressure or pre-eclampsia | Emergency immediately |

Sudden shortness of breath, chest pain, or pain, swelling, and redness in one leg | Blood clot | Emergency immediately |

Fever with chills, lower abdominal pain, foul-smelling discharge, rapid pulse, or confusion | Infection or sepsis | Emergency immediately |

Thoughts of harming yourself or your baby | Acute mental health crisis | 911 or nearest emergency room |

Redness or swelling of the wound, pus discharge, or opening of the stitches | Infection of the cesarean or stitched wound | See a doctor the same day, and go to the emergency room if accompanied by fever or worsening |

The only time it is safe to wait is when none of these symptoms are present. If you are unsure, it is safest to be evaluated by a doctor. You can also read our article on childbirth complications, management, and medical interventions.

## Is bleeding after birth and passing clots always dangerous?

Not always. Intermittent bleeding that lightens over the days is expected, as mentioned above [\[3\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S3>). The danger lies in heavy bleeding that does not slow down, bleeding accompanied by dizziness, paleness, or palpitations, or passing large clots.

The WHO states that severe bleeding after birth can kill a healthy woman within hours if she does not receive care [\[2\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S2>). It adds that giving injectable uterotonic drugs (oxytocics) immediately after birth effectively reduces the risk of bleeding [\[2\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S2>). This is a procedure decided by the hospital medical team, and no mother should treat bleeding on her own.

### What should you do if you suspect severe bleeding?

- Lie down and ask for help immediately, and call 911.

- Do not wait to see whether the bleeding improves on its own.

- Note how many pads become soaked and the time, so you can inform the doctor.

For more on the causes and evaluation, read postpartum bleeding: causes and signs that require emergency care or the [postpartum hemorrhage](<https://clinicsjo.com/en/condition/postpartum-hemorrhage>) page.

### Blood clots in the leg or lung

The body's tendency to form blood clots increases after pregnancy and childbirth. Therefore, the following symptoms must be taken seriously:

- Pain, swelling, and redness in one leg.

- Sudden shortness of breath.

- Chest pain.

- Cough with blood.

These symptoms require immediate emergency care, because a clot can travel to the lung.

## Can blood pressure rise or pre-eclampsia occur after birth?

Yes, blood pressure problems may appear in the days following birth, even if the pregnancy was uneventful. The WHO confirms that pre-eclampsia must be detected and treated before it progresses to seizures (eclampsia) and life-threatening complications, and that drugs such as magnesium sulfate reduce the risk of eclampsia [\[2\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S2>). This treatment is given by a doctor inside the hospital.

NHS guidance states that blood pressure is measured at the postnatal check if there were problems during pregnancy or immediately after birth [\[1\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S1>). However, severe symptoms do not wait for that appointment.

### Symptoms that require emergency care

- Severe headache that does not respond to rest.

- Blurred vision or seeing flashing lights.

- Pain in the upper abdomen under the ribs.

- Sudden swelling in the face or hands.

- Seizures or loss of consciousness.

If you had high blood pressure during pregnancy, read high blood pressure in pregnancy: when it becomes pre-eclampsia. You can also have your blood pressure checked at clinics offering [vital signs assessment](<https://clinicsjo.com/en/service/vital-signs-assessment>).

## How do I know if I have an infection or an inflamed cesarean wound?

The WHO notes that postpartum infection can be prevented with good hygiene, and that early recognition of its signs and timely treatment are critical [\[2\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S2>). Alarming signs include:

- Fever or chills.

- Increasing pain in the lower abdomen.

- Foul-smelling vaginal discharge.

- Rapid heartbeat.

- Feeling extremely unwell or confused.

If fever occurs together with rapid deterioration in your condition, it may be sepsis and requires immediate emergency care.

### Cesarean incision or birth stitches

NHS notes that you may be offered an examination to check that the stitches have healed if you had an episiotomy or a cesarean section [\[1\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S1>). However, do not wait for this examination if you notice:

- Redness or swelling that increases around the wound.

- Pus or foul-smelling fluid discharge.

- Pain that worsens instead of improving.

- The edges of the wound opening.

Do not press on the wound, do not apply home remedies, and do not start any antibiotic without a prescription from a doctor. NHS advises telling your doctor about any pain in the area between the vagina and the anus (perineum) [\[1\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S1>).

## When does sadness after birth become depression that needs help?

Feeling sad and crying after birth is common, and it is called "baby blues." However, if you are extremely sad or unable to care for yourself or your baby, it may be postpartum depression, which is a serious condition [\[3\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S3>).

NHS encourages you to tell your doctor if you feel sad, anxious, or have any concerns about your mental health. It emphasizes that caring for a baby can feel very exhausting, that you do not have to suffer alone or appear strong, and that this is not a sign that you are a bad mother [\[1\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S1>).

### When does it become an emergency?

If you have thoughts of harming yourself or your baby, or you feel you cannot keep yourself safe, call 911 or go to the nearest emergency room, and tell someone close to you immediately. This is a treatable health condition, not a weakness. Read about [postpartum depression](<https://clinicsjo.com/en/condition/postpartum-depression>) and about suicide warning signs and how to ask for urgent help.

## What should I do as soon as one of these signs appears?

1. Call 911, or ask a family member to call. Do not drive yourself.

2. Tell the paramedic or emergency staff the date and type of birth (vaginal or cesarean) and any problem that occurred during pregnancy.

3. Bring your hospital discharge report and the names of any medications you are taking.

4. Arrange for a trusted person to care for the baby, or bring the baby with you if no one is available.

5. Do not wait for your routine follow-up visit if symptoms are severe.

As for routine follow-up, NHS recommends a postnatal check between the sixth and eighth week, and you can request an appointment yourself, especially if you have concerns [\[1\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S1>). In Jordan, health centers and mother and child care centers affiliated with the Ministry of Health are available in the governorates, where you can inquire about postpartum follow-up. This does not replace emergency care for any warning sign.

Remember that knowing these signs gives you and your family the chance to act early. It is also good to share this list with your husband, your mother, or whoever cares for you in the first days. NHS advises writing down your questions before the appointment [\[1\]](<https://clinicsjo.com/en/articles/postpartum-warning-signs-when-to-go-to-emergency#ref-S1>).

## How do I find the right doctor or clinic to follow up on postpartum warning signs in Jordan?

For postpartum follow-up, search for the obstetrics and gynecology specialty through the [obstetrics and gynecology doctors in Jordan](<https://clinicsjo.com/en/specialty/obstetrics-gynecology>) page, then narrow your search by governorate:

- [Obstetrics and gynecology in Amman](<https://clinicsjo.com/en/specialty/obstetrics-gynecology/amman>)

- [Obstetrics and gynecology in Irbid](<https://clinicsjo.com/en/specialty/obstetrics-gynecology/irbid>)

- [Obstetrics and gynecology in Zarqa](<https://clinicsjo.com/en/specialty/obstetrics-gynecology/zarqa>)

- [Obstetrics and gynecology in Aqaba](<https://clinicsjo.com/en/specialty/obstetrics-gynecology/aqaba>)

### What should you check in a doctor's profile?

- The listed specialty and qualifications.

- The clinic's location and ease of access.

- Working hours and the contact or booking methods available in the profile.

- Whether postpartum follow-up takes place at the clinic or whether you will need a hospital.

The directory presents information about doctors and clinics so you can make your decision. It does not provide prescriptions or medical consultations, and it does not rank doctors by medical quality. No booking replaces emergency care when a warning sign appears.

## Frequently asked questions

### What are the most important postpartum warning signs that require emergency care?

Several signs require immediate emergency care: heavy bleeding that does not slow down, severe headache with blurred vision, seizures, shortness of breath or chest pain, fever with lower abdominal pain or foul-smelling discharge, and thoughts of harming yourself or your baby. Call 911 or go to the nearest emergency room without waiting.

### Is bleeding after birth normal, and how long does it last?

Yes, spotting or bleeding similar to a menstrual period may occur on and off for up to six weeks, and it should gradually decrease. However, heavy bleeding that increases, or that is accompanied by dizziness, paleness, palpitations, or large clots, is a warning sign, and you need emergency care immediately.

### Can pre-eclampsia occur after birth?

Yes, blood pressure problems may appear in the days after birth, even if the mother did not have them during pregnancy. Alarming symptoms include severe headache, blurred vision, upper abdominal pain, sudden swelling in the face and hands, and seizures. These symptoms require immediate emergency care.

### What are the signs of an infected cesarean wound?

Signs include increasing redness or swelling of the wound, pus or foul-smelling fluid discharge, pain that worsens instead of improving, and opening of the wound edges, especially with fever. See a doctor the same day, or go to the emergency room if the condition worsens. Do not apply home remedies or start an antibiotic without a prescription.

### What is the difference between baby blues and postpartum depression?

Baby blues (sadness and mood swings) are common after birth. However, if you are extremely sad or unable to care for yourself or your baby, it may be postpartum depression, a serious but treatable condition. Tell your doctor early, and if you have thoughts of harming yourself, call 911.

### Should I wait for the six-week check if I have worrying symptoms?

No. The postnatal check is usually between the sixth and eighth week, but you can request an appointment earlier if you have concerns. Acute warning signs such as heavy bleeding, severe headache, shortness of breath, or fever with a worsening condition require immediate emergency care.
