Adolescent PCOS

## Summary

Adolescent polycystic ovary syndrome is a hormonal and metabolic disorder requiring puberty-appropriate diagnostic criteria. Irregular periods or an ultrasound showing multiple follicles alone do not confirm it.

## Overview

In 2026, the broader name polyendocrine metabolic ovarian syndrome, or PMOS, was introduced. PCOS remains familiar terminology. The name change does not require ovarian cysts or justify changing a diagnosis independently.

## Causes

Inherited susceptibility interacts with androgen and metabolic disturbances, sometimes including insulin resistance. The condition is not the teenager’s fault and does not require excess body weight.

## Symptoms

Persistent widely spaced cycles, excess hair or severe acne may occur, although some changes are normal early in puberty. Interpretation considers years since the first period and symptom impact.

## Risk factors

Family history increases likelihood, and insulin resistance can worsen some features. Weight or family history alone does not diagnose the condition. Review continues during transition to adult care.

## Diagnosis

Diagnosis requires age-defined persistent cycle irregularity and hyperandrogenism after excluding other causes. Ultrasound and anti-Müllerian hormone are not used to confirm adolescent diagnosis. Those with incomplete criteria can be followed as at risk.

## Treatment

Care targets menstrual, skin or metabolic concerns. Selected hormonal treatments or metformin may be combined with balanced nutrition and activity. Antiandrogen or hormonal medicines require assessment and monitoring rather than unsupervised use.

## Complications

Symptoms may affect confidence and mood, and some cases involve metabolic risks or later ovulatory difficulty. Diagnosis does not predict inevitable infertility; early care addresses present health needs.

## Prevention

No prevention method is guaranteed. Sleep, activity, adequate nutrition and positive body-image support are valuable. Avoid restrictive diets and weight stigma; metabolic and mental-health monitoring is individualized.

## When to seek care

Seek evaluation for markedly infrequent or absent periods or distressing hair growth or acne. Rapid voice change or severe androgenic changes need prompt assessment for other causes.
