## Summary

Atherosclerosis is a slow buildup of fatty and calcium deposits inside artery walls, narrowing blood vessels and reducing their elasticity, increasing the risk of heart attacks and strokes. Treatment involves medications, lifestyle changes, and medical interventions based on severity.

## Overview

Atherosclerosis is a chronic disease characterized by the gradual buildup of plaque—a mixture of fats, cholesterol, calcium, and inflammatory substances—inside the inner layer of artery walls. This accumulation narrows the arterial lumen, obstructing blood flow and reducing the elasticity of vessel walls, leading to elevated blood pressure and increased risk of sudden clot formation.

Atherosclerosis is the leading cause of cardiovascular disease worldwide, responsible for millions of deaths annually. In the Middle East and Jordan, incidence rates are rising due to increasing obesity, diabetes, hypertension, and sedentary lifestyles. The disease often progresses silently without symptoms in early stages, but as it advances, it can cause heart attacks, strokes, and peripheral arterial disease.

The good news is that atherosclerosis is preventable, can be slowed, and may even be partially reversed through early detection, appropriate treatment, and risk factor modification. Starting treatment early significantly reduces the risk of acute complications.

## Causes

- **High Cholesterol (Hypercholesterolemia):** Elevated levels of LDL cholesterol promote plaque accumulation in artery walls, especially when the artery lining is damaged.

- **High Blood Pressure (Hypertension):** Elevated pressure causes continuous stress and injury to artery walls, facilitating plaque infiltration and buildup.

- **Smoking:** Tobacco smoke damages the arterial lining, increases inflammation and clotting, and reduces protective HDL cholesterol.

- **Diabetes:** High blood sugar causes inflammation and vessel injury, significantly accelerating plaque accumulation.

- **Obesity and Overweight:** Increases inflammation, cholesterol, diabetes risk, and directly raises blood pressure.

- **Sedentary Lifestyle:** Lack of exercise weakens heart and blood vessels, elevates cholesterol, and contributes to obesity.

- **Poor Diet:** High intake of saturated fats, sugars, and salt raises cholesterol, blood sugar, and blood pressure.

- **Chronic Inflammation:** Some inflammatory diseases (such as rheumatoid arthritis) increase atherosclerosis risk.

## Symptoms

In early stages of atherosclerosis, there may be no symptoms at all, making the disease particularly dangerous—you may discover it only on routine screening. As the disease progresses and narrowing exceeds 70%, symptoms begin to appear and vary by affected artery, gender, and age. Women may experience atypical symptoms (general fatigue, shortness of breath, exhaustion) different from the classic chest pain men experience.

- **Chest Pain (Angina):** Pressure or tightness in the chest worsening with physical exertion or stress, relieved by rest. May radiate to the arm, jaw, or neck.

- **Shortness of Breath:** Difficulty breathing, especially with exertion or lying down, due to weakened heart pumping ability.

- **Fatigue and Exhaustion:** Unexplained tiredness even with normal activities, especially in women and elderly.

- **General Weakness:** Feeling weak and debilitated, especially in limbs.

- **Heart Palpitations:** Sensation of rapid, pounding, or irregular heartbeats.

- **Dizziness and Fainting:** May occur with reduced brain blood flow or sudden blood pressure drops.

- **Leg Pain (Claudication):** When peripheral arteries are affected, pain in the legs during walking relieved by rest.

- **Cold Limbs and Color Changes:** Extremities (especially fingers) may become cold, swollen, or bluish due to poor blood circulation.

- **Sexual Problems in Men:** Erectile dysfunction may be an early sign of atherosclerosis.

- **Transient Ischemic Attack (TIA):** Sudden weakness in speech, vision, or facial movement lasting minutes to hours.

## Risk factors

- **Age and Gender:** Men over 45 and women over 55 (post-menopausal) are at higher risk.

- **Family History:** First-degree relatives with early heart disease (before 55 in men, 65 in women) significantly increase risk.

- **Total, LDL, and Low HDL Cholesterol:** Unhealthy cholesterol ratios indicate high risk.

- **Uncontrolled High Blood Pressure:** BP > 140/90 mmHg is a major risk factor.

- **Diabetes and Elevated Fasting Glucose:** Both types of diabetes greatly increase risk; fasting glucose 100-125 mg/dL is also concerning.

- **Smoking and Secondhand Smoke Exposure:** Even light smoking or passive exposure increases risk.

- **Obesity (BMI > 30):** Central obesity (abdominal fat) is particularly dangerous.

- **Psychological Stress and Depression:** Chronic stress and depression increase inflammation and clotting.

- **Early Menopause in Women:** Loss of estrogen protection raises heart disease risk.

- **Elevated Uric Acid or Lipoprotein(a):** Other blood markers indicating increased risk.

## Diagnosis

Early diagnosis of atherosclerosis is essential to prevent complications. It relies on clinical examination, blood tests, and imaging. Clinical assessment includes medical history and risk factor evaluation, followed by auscultation for arterial bruits. Blood tests measure cholesterol, glucose, and inflammation markers to assess risk level. Advanced imaging (echocardiography, nuclear scans, angiography) reveals the degree of narrowing and need for intervention.

- **Lipid Panel Blood Test:** Measures total cholesterol (target < 200 mg/dL), LDL (< 100), HDL (> 40 for men, > 50 for women), and triglycerides (< 150). Repeated every 4-6 years in healthy adults, more frequently in those with risk factors.

- **Fasting Glucose and Glucose Tolerance Test:** Normal fasting glucose < 100 mg/dL; 2-hour post-challenge < 140. HbA1c < 5.7% is normal.

- **High-Sensitivity CRP Test:** Measures general inflammation. Elevated levels (> 3 mg/L) indicate higher risk.

- **Electrocardiogram (ECG):** Shows history of prior heart attacks or current ischemia.

- **Stress Test:** Performed on a treadmill or with medication to reproduce symptoms and assess heart tolerance.

- **Carotid Ultrasound:** Measures artery wall thickness (intima-media) and detects plaques. Good, safe screening tool.

- **CT Angiography:** Fast, relatively safe imaging showing degree of artery narrowing with high accuracy without catheterization.

- **Nuclear Stress Test (Myocardial Perfusion Imaging):** Measures blood flow to heart muscle under stress and rest.

- **Magnetic Resonance Imaging (MRI):** Provides detailed images of vessel walls and plaques for precise assessment.

- **Coronary Angiography (Cardiac Catheterization):** Gold standard when severe narrowing is suspected. A thin tube is advanced through an artery and contrast injected to visualize coronary vessels precisely. May be combined with immediate angioplasty.

## Treatment

### Medications

- **Statins:** Reduce bad cholesterol production and stabilize plaques. Examples: atorvastatin 20-80 mg daily, rosuvastatin 10-40 mg. Used for all atherosclerosis patients or those with high risk. Liver and muscle function must be checked before starting.

- **ACE Inhibitors:** Dilate arteries, reduce blood pressure and heart workload. Examples: lisinopril 10 mg daily, enalapril 20 mg. Especially useful in patients with hypertension or heart failure.

- **Beta-Blockers:** Reduce heart rate, blood pressure, and improve heart efficiency. Examples: metoprolol 50-100 mg twice daily, bisoprolol 5-10 mg. Particularly beneficial after heart attacks.

- **Aldosterone Antagonists:** Such as spironolactone, reduce salt and water retention and improve heart function. Used in severe heart failure.

- **SGLT2 Inhibitors:** Newer diabetes drugs that protect the heart and kidneys. Examples: empagliflozin 10 mg, dapagliflozin 10 mg.

- **Aspirin:** Blood thinner reducing clotting risk, preventing attacks and strokes. Standard dose 75-100 mg daily for secondary prevention. May not be recommended for primary prevention in healthy adults.

- **P2Y12 Inhibitors:** Such as clopidogrel (Plavix) 75 mg daily, ticagrelor. Used after acute clotting or vascular interventions.

- **Nitrates:** Dilate arteries, relieve chest pain. Examples: isosorbide dinitrate 20-60 mg; or fast-acting nitroglycerin spray (0.3-0.6 mg) under the tongue during anginal attacks.

- **Additional Antihypertensives:** Calcium channel blockers (amlodipine 5-10 mg) or diuretics (hydrochlorothiazide 25 mg) may be added if blood pressure remains uncontrolled.

### Procedures and Surgery

- **Percutaneous Coronary Intervention (PCI) / Angioplasty with Stenting:** Minimally invasive procedure: a thin catheter is threaded through the femoral or radial artery and a balloon is inflated to widen the narrowed artery. Often a metal stent (bare or drug-coated) is placed to keep it open. Used for narrowing > 70% or symptomatic disease.

- **Coronary Artery Bypass Graft (CABG):** Open-heart surgery where a healthy blood vessel (usually from the leg or arm) is grafted to bypass the blockage. Used for multiple or complex narrowings unsuitable for catheterization.

- **Endarterectomy:** Surgery for severe narrowing in specific arteries (e.g., carotid artery). The surgeon removes plaque from the artery wall.

- **Extracorporeal Shock Wave Therapy (ESWT):** Newer technique to improve blood flow, still under investigation.

### Lifestyle Modifications (Cornerstone of Treatment)

- **Quit Smoking Immediately:** This is paramount. Smoking continuously damages vessels. Within a month of quitting, improvement begins; within a year, risk drops to 50%.

- **Follow a Heart-Healthy Diet (Mediterranean or DASH Diet):** Increase fruits, vegetables, whole grains, fish, and nuts. Reduce saturated and trans fats, salt, and sugars. Aim for dietary cholesterol < 200 mg daily.

- **Regular Physical Activity:** 150 minutes of moderate-intensity walking weekly (or 75 minutes vigorous) plus twice-weekly muscle-strengthening exercises. Reduces weight, blood pressure, glucose, and cholesterol.

- **Maintain Healthy Weight (BMI 18.5-24.9):** Losing 5-10% of excess weight significantly improves all markers.

- **Manage Psychological Stress:** Relaxation and mindfulness techniques, cognitive behavioral therapy (CBT), may reduce inflammation. Exercise and social engagement help.

- **Adequate Sleep Quality:** 7-9 hours of consistent nightly sleep reduces inflammation and improves heart health.

- **Limit Alcohol:** If consumed, moderate intake (maximum 1 unit daily for women, 2 for men).

- **Monitor Blood Pressure, Glucose, and Lipids:** Measure BP weekly with a home device; periodic blood tests every 3-6 months.

## Complications

- **Myocardial Infarction (Heart Attack):** When a plaque fragment detaches and causes a clot suddenly blocking an artery, blood supply to part of the heart is cut off, causing permanent cell death. This life-threatening emergency may result in death or permanent heart weakness.

- **Stroke (Cerebrovascular Accident):** When a clot from plaque blocks a brain artery, part of the brain is damaged, causing paralysis, speech loss, or vision loss. May leave permanent disability.

- **Heart Failure:** Repeated ischemia and clotting gradually weaken the heart muscle and impair its pumping ability, causing fluid accumulation in lungs and limbs.

- **Peripheral Arterial Disease:** When peripheral arteries are affected, blood supply to fingers or entire limbs may be cut off, potentially leading to gangrene or amputation.

- **Pulmonary Embolism:** A clot from deep veins may lodge in a lung artery, suddenly blocking blood flow, causing severe shortness of breath and potential death.

- **Aneurysm:** Plaque-induced vessel wall weakness may cause outpouching and enlargement; rupture results in life-threatening internal bleeding.

- **Renal Complications:** Atherosclerosis of renal arteries may lead to chronic kidney failure requiring dialysis.

## Prevention

- **Quit Smoking and Avoid Secondhand Smoke:** Do not smoke and try to avoid smoking environments. If you smoke, seek medical help or cessation programs (nicotine replacement therapy, counseling).

- **Follow a Balanced, Healthy Diet:** Choose whole grains, fresh fruits and vegetables, fish (especially fatty, omega-3 rich), and nuts. Reduce red and processed meats, saturated fats, salt, sugar, and alcohol.

- **Maintain Healthy Weight:** Exercise regularly (30-minute daily walks or 150 minutes weekly), eat moderate portions. Obesity increases blood pressure, glucose, and cholesterol.

- **Monitor Blood Pressure, Glucose, and Cholesterol:** Check blood pressure regularly (at least yearly, monthly if elevated); have periodic blood tests (every 4-6 years for healthy adults, annually for those with risk factors or chronic diseases).

- **Take Prescribed Medications as Directed:** If your doctor prescribes medications (especially statins or blood pressure drugs), adhere strictly even if you feel better. Non-compliance significantly increases risk.

- **Manage Chronic Conditions (Diabetes, Hypertension):** Keep doctor appointments, attend check-ups, and maintain blood sugar and pressure within target ranges.

- **Reduce Psychological Stress:** Practice mindfulness, yoga, or relaxation 10-20 minutes daily. Good social and family relationships reduce stress.

- **Regular Checkups and Early Detection:** Begin heart screening from age 40-45 (earlier if family history), especially post-menopausal women. Consult your doctor about appropriate screening for your situation.

- **Vaccinations and Prevention of Infections:** Some infections (flu, pneumonia) may worsen heart disease complications, so get recommended vaccinations.

- **Learn First Aid and Recognize Warning Signs:** Learn CPR and recognize symptoms of heart attack and stroke to seek emergency help quickly.
