## Summary

Benign paroxysmal positional vertigo is the most common cause of dizziness — a short, intense spinning sensation triggered by head-position changes, due to tiny crystals moving inside the inner ear. It is not dangerous and is often effectively treated with repositioning maneuvers performed by the doctor.

## Overview

Benign paroxysmal positional vertigo is one of the most common causes of dizziness and vertigo, especially in older adults. It is characterized by short attacks of intense vertigo (a sensation that the surroundings are spinning) triggered by changes in head position, such as lying down, getting up from bed, turning over, or looking up.

The cause of this condition is the movement of tiny calcium crystals (called otoliths) from their normal place in part of the inner ear into the semicircular canals responsible for the sense of balance and motion. When the head moves, these crystals move within the canal and send wrong signals to the brain suggesting an unreal spinning motion, producing vertigo. An attack usually lasts seconds to less than a minute and may be accompanied by nausea and imbalance.

As its name indicates, this vertigo is benign, meaning it is not a sign of a serious brain disease, although it is bothersome and sometimes frightening. The excellent news is that it is among the most treatable causes of dizziness: a specialist can diagnose it with a simple test, then treat it very effectively through canalith repositioning maneuvers (such as the Epley maneuver) that return the crystals to their correct place, and symptoms often disappear after one or several sessions. Nevertheless, any dizziness accompanied by other neurological symptoms warrants evaluation to rule out other causes.

## Causes

- Movement of calcium crystals in the inner ear into the semicircular canals.

- Often without a clear cause, and increases with age.

- Sometimes after a head injury, ear inflammation, or prolonged bed rest.

## Symptoms

- Short, intense vertigo attacks on changing head position.

- A sensation that the surroundings are spinning.

- Nausea and imbalance.

- An attack lasts seconds to less than a minute.

## Risk factors

BPPV becomes more likely after age 50 and is more common in women, although any age can be affected. Head injury and disorders of the inner-ear balance organs can increase risk; osteoporosis may also be associated. Moving the head triggers an existing episode rather than necessarily causing the underlying condition.

## Diagnosis

- Clinical diagnosis through a head-position test (Dix-Hallpike maneuver) to provoke the vertigo and observe eye movement.

- Evaluation to rule out other causes when atypical symptoms are present.

## Treatment

### Canalith repositioning maneuvers

- The Epley and other maneuvers performed by the doctor to return the crystals to place, which are very effective.

### Supportive measures

- Home balance exercises guided by the doctor.

- Anti-nausea medication when needed for a short period.

This content is educational and does not replace medical advice.

## Complications

- Risk of falls and injuries, especially in older adults.

- Anxiety and avoiding movement out of fear of attacks.

- It may recur after a while and need the maneuver repeated.

## Prevention

- There is no proven prevention, but moving carefully reduces fall risk.

- Seeing the doctor when attacks recur to repeat the maneuver.

- Balance exercises for at-risk groups.

## When to seek care

See a doctor for recurrent vertigo attacks with head-position changes. Go to the emergency department if dizziness comes with a sudden severe headache, weakness, numbness, speech difficulty, or double vision, which may indicate neurological causes needing urgent evaluation. **Book an appointment with an ENT specialist or neurologist on ClinicsJo**.
