Chemotherapy-Induced Anemia

## Summary

Anemia during chemotherapy means reduced hemoglobin and oxygen-carrying capacity. Fatigue is common but does not establish the diagnosis; care depends on the cause and symptoms.

## Overview

Anemia is not always iron deficiency. Marrow suppression, bleeding, nutritional deficiencies and kidney disease may overlap, and the intent of cancer treatment affects management.

## Causes

Chemotherapy or cancer involving the marrow can reduce red-cell production. Inflammation and iron, vitamin B12 or folate deficiency are additional possible contributors.

## Symptoms

Symptoms include unusual fatigue, pallor, dizziness, palpitations and breathlessness on exertion. Mild anemia may be silent; heart or lung disease can worsen its impact.

## Risk factors

Marrow-suppressing treatment, blood loss, poor nutrition and kidney impairment increase risk. Anemia present before cancer treatment also needs assessment.

## Diagnosis

A complete blood count measures hemoglobin. Iron studies, vitamin levels, kidney tests or bleeding investigations may follow according to the history and findings.

## Treatment

Treatment may correct a proven deficiency, provide red-cell transfusion or adjust chemotherapy. Erythropoiesis-stimulating agents suit selected cases, generally with noncurative treatment, after discussing clotting risks.

## Complications

Anemia may restrict function, strain the heart and delay treatment. No single hemoglobin value determines transfusion for everyone without considering symptoms and risks.

## Prevention

Balance activity and rest, maintain appropriate nutrition and attend blood tests. Do not start iron or blood-stimulating injections solely for fatigue without identifying the cause.

## When to seek care

Breathlessness at rest, chest pain, fainting or persistent bleeding requires emergency care. Tell the oncology team about worsening dizziness or fatigue that limits daily activities.
