Chemotherapy-Induced Neutropenia

## Summary

Chemotherapy-related neutropenia is a reduction in infection-fighting white blood cells called neutrophils. It may cause no symptoms, but fever during this period requires urgent contact with the oncology team.

## Overview

Infection risk depends on the absolute neutrophil count, the depth of the decrease and its duration, not the total white-cell count alone. Timing varies between treatment regimens.

## Causes

Chemotherapy can suppress bone-marrow production. Cancer, infections or other medicines may also contribute, so every low count should be interpreted in context.

## Symptoms

Possible infection signs include fever, chills, sore throat, cough, painful urination or redness around a catheter. Inflammation may be subtle despite a serious infection.

## Risk factors

Risk depends on regimen intensity, age, other illnesses, nutrition and previous episodes. Growth-factor prevention is not automatically appropriate for everyone receiving chemotherapy.

## Diagnosis

A blood count with differential measures neutrophils. Fever may require cultures and tests for an infection source, without delaying necessary urgent treatment.

## Treatment

Febrile neutropenia requires rapid assessment and appropriate antimicrobial treatment. The team may adjust chemotherapy and use white-cell growth factors for selected patients based on risk.

## Complications

Infection may progress to sepsis, hospitalization or treatment delays. Early detection is more useful than relying on a particular food or supplement to raise counts.

## Prevention

Use good hand, mouth and catheter hygiene, follow food-safety advice and avoid close contact with people who have infections. Do not self-start preventive antibiotics or growth-factor injections.

## When to seek care

Call immediately for a temperature of 38°C or above, or the threshold your team specifies. Do not wait for fever medicine to work. Confusion, breathlessness or fainting warrants emergency care.
