## Summary

Childhood obesity is a multifactorial condition assessed in the context of age, growth and overall health. Supportive family care is essential; specialist programmes or additional treatment may be appropriate according to severity, age and complications.

## Overview

Appearance or weight alone cannot determine a child’s health. BMI is interpreted against age, sex and growth trajectory, with different assessment in infancy. The aim is healthy growth, function, sleep and wellbeing rather than rapid achievement of a target weight.

## Causes

Genes, appetite regulation, sleep, eating patterns, activity and social environment interact. Medicines or medical conditions contribute in some children. Obesity is not explained by poor willpower and does not automatically indicate a hormonal disorder requiring extensive tests.

## Symptoms

Some children have no clear symptoms. Others develop snoring, disturbed sleep, joint pain or difficulty with activity. Discuss bullying, mood and eating behaviours too. Fatigue or school difficulties need assessment rather than automatic attribution to weight.

## Risk factors

Family history, certain medicines, inadequate sleep and limited access to nutritious food or safe activity spaces may contribute. Plans should reflect family resources, routines and the child’s abilities rather than assume the same solution suits everyone.

## Diagnosis

Clinicians review growth charts, diet, movement, sleep, medicines and family history, and assess blood pressure and development. Lipid, glucose, liver or other tests depend on age, severity and symptoms. Fixed adult BMI cut-offs cannot diagnose childhood obesity.

## Treatment

Care centres on a practical family plan with varied food, suitable activity, behavioural support and follow-up. Some children may benefit from slower weight gain while growing taller. Eligible patients may discuss medicines or surgery with specialists; criteria and local availability vary.

## Complications

Risks include type 2 diabetes, abnormal lipids, high blood pressure, fatty liver and sleep apnoea. Bullying can harm mental health and eating behaviour. These complications do not affect every child; clinical assessment guides appropriate follow-up.

## Prevention

Provide water, varied food, regular meals, enjoyable activity and age-appropriate sleep. Make changes together rather than isolating the child with punitive dieting. Reduce sugary drinks and prolonged sitting, coordinating with school and caregivers without stigmatizing the child.

## When to seek care

Arrange assessment for rapid weight gain, growth concerns, persistent snoring or pain limiting movement. Vomiting with marked thirst, frequent urination or unusual drowsiness needs urgent care and should not be explained by weight alone.
