Diabetes Insipidus

## Summary

Diabetes insipidus is a water-balance disorder causing large volumes of dilute urine and thirst. It differs from diabetes mellitus, which involves high blood glucose. Testing identifies the cause and guides safe fluid and treatment plans.

## Overview

It may result from vasopressin deficiency, now also called AVP deficiency, or kidney resistance to the hormone. Pregnancy-related forms exist, while primary excessive drinking can resemble the condition. Frequent bathroom visits alone do not establish excessive urine volume.

## Causes

Vasopressin is made in the hypothalamus and released by the posterior pituitary to conserve water. Surgery, injury or brain disease may impair its release. Kidney resistance can involve genetic causes, medicines such as lithium, electrolyte disturbances or kidney disease.

## Symptoms

Large amounts of pale urine by day and night, thirst, fatigue and disrupted sleep are typical. Infants may show irritability, poor growth or dehydration. Symptoms overlap with high glucose and other conditions; urine appearance cannot establish the diagnosis.

## Risk factors

Risk factors include hypothalamic or pituitary disease or surgery, head injury, certain medicines, and genetic or kidney disorders. Pregnancy changes the assessment of new symptoms. Family history may help but is not required for diagnosis.

## Diagnosis

Evaluation measures urine volume and blood and urine concentration, glucose, sodium and kidney function. Specialist testing or pituitary imaging may be needed. A water-deprivation test, when appropriate, requires clinical monitoring; never attempt water restriction as a home diagnostic test.

## Treatment

Treatment depends on type and cause. Desmopressin can help suitable patients with hormone deficiency; kidney resistance requires cause-specific management and other selected treatments. Sodium and fluids need monitoring. Excess medication or excessive drinking while taking it can cause dangerously low sodium.

## Complications

Water loss can cause dehydration and high sodium, while an unbalanced treatment regimen can lower sodium. Either disturbance may affect the brain. Accurate diagnosis and laboratory follow-up reduce risk without a universal fluid prescription.

## Prevention

Not all causes are preventable. Medication review and follow-up after pituitary surgery support early detection. Carry diagnostic and medication information and agree on a plan for illness or medical fasting. Do not independently stop chronic medicines or impose arbitrary fluid amounts.

## When to seek care

Confusion, seizures, marked drowsiness, fainting or inability to replace fluid losses requires emergency care. During desmopressin treatment, severe headache with nausea or confusion needs urgent sodium assessment and direct instructions about the next dose.
