Recurrent Falls in Older Adults

## Summary

Recurrent falls in older adults need assessment of modifiable causes rather than being accepted as inevitable aging. Balance problems, medicines, vision and home hazards may combine. Individual prevention helps preserve independence.

## Overview

A fall without a fracture can still reveal a treatable problem. Describe events before, during and after it, especially dizziness or loss of consciousness, instead of assuming every episode was a simple trip.

## Causes

Causes include weakness, impaired balance, postural blood-pressure drops, medicine effects, foot or vision problems and neurological disease. Hazards, infection or dehydration may contribute, often with several factors together.

## Symptoms

Dizziness, instability or palpitations may precede a fall, or there may be no warning. Fear afterward can reduce activity and strength. Confidence and ability to get up safely also matter.

## Risk factors

Previous falls, frailty, multiple medicines and impaired vision or balance are important risks. Age does not replace individual assessment or mean improvement is impossible.

## Diagnosis

Assessment reviews falls, medicines, gait, balance and blood pressure in relevant positions, with vision, feet, function and cognition as needed. Further tests and referrals follow clinical suspicion, not one universal test.

## Treatment

Care combines suitable strength and balance training, medication review, treatment of identified causes, environmental changes and appropriate walking aids. Do not independently stop sedatives or blood-pressure treatment. Address bone health or deficiency when indicated; supplements are not a universal fall-prevention treatment.

## Complications

Falls may cause fractures, head injury, fear and reduced mobility. A prolonged time on the floor can cause dehydration or pressure injury. Reporting falls helps prevent recurrence rather than threatening independence.

## Prevention

Clear walkways, secure rugs, improve lighting and bathroom supports and use stable footwear. Practice safe mobility with professional guidance and keep a way to call for help. Avoid unplanned bed rest driven by fear.

## When to seek care

Call emergency services if unable to rise, a head, hip or neck injury is suspected, or there is loss of consciousness or neurological change. Do not forcefully lift the person. A head strike while taking anticoagulants needs urgent assessment even if apparently minor.
