## Summary

Felty syndrome is a rare complication associated with rheumatoid arthritis and neutropenia, often with an enlarged spleen. Infection risk is central; not every low blood count in RA is Felty syndrome.

## الأسباب

تشارك آليات مناعية معقدة في نقص العدلات، وقد يؤثر الطحال في تعداد الخلايا. السبب الكامل غير محسوم ولا يعني التشخيص أن المريض تسبب فيه.

## الأعراض

قد توجد عدوى متكررة وتعب أو قرح جلدية ونقص وزن مع علامات الروماتويد. قد يكون تضخم الطحال دون ألم، ولا يمكن تقدير العدلات من الأعراض.

## عوامل الخطر

يشيع الارتباط بالروماتويد طويل المدة، خصوصًا ذي المظاهر خارج المفاصل. لا يكفي تضخم الطحال وحده لإثبات المتلازمة، والمتابعة تحدد الحاجة لفحوص إضافية.

## التشخيص

يشمل التقييم تعداد الدم التفريقي والفحص ومراجعة الأدوية، وتصوير الطحال عند الحاجة. قد يحتاج اختصاصي الدم فحوصًا إضافية لاستبعاد ابيضاض الخلايا اللمفاوية الحبيبية الكبيرة.

## العلاج

يركز العلاج على ضبط الروماتويد ونقص العدلات ومعالجة العدوى. تُختار الأدوية ومحفزات العدلات بحسب الحالة، ويُبحث استئصال الطحال في حالات محددة لا كخيار تلقائي.

## المضاعفات

قد تحدث عدوى شديدة ومتكررة ومشكلات التئام جروح أو اضطرابات تعداد أخرى. تختلف المخاطر وفق درجة نقص العدلات والأمراض والأدوية المرافقة.

## الوقاية

التزم بعلاج الروماتويد وفحوص الدم واللقاحات المناسبة. راجع الجروح مبكرًا وناقش علامات العدوى؛ لا تعدّل مثبطات المناعة أو توقفها دون خطة طبية.

## متى تراجع الطبيب؟

حمى أو قشعريرة أو تدهور عام عند وجود نقص عدلات تحتاج تقييمًا عاجلًا. ضيق النفس أو الارتباك أو هبوط الضغط يستدعي الطوارئ.

Joint disease may be longstanding while neutropenia is detected on blood testing. Drug effects, infections and blood disorders with similar findings must be excluded.

## Causes

Complex immune mechanisms contribute to neutrophil loss, and the spleen may affect blood counts. The full cause is uncertain and is not evidence of personal fault.

## Symptoms

Recurrent infections, fatigue, skin ulcers or weight loss may accompany RA findings. Splenic enlargement may be painless; symptoms cannot determine the neutrophil count.

## Risk factors

Longstanding RA, particularly with extra-articular features, increases suspicion. An enlarged spleen alone does not prove the syndrome; follow-up guides further investigation.

## Diagnosis

Assessment includes differential blood counts, examination, medicine review and selected splenic imaging. Hematology testing may be required to exclude large granular lymphocytic leukemia.

## Treatment

Care targets rheumatoid disease, neutropenia and infection. Medicines and neutrophil growth factors are individualized; splenectomy is reserved for selected circumstances.

## Complications

Severe or recurrent infections, poor wound healing and other blood-count abnormalities may occur. Risk depends on neutropenia severity, coexisting disease and medicines.

## Prevention

Follow RA treatment, blood monitoring and appropriate vaccination plans. Obtain early wound assessment and discuss infection warnings; do not independently change immune-suppressing medicines.

## When to seek care

Fever, chills or marked deterioration with neutropenia requires urgent assessment. Breathlessness, confusion or low blood pressure warrants emergency care.
