## Summary

Gout is a painful inflammatory arthritis caused by uric acid crystal deposition in joints, typically affecting the big toe. Treatment includes anti-inflammatory medications, urate-lowering drugs like allopurinol, and dietary modifications.

## Overview

Gout is an acute and painful form of arthritis caused by deposition of monosodium urate crystals in joints and surrounding tissues. It develops when serum uric acid levels exceed the saturation point (above 6.8 mg/dL), triggering sharp crystals that provoke intense inflammatory response. The big toe joint is typically affected first, but ankles, knees, wrists, and fingers may also be involved.

Gout is prevalent worldwide and affects millions, with rising incidence in Gulf Cooperation Council countries and the Middle East, including Jordan, particularly in males and middle-aged to older populations. The condition is linked to modern lifestyle patterns including high consumption of red meat, seafood, sugary beverages, and alcohol.

Gout occurs in acute flares of severe pain lasting days to weeks, followed by periods of remission that may extend months or years. Without proper management, it can progress to chronic tophaceous gout with permanent joint deformities and renal damage.

## Causes

- **Hyperuricemia (High uric acid levels)**: Serum uric acid exceeding 6.8 mg/dL supersaturates body fluids, causing crystal deposition in joints and soft tissues.

- **High-purine diet**: Red meats, organ meats, shellfish (shrimp, oysters), and certain fish are rich in purines that metabolize to uric acid.

- **Sugary beverages and alcohol**: Soft drinks, fruit juices, beer, and spirits increase uric acid production and reduce renal clearance.

- **Genetic predisposition**: Inherited factors affecting uric acid metabolism; family history of gout increases risk substantially.

- **Chronic kidney disease**: Reduced glomerular filtration impairs uric acid excretion, leading to accumulation.

- **Obesity and weight gain**: Increases insulin resistance and uric acid production while decreasing renal urate clearance.

- **Surgery and severe stress**: Rapid changes in uric acid levels can precipitate acute gout flares.

- **Hematologic malignancies**: Rapid cell death releases large amounts of purines converted to uric acid.

## Symptoms

Gout symptoms range from acute severe flares to asymptomatic periods. An acute attack typically begins abruptly at night and peaks within 24-48 hours. Symptom severity may vary by patient age and attack frequency history.

- **Intense joint pain**: Usually affecting the big toe, but may involve ankles, knees, wrists, and fingers; patients describe it as unbearable sharp pain.

- **Redness and swelling**: The affected joint becomes visibly red, inflamed, and swollen.

- **Warmth and heat over the joint**: Skin over the affected joint feels hot to the touch.

- **Morning stiffness**: Particularly in affected joints; may persist for hours.

- **Itching and skin peeling**: Several days into the flare, mild skin desquamation may occur around the affected area.

- **Mild fever**: Acute flares may be accompanied by low-grade fever (38-39°C).

- **Fatigue and mood changes**: General exhaustion and psychological distress from severe pain.

- **Difficulty walking and moving**: When the big toe is affected, ambulation becomes extremely painful and mobility is severely limited.

- **Recurrent flares**: Most patients experience one flare annually initially, but without treatment attacks become increasingly frequent.

- **Tophi (small nodules)**: In chronic gout, white chalky deposits of uric acid crystals appear on ears, fingers, and elbows.

## Risk factors

- **Male gender**: Men are 3-4 times more likely to develop gout than women, especially after age 40.

- **Advanced age**: Gout risk increases with age, peaking at 40-50 years in men and 60 years in women.

- **Hypertension**: High blood pressure may reduce renal urate excretion and increase gout risk.

- **Diabetes and glucose intolerance**: Elevate uric acid levels and impair renal clearance.

- **Medication use**: Diuretics (thiazides, loops), cardiac drugs, and low-dose aspirin can raise uric acid.

- **Frequent red meat and organ meat consumption**: Regular intake of purine-rich foods significantly increases risk.

- **Alcohol consumption, especially beer**: Impairs uric acid excretion and rapidly elevates levels.

- **Genetic factors**: Family history of gout doubles risk substantially.

- **Chronic kidney and liver disease**: Reduce body's ability to eliminate uric acid.

- **Menopause in women**: Post-menopausal gout increases due to loss of estrogen's protective effect on renal urate handling.

## Diagnosis

Accurate gout diagnosis relies on clinical examination, laboratory tests, and imaging. Clinical assessment and medical history are important, but confirmation requires additional investigations.

- **Clinical examination**: A rheumatologist or primary care physician examines the affected joint for pain, swelling, erythema, and warmth. Detailed history of family, medications, and diet is obtained.

- **Serum uric acid level**: Normal value is below 6.8 mg/dL. Elevated levels support diagnosis, but may be normal during acute flares. Measurement should be repeated 2-4 weeks after flare resolution.

- **Synovial fluid analysis (joint aspiration)**: Fluid withdrawn from the joint by needle is examined under polarized light microscopy for needle-shaped, negatively birefringent monosodium urate crystals. This is the gold standard diagnostic test.

- **X-ray**: Acute gout may not appear on plain radiographs, but chronic cases show characteristic punched-out erosions around affected joints.

- **Ultrasound**: Modern imaging may reveal crystal deposits in joints and soft tissues, improving chronic gout diagnosis.

- **Renal and hepatic function tests**: Measurement of creatinine, eGFR, and liver function assess kidney and liver capacity for uric acid elimination and guide medication safety.

- **24-hour urine uric acid**: Helps classify gout as underexcretion or overproduction and directs long-term management strategy.

## Treatment

### Medications

- **Non-steroidal anti-inflammatory drugs (NSAIDs)**: Such as indomethacin, ibuprofen, and naproxen. Used for acute flares to reduce inflammation and pain. Avoid in severe renal impairment or gastrointestinal sensitivity.

- **Colchicine**: Highly effective for acute gout flares, especially if started within 12-24 hours of onset. High doses may cause diarrhea.

- **Corticosteroids**: Such as prednisone. Used when patients cannot tolerate NSAIDs or colchicine. Effective but chronic use should be avoided.

- **Allopurinol**: Reduces uric acid production significantly. Used for long-term management and prevention of recurrent flares. Initial dose 50 mg daily, titrated gradually to 300 mg daily based on response.

- **Febuxostat**: Modern alternative to allopurinol, more selective and potent. Used in allopurinol-resistant or allergic patients.

- **Benzbromarone and lesinurad**: Uricosuric agents that increase renal uric acid excretion. Used in underexcretion-type gout.

- **Pegloticase**: Highly potent newer drug that directly metabolizes uric acid. Used for severe, refractory gout. Administered intravenously.

### Procedures

- **Joint aspiration**: Not a treatment but essential for diagnosis confirmation; may provide temporary pressure relief.

- **Intra-articular steroid injection**: Direct joint injection of corticosteroid provides rapid relief and local anti-inflammatory effect.

- **Tophus removal**: In severe chronic cases, surgical removal may improve mobility and appearance.

- **Arthroscopy/Arthrocentesis**: Rarely needed, but may be used in complicated chronic gout with severe deformities.

### Lifestyle Modifications

- **Avoid purine-rich foods**: Reduce red meat, organ meats, and high-purine seafood (shrimp, oysters, salmon). Choose poultry, eggs, and low-fat dairy instead.

- **Avoid sugary drinks and alcohol**: Eliminate soft drinks and fruit juices; limit alcohol, especially beer. Drink adequate water (2-3 liters daily) to enhance uric acid excretion.

- **Gradual weight loss**: Losing 0.5-1 kg monthly significantly reduces uric acid levels.

- **Regular moderate exercise**: Improves cardiovascular health, weight, and metabolism, but avoid intense activity during acute flares.

- **Blood pressure and diabetes control**: Maintain normal BP and glucose levels to reduce flare risk.

- **Increase fiber, fruits, and vegetables**: Especially cherries and berries, which may help reduce uric acid.

- **Limit saturated fats and cholesterol**: May worsen inflammation and impair uric acid excretion.

- **Avoid fasting and crash diets**: May increase uric acid production and trigger acute flares.

## Complications

- **Chronic tophaceous gout**: Repeated flares lead to chronic gout with white chalky deposits (tophi) on ears, fingers, and elbows, causing permanent joint deformities and reduced mobility.

- **Uric acid kidney stones**: Crystal deposition in the urinary system causes extremely painful stones and may lead to acute renal failure.

- **Chronic kidney disease**: Recurrent, untreated gout causes permanent kidney damage and chronic renal failure requiring dialysis.

- **Secondary hypertension**: Gout-related kidney damage may result in moderate to severe hypertension.

- **Dementia and stroke**: Some studies link chronic gout to increased risk of dementia and cerebrovascular accidents, especially with concurrent hypertension.

- **Cardiac disease and thrombosis**: Gout patients have higher risk of coronary artery disease, myocardial infarction, and pulmonary embolism.

- **Secondary infections and sepsis**: Chronic gout with tophi may develop secondary infections leading to fever and sepsis.

- **Quality of life and mental health**: Chronic pain and physical deformities cause depression, anxiety, and social isolation.

## Prevention

- **Maintain healthy weight**: Avoid obesity through regular exercise and balanced nutrition. Excess weight directly increases gout risk.

- **Reduce red meat, organ meats, and seafood**: Limit consumption to once weekly or less. Choose poultry, white fish, eggs, and dairy products.

- **Avoid sugary drinks and alcohol**: Especially beer and soft drinks. Replace with water, black tea, and unsweetened coffee.

- **Drink adequate water**: 2-3 liters daily improves renal uric acid excretion and reduces kidney stone risk.

- **Control comorbid conditions**: Well-manage hypertension, diabetes, and chronic kidney disease.

- **Avoid certain medications if possible**: Consult your doctor about drugs elevating uric acid (diuretics, low-dose aspirin).

- **Avoid fasting and crash diets**: May trigger acute flares due to rapid uric acid level changes.

- **Eat protective foods**: Consume cherries, berries, yogurt, coffee, and green tea, which may help reduce uric acid.

- **Regular uric acid screening**: If you have family history or risk factors, ask your doctor for annual uric acid measurement.

- **Maintain active lifestyle**: Regular exercise (150 minutes weekly) improves metabolism and reduces gout risk.

## When to seek care

Seek medical attention immediately if you experience sudden severe joint pain, especially with obvious swelling and redness. Do not wait—gout requires accurate diagnosis and prompt treatment. Schedule an appointment with a doctor on **Clinics JO** if you suspect symptoms.

- **Sudden severe joint pain**: Especially in the big toe, ankle, or knee, with visible swelling and redness.

- **Recurrent joint pain flares**: If two or more episodes occur in one year.

- **Significant difficulty walking or moving the joint**: When pain is severe enough to limit normal function.

- **Fever accompanying pain and swelling**: May indicate severe inflammation or secondary infection.

- **White nodules on ears or fingers**: May be tophi indicating chronic gout.

- **Elevated serum uric acid levels**: If measured above 6.8 mg/dL without symptoms.

- **Strong family history of gout**: Consult your doctor if first-degree relatives have gout, even without symptoms.

- **Difficulty controlling symptoms despite treatment**: Persistent flares may require treatment adjustment.

- **Visit the emergency department if**: You experience unbearable pain, high fever (>39°C) with pain, or complete inability to move the joint.

Published questions

## Is gout hereditary?

Gout has a strong genetic component—if a parent has gout, your risk increases significantly. However, genetics alone don't cause gout; the disease requires interaction between genes and environmental factors like diet, obesity, and medications.

## Can gout be cured permanently?

Gout is a chronic disease that cannot be permanently cured, but can be managed very well. With proper treatment and dietary/lifestyle changes, you may go years without flares. The goal is maintaining uric acid levels in the safe range (below 6 mg/dL).

## Does gout affect women the same way as men?

Women are less susceptible to gout before menopause, but risk equalizes afterward. Post-menopausal women may experience more severe gout. Pre-menopausal estrogen enhances renal uric acid excretion; its decline after menopause increases risk substantially.

## What foods should be completely avoided with gout?

Avoid red meat especially organ meats, high-purine seafood (shrimp, oysters), sugary drinks, and beer. Limit poultry and salmon; avoid alcohol. Choose vegetables, low-fat dairy, and whole grains instead.

## Are coffee and tea allowed for gout patients?

Yes, black tea, green tea, and coffee are safe and may be beneficial. Some studies suggest coffee may lower uric acid levels. Just avoid added sugar and excessive amounts (more than 3-4 cups daily).

## How long does a gout flare typically last?

An acute gout flare usually lasts 3-10 days, with peak pain in the first 24-48 hours. With proper treatment, it may resolve faster. Without treatment, it may extend for weeks. Pain gradually decreases until complete symptom resolution.

## Can a gout flare occur while taking allopurinol?

Yes, acute flares may occur when starting allopurinol or increasing the dose. Rapid uric acid reduction can release crystals from tissues. Therefore, concurrent anti-inflammatory agents (indomethacin or colchicine) are recommended for 3-6 months when initiating allopurinol.

## Does fasting and strict dieting increase gout flare risk?

Yes, strongly. Fasting and crash diets increase uric acid production from increased tissue breakdown and reduce renal excretion. Even rapid weight loss can trigger a flare. Lose weight gradually (500g monthly) with a sustainable, balanced diet.

## Does diabetes affect gout?

Yes, diabetic patients have higher gout risk. High blood glucose impairs renal uric acid excretion, elevating levels. Good diabetes control helps reduce gout risk and complications.

## Can gout be completely prevented if there is strong family history?

Complete prevention is unlikely with strong genetic predisposition, but onset can be delayed and severity significantly reduced. Maintaining healthy weight, avoiding purine-rich foods, limiting alcohol, and regular exercise substantially reduce risk despite genetic susceptibility.
