Hepatic Echinococcosis

## Summary

Hepatic echinococcosis is a parasitic infection that can form hydatid cysts in the liver. It differs from a simple cyst; care depends on parasite type, cyst stage, location and complications.

## Overview

Cysts can remain silent for years and be found on imaging. Cystic disease differs from the more infiltrative alveolar form; not every liver mass is parasitic.

## Causes

Infection follows ingestion of eggs through food, water or hands contaminated by infected animal feces, particularly dogs in the hydatid cycle. Ordinary person-to-person contact does not usually transmit it.

## Symptoms

Upper abdominal discomfort, fullness or nausea may occur. Bile-duct involvement or infection may cause jaundice or fever, while many early cases have no symptoms.

## Risk factors

Exposure increases in livestock-raising settings involving infected dogs and unsafe food or water. Owning a dog alone does not establish infection.

## Diagnosis

Ultrasound evaluates cyst characteristics and stage, supported by other imaging and antibody tests when appropriate. A suspected cyst should not undergo routine puncture without specialist planning.

## Treatment

Selected inactive uncomplicated cysts are monitored with imaging. Other cases need albendazole, specialized percutaneous treatment or surgery, with expert precautions against leakage and complications.

## Complications

Rupture can cause severe allergic reactions, parasite spread, biliary obstruction or infection. Long-term follow-up checks cyst activity and recurrence after treatment.

## Prevention

Wash hands and produce, use safe water and arrange appropriate veterinary dog care. Avoid feeding dogs raw offal and unsafe slaughter practices that sustain parasite transmission.

## When to seek care

Sudden severe pain or rash with breathlessness or dizziness after possible cyst rupture requires emergency help. Fever or jaundice with abdominal pain needs prompt assessment.
