## Summary

Lichen simplex chronicus, also called neurodermatitis, is localized skin thickening within an itch–scratch cycle. Care addresses itch triggers and skin healing; the name does not mean the condition is purely psychological or contagious.

## Overview

An itchy patch can become thicker and itchier with repeated rubbing, maintaining the cycle. Common sites include the neck, limbs, scalp and genital area. Other disorders can resemble it, so assessment is useful before applying potent creams.

## Causes

Dryness, eczema, irritation or friction may start the itch, although the trigger is not always clear. Stress can intensify scratching but is not the only explanation. Other skin disorders may coexist and require separate treatment.

## Symptoms

One or several defined patches may itch intensely, with thickened skin, prominent lines, colour change and scratches. Itching at rest or at night may disrupt sleep. Repeated scratching can cause wounds, bleeding or localized hair loss.

## Risk factors

Eczema, dry skin, repeated friction and stress can increase susceptibility in some people. Not every itchy patch has this diagnosis, and anxiety does not mean symptoms are imagined. Recognizing triggers helps reduce recurrence.

## Diagnosis

A clinician examines the distribution and appearance and asks about onset, products and medicines. Suspected infection may need a swab; allergy testing or biopsy may be appropriate. Persistent generalized itching needs investigation rather than automatic labelling as lichen simplex.

## Treatment

Treatment breaks the itch–scratch cycle through moisturization, trigger avoidance and habit changes. A topical corticosteroid may be prescribed with strength and duration matched to the site; potent products do not suit all areas, especially face and folds. Alternatives and confirmed infection are managed individually.

## Complications

Scratching may lead to infection, scars or lasting pigment changes. Poor sleep and persistent itch can affect concentration and mood. Relapses can occur; follow-up helps adjust treatment and avoid adverse effects from overusing medicated creams.

## Prevention

Use fragrance-free moisturizer, keep nails short and try brief cool compresses instead of scratching. Choose comfortable clothing and avoid provoking heat or friction. Do not cover medicated creams with an occlusive dressing unless directed, because absorption may increase.

## When to seek care

See a dermatologist for persistent itching, disrupted sleep or genital symptoms. Increasing pain, warmth or pus needs prompt assessment, especially with fever. A changing or non-healing ulcerated patch also needs examination.
