## Summary

Liver cirrhosis is permanent scarring of the liver caused by chronic diseases such as viral hepatitis, alcohol abuse, and fatty liver disease. It leads to advanced liver failure and serious complications that may require transplantation. Treatment focuses on slowing progression and managing complications.

## Overview

Liver cirrhosis is a chronic liver disease and one of the most serious hepatic conditions. It involves progressive destruction of healthy liver cells with replacement by fibrous scar tissue. This scarring leads to loss of liver function and impaired blood flow through the liver. The disease develops silently in early stages but eventually progresses to complete liver failure that threatens life.

Major causes of cirrhosis include alcohol abuse (the leading cause worldwide), chronic viral hepatitis (especially hepatitis B and C), non-alcoholic fatty liver disease, chronic biliary disease, and autoimmune hepatitis. In the Middle East and Jordan, cirrhosis is prevalent due to viral hepatitis B and C, alongside alcohol and increasing obesity rates.

When the liver becomes cirrhotic, multiple complications emerge including ascites (fluid accumulation in the abdomen), variceal bleeding (severe bleeding from blood vessels), hepatic encephalopathy (altered consciousness and memory), and acute kidney failure. The disease requires continuous medical monitoring and may ultimately necessitate liver transplantation.

## Causes

- **Chronic alcohol abuse:** Excessive alcohol consumption is the leading cause of cirrhosis worldwide, causing persistent inflammation and gradual destruction of liver cells.

- **Chronic viral hepatitis (B and C):** Ongoing infection with hepatitis B or C viruses leads to chronic inflammation and liver scarring over years or decades.

- **Non-alcoholic fatty liver disease (NAFLD):** Fat accumulation in the liver without alcohol consumption, especially in obese and diabetic individuals, causes inflammation and progressive fibrosis.

- **Chronic biliary disease:** Persistent obstruction or inflammation of bile ducts leads to bile accumulation and liver cirrhosis.

- **Autoimmune hepatitis:** Immune system attack on liver cells causes chronic inflammation and scarring.

- **Hereditary hemochromatosis:** Excess iron accumulation in liver tissues causes cirrhosis.

- **Nutritional deficiencies and genetic factors (especially alpha-1 antitrypsin deficiency):** Rare genetic disorders may cause early-onset cirrhosis.

## Symptoms

Symptoms of liver cirrhosis may be mild or absent in early stages, but become severe and life-threatening as the disease progresses. Watch for warning signs and see a doctor immediately if you experience any of the following symptoms:

- **Jaundice (yellowing):** Yellowing of skin and eyes due to bilirubin accumulation.

- **Ascites:** Swelling and distension of the abdomen from fluid accumulation.

- **Leg and ankle swelling:** Fluid retention in lower extremities (edema).

- **Fatigue and general weakness:** Severe tiredness that doesn't improve with rest.

- **Loss of appetite and nausea:** Unwillingness to eat and persistent nausea.

- **Unintentional weight loss:** Rapid weight loss without trying.

- **Abdominal pain and bloating:** Pain in upper abdomen, especially on the right side.

- **Changes in urine and stool color:** Dark urine and pale-colored stools.

- **Persistent skin itching:** Severe itching without obvious rash.

- **Confusion and memory problems:** Difficulty concentrating and mental impairment (hepatic encephalopathy).

- **Nosebleeds or vomiting blood:** Dangerous sign indicating variceal bleeding.

- **Dizziness and fainting:** Especially with internal bleeding.

## Risk factors

- **Excessive alcohol consumption:** Drinking more than 2-3 alcoholic beverages daily for women and 3-4 for men significantly increases risk.

- **Chronic hepatitis B or C infection:** People with chronic infection have much higher risk of cirrhosis.

- **Obesity and diabetes:** Overweight and obesity, especially in diabetics, increase risk of fatty liver and cirrhosis.

- **Advanced non-alcoholic fatty liver disease (NASH):** The inflammatory stage of fatty liver significantly raises cirrhosis risk.

- **Family history of liver disease:** Having relatives with cirrhosis or genetic liver diseases increases risk.

- **Weakened immune system:** HIV patients or those on immunosuppressive medications are at higher risk.

- **Occupational toxin exposure:** Continuous exposure to toxic chemicals may cause cirrhosis.

- **Age and gender:** Men and older individuals are at higher risk.

## Diagnosis

Diagnosing liver cirrhosis requires a combination of clinical examination and laboratory and imaging tests. The doctor typically begins with careful physical examination, then progresses to specialized tests that confirm the diagnosis and determine disease severity.

- **Physical examination:** Detection of cirrhosis signs such as jaundice, enlarged or shrunken liver, spider angiomas (red spots), and abdominal swelling.

- **Liver function tests:** Measurement of ALT and AST (liver enzymes), bilirubin, albumin and proteins. Reference values: ALT 7-56 U/L, AST 10-40 U/L (elevated in cirrhosis).

- **INR test (prothrombin time ratio):** Measures blood clotting ability; cirrhotic liver produces low amounts of clotting factors.

- **Platelet count:** Cirrhosis causes platelet reduction (<150,000 cells/microliter is abnormal).

- **Ultrasound (sonography):** Visualizes liver scarring, size, and signs of portal hypertension.

- **MRI or CT scan:** Provides detailed assessment of fibrosis severity and esophageal varices.

- **Elastography (FibroScan):** Non-invasive test measuring liver stiffness and fibrosis grade.

- **Liver biopsy:** Tissue sampling to confirm diagnosis and assess fibrosis grade (rarely used now).

- **Esophageal endoscopy:** To detect esophageal varices if variceal bleeding is suspected.

## Treatment

### Medications

- **Beta-blockers (propranolol, carvedilol):** Reduce portal hypertension and prevent variceal bleeding.

- **Diuretics (furosemide, spironolactone):** Reduce ascites and swelling by removing excess fluid.

- **Antiviral medications:** For hepatitis B or C patients, antivirals may stop disease progression if started early.

- **Antacids and anti-nausea drugs:** Omeprazole and antiemetics relieve symptoms.

- **Antibiotics (norfloxacin):** Prevent bacterial infections in severe ascites and hepatic encephalopathy.

- **Lactulose and rifaxomicin:** Treat hepatic encephalopathy by reducing ammonia absorption.

### Procedures and Surgery

- **Endoscopic variceal ligation:** Emergency procedure to band bleeding varices and stop severe hemorrhage.

- **Sclerotherapy:** Injection of sclerosing agent into varices to seal them and prevent bleeding.

- **TIPS (Transjugular Intrahepatic Portosystemic Shunt):** Interventional procedure creating a bypass to reduce portal hypertension in treatment-resistant cases.

- **Paracentesis:** Fluid aspiration from the abdomen to relieve symptoms in severe treatment-resistant ascites.

- **Liver transplantation:** The only definitive cure for advanced cirrhosis and liver failure, especially for children and young adults.

### Lifestyle Changes and Supportive Measures

- **Complete alcohol abstinence:** If cirrhosis is alcohol-related, total cessation is mandatory; even small amounts cause further damage.

- **Low-sodium healthy diet:** Reduce salt intake to minimize ascites and swelling; consume moderate, healthy protein.

- **Avoid fatty and processed foods:** Reduce fat and calories to decrease liver strain.

- **Weight management:** Gradual weight loss if obese reduces liver burden.

- **Regular follow-up monitoring:** Check-ups every 3-6 months to track disease progression and complications.

- **Preventive vaccinations:** Hepatitis A and B, influenza, and pneumococcal vaccines prevent additional infections.

- **Avoid hepatotoxic medications:** Avoid medications toxic to the liver, such as high-dose NSAIDs and many herbal supplements.

- **Take B vitamins and folic acid:** Cirrhotic liver stores these poorly and supplementation may be needed.

## Complications

- **Hepatic Encephalopathy:** Accumulation of toxins in blood due to liver failure affects the brain, causing confusion, memory loss, and potentially coma. One of the most dangerous complications and potentially fatal.

- **Variceal Bleeding:** Life-threatening hemorrhage from enlarged blood vessels in the esophagus and stomach requiring immediate emergency intervention.

- **Refractory Ascites:** Massive fluid accumulation in the abdomen unresponsive to medications, causing severe discomfort and difficulty breathing.

- **Hepatorenal Syndrome:** Acute kidney failure resulting from advanced liver complications; very dangerous and potentially fatal.

- **Acute Liver Failure:** Sudden, rapid collapse of liver function occurring within days or weeks, requiring emergency transplantation.

- **Hepatocellular Carcinoma (HCC):** Cirrhotic liver is at very high risk for cancer development, especially in those with chronic viral hepatitis.

- **Bacterial Infections (Spontaneous Bacterial Peritonitis):** Bacterial infection of abdominal fluid that can be fatal if not treated rapidly.

- **Portal Hypertension:** Elevated pressure in the portal vein causes vessel enlargement and increases risk of bleeding, ascites, and spleen enlargement.

## Prevention

- **Avoid alcohol completely or drink minimally:** Abstinence or significant reduction is the best prevention for alcohol-related cirrhosis. If addicted, seek professional help immediately.

- **Vaccination against hepatitis A and B:** Safe and highly effective vaccines (3-dose series for hepatitis B) provide lasting protection against these viruses.

- **Avoid blood exposure:** Use gloves when handling others' wounds; don't share toothbrushes, razors, or needles.

- **Safe sexual practices:** Condom use reduces risk of hepatitis B and C transmission.

- **Maintain healthy weight and combat obesity:** Weight loss if obese significantly reduces risk of fatty liver and cirrhosis.

- **Manage diabetes and high blood pressure:** Good control of these chronic conditions reduces liver strain.

- **Avoid hepatotoxic medications:** Avoid high doses of painkillers (especially acetaminophen and NSAIDs) and many herbal supplements.

- **Regular screening for viruses:** Especially if in high-risk groups (healthcare workers, high-risk behavior individuals).

- **Practice safe hygiene:** Hand washing before eating and after toilet use reduces infection transmission.

- **Get immediate treatment for hepatitis:** If diagnosed with hepatitis (viral or otherwise), start treatment early to prevent progression to cirrhosis.

Published questions

## Is liver cirrhosis contagious?

Cirrhosis itself is not contagious, but its cause may be. If caused by viral hepatitis (B or C), the virus is contagious and spreads through blood or body fluids, but the resulting cirrhosis stage is not contagious.

## What is the average life expectancy with liver cirrhosis?

Life expectancy depends on disease stage. Without complications and with good monitoring, patients may live for years. With serious complications (bleeding, kidney failure) without transplant, prognosis is worse. Transplantation significantly improves survival.

## Can liver cirrhosis be reversed or cured?

Cirrhosis itself cannot be completely reversed because scar tissue doesn't transform back to healthy tissue. However, if the cause is eliminated (alcohol cessation, viral treatment), progression may halt and stabilize. Transplantation is the only complete cure.

## Is light alcohol consumption safe for cirrhosis patients?

No, even small amounts of alcohol are harmful for cirrhosis patients. The cirrhotic liver is very sensitive, and any alcohol causes additional damage and accelerates disease progression. Complete abstinence is necessary.

## Is liver cirrhosis a hereditary disease?

Most cirrhosis cases are not directly hereditary, but some rare causes are genetic (alpha-1 antitrypsin deficiency, hemochromatosis). Genetic predisposition to obesity and diabetes may increase risk of fatty liver and cirrhosis.

## Can cirrhosis be diagnosed without biopsy?

Yes, most cirrhosis cases can be diagnosed without biopsy using blood tests, FibroScan (elastography), and imaging (ultrasound or MRI). Biopsy is rarely used now.

## What is the difference between cirrhosis and fatty liver?

Fatty liver is fat accumulation without scarring and may be reversible if the cause is removed. Cirrhosis is permanent scarring that doesn't return to normal. Fatty liver may progress to cirrhosis if untreated, especially NASH.

## Is cirrhosis contagious to family members?

Cirrhosis itself is not contagious, but if the cause is infection (like hepatitis B or C), the infection may spread to family without precautions. Vaccination and good hygiene prevent transmission.

## Can you live a normal life with stable cirrhosis?

Yes, if cirrhosis is stable without active complications, you can live a relatively normal life with regular medical follow-up, medications, and lifestyle changes. However, alcohol abstinence and caution are essential.

## Are there new treatments for liver cirrhosis?

Current treatments focus on slowing progression and managing complications. New drugs for fatty liver and cirrhosis are under research, but transplantation remains the only solution for advanced cirrhosis. Early detection and antiviral therapy may prevent progression.
