## Summary

Meningitis is inflammation of the membranes surrounding the brain and spinal cord, and may be a serious life-threatening bacterial type or a milder viral one. Its characteristic signs are fever, severe headache, neck stiffness, and light sensitivity. The bacterial type is an emergency needing antibiotics immediately, and vaccines protect against important types of it.

## Overview

Meningitis is inflammation of the three membranes (the meninges) that surround and protect the brain and spinal cord. Its danger lies in its closeness to the central nervous system, as it may progress rapidly and lead to serious complications if it is the bacterial type and not treated immediately.

Meningitis has types by cause: viral meningitis is the most common and usually milder, and most affected people recover on their own; bacterial meningitis is less common but very dangerous and is a life-threatening emergency needing immediate intravenous antibiotics, and any delay may be fatal. The characteristic signs everyone should watch for include: a sudden high fever, a severe unusual headache, neck stiffness (difficulty bending the neck forward), severe light sensitivity, nausea and vomiting, and confusion or severe drowsiness. In infants signs may differ, appearing as fever, persistent sharp crying, refusal to feed, bulging of the fontanelle (the soft area on the head), and lethargy.

An important warning sign is a hemorrhagic rash (red or purple spots that do not fade when pressed), which may indicate a severe bacterial infection (meningococcal sepsis) and requires calling for an ambulance immediately. Meningitis is diagnosed by clinical examination, blood tests, and often a lumbar puncture (taking a sample of fluid around the spinal cord) to determine the type. Treatment begins rapidly when the bacterial type is suspected, with intravenous antibiotics without waiting for all results, along with supportive care in hospital, while the viral type is often treated with rest, fluids, and painkillers. Prevention is partly possible through vaccines (against meningococcus, pneumococcus, and Haemophilus influenzae) that protect against the most important causes of bacterial meningitis, in addition to hygiene and avoiding contact with the infected.

## Causes

- Viral infection (the most common, usually milder).

- Bacterial infection (less common but dangerous and an emergency).

- Less common causes (fungal or non-infectious).

## Symptoms

- A sudden high fever and a severe unusual headache.

- Neck stiffness and severe light sensitivity.

- Nausea, vomiting, and confusion or severe drowsiness.

- A hemorrhagic rash that does not fade with pressure (a serious warning sign).

- In infants: sharp crying, refusal to feed, bulging fontanelle, lethargy.

## Risk factors

Risk factors vary with the cause of meningitis. Some forms are associated with incomplete vaccination, young age, communal living, immune suppression or an absent spleen. Pregnancy increases susceptibility to listeria infection. These factors raise concern but do not diagnose meningitis; fever with neck stiffness or altered awareness requires urgent assessment.

## Diagnosis

- Clinical examination and signs of meningeal irritation.

- Blood tests.

- Lumbar puncture to determine the type (bacterial/viral).

## Treatment

### Bacterial type (emergency)

- Immediate intravenous antibiotics on suspicion without waiting for all results.

- Supportive hospital care and monitoring for complications.

### Viral type

- Often rest, fluids, and painkillers with monitoring, and much of it recovers on its own.

This content is educational and does not replace urgent medical care; suspected meningitis is an emergency.

## Complications

- Hearing loss, seizures, and brain effects in severe bacterial type.

- Meningococcal sepsis and effects on the limbs.

- May be life-threatening with delayed treatment.

## Prevention

- Vaccines (meningococcus, pneumococcus, and Haemophilus influenzae).

- Hygiene, hand washing, and avoiding sharing utensils.

- Preventive treatment for close contacts of a confirmed bacterial case under medical supervision.

## When to seek care

Call for an ambulance immediately for fever with severe headache, neck stiffness, light sensitivity, and confusion, or a hemorrhagic rash that does not fade with pressure, or in an infant with fever, sharp crying, refusal to feed, and lethargy. Suspected bacterial meningitis is an emergency that cannot tolerate delay. **Go to the emergency department directly**.
