Occupational Hearing Loss

## Summary

Occupational hearing loss may result from workplace noise or chemicals harmful to hearing. Prevention focuses on exposure reduction and hearing surveillance. Rehabilitation supports communication but cannot repair permanent inner-ear damage.

## Overview

Damage may develop gradually without pain, making it easy to miss. A single explosive sound can also injure hearing. Occupational hearing loss does not necessarily mean complete deafness, and other causes should be assessed rather than assumed.

## Causes

Intense or repeated noise damages inner-ear cells. Certain solvents and ototoxic substances can add risk, especially alongside noise. Sound intensity, duration and pattern influence harm; a job title alone cannot establish exposure or causation.

## Symptoms

Clues include difficulty understanding speech in background noise, missing quiet sounds, tinnitus or needing higher volume. Reduced hearing after a shift matters even if it later improves; recovery does not prove that exposure was safe.

## Risk factors

Repeated or impulsive noise, inadequate source control, poorly fitted protection and ototoxic chemical exposure increase risk. Recreational noise and some medicines also contribute, so hearing and occupational-health specialists should review the whole exposure history.

## Diagnosis

Assessment includes ear examination, hearing tests and workplace, recreational and medication history. Comparing periodic audiograms with a baseline helps detect change. Asymmetry or causes such as wax and middle-ear disease need assessment rather than automatic attribution to noise.

## Treatment

Stop harmful exposure through workplace controls and treat other identified causes. Hearing aids, communication devices and tinnitus support may help. Cochlear implants suit selected candidates. No general ear-drop or surgical treatment restores noise-damaged hearing cells.

## Complications

Hearing loss may affect warning-signal detection, conversation, wellbeing and social participation. Permanent damage can remain after changing jobs, but exposure reduction protects remaining hearing. Support includes safer communication and environmental adaptations.

## Prevention

Prioritize quieter equipment, source enclosure, maintenance and reduced exposure, alongside correctly fitted hearing protection. Noise measurement and hearing surveillance should match workplace risks. Ordinary music headphones are not automatically a substitute for occupational hearing protection.

## When to seek care

Sudden hearing loss, especially in one ear, needs urgent assessment rather than waiting for routine workplace testing. Seek care after an explosion or injury, or for one-sided tinnitus or persistent dizziness. Gradual decline also deserves diagnosis and rehabilitation.
