Parathyroid Disorders

## Summary

Parathyroid disorders affect a hormone that regulates calcium and phosphorus. Excess and deficient hormone activity have different causes, tests and treatments. These conditions are distinct from disorders of the thyroid gland itself.

## Overview

The small glands usually sit behind the thyroid. Hormone levels may rise from a primary gland problem or in response to kidney disease or vitamin D deficiency. Deficiency can lower calcium; hormone results must be interpreted alongside calcium and clinical context.

## Causes

A benign adenoma or enlargement of several glands may cause primary hyperparathyroidism; cancer is rare. Deficiency can follow neck surgery or immune or genetic disease. Low magnesium can also interfere with hormone secretion and action.

## Symptoms

Some cases are found on routine blood tests. High calcium may cause thirst, increased urination, constipation, weakness or stones. Low calcium may cause tingling around the mouth or extremities and muscle cramps. Symptoms alone cannot determine the direction of the abnormality.

## Risk factors

Relevant factors include neck surgery, kidney disease, vitamin D deficiency, inherited syndromes and certain medicines. Review supplements and family history. An elevated parathyroid hormone result alone does not establish a tumor requiring surgery.

## Diagnosis

Assessment reviews calcium and parathyroid hormone alongside kidney function, phosphorus, magnesium and vitamin D when indicated. Urine calcium and bone-density testing may help. Gland imaging localizes tissue before planned surgery; it does not replace biochemical diagnosis.

## Treatment

Primary excess may require surgery, or selected monitoring and medicines; secondary excess requires treatment of its cause. Deficiency may need calcium, active vitamin D and specialist options according to response. Severe abnormalities require hospital care. Do not arbitrarily increase supplements or eliminate dietary calcium.

## Complications

Primary excess may contribute to osteoporosis, fractures, kidney stones or impaired kidney function. Low calcium can cause arrhythmias or seizures. Excess replacement can also harm the kidneys, so laboratory monitoring continues even when symptoms improve.

## Prevention

Not all causes are preventable. Calcium follow-up after neck surgery and during kidney disease, and avoiding excessive supplements, help detect problems and reduce complications. Dietary, fluid and supplement requirements are individualized.

## When to seek care

Seizures, breathing difficulty, fainting or marked confusion require emergency care. Report tingling and cramps promptly after thyroid surgery. Severe thirst with vomiting, weakness or dehydration needs urgent calcium assessment.
