## Summary

Constipation in children is a very common problem of hard, infrequent, painful stools, usually due to diet or stool withholding rather than an organic disease. It is treated by increasing fiber and fluids and adjusting habits, sometimes with laxatives under a doctor's guidance, along with reassuring parents.

## Overview

Constipation is one of the most common digestive problems in children and one of the most frequent reasons for visiting a pediatrician. It is defined as infrequent bowel movements with hard stools and difficulty or pain on passing them, and it may be accompanied by abdominal pain, loss of appetite, and involuntary stool soiling in chronic cases.

In the vast majority of cases, constipation is functional, meaning there is no organic disease behind it; rather, it results from factors such as low fiber and fluids in the diet, changes in diet such as switching from breast milk to formula or introducing solid food, or most importantly, the child withholding stool out of fear of pain, being busy with play, or avoiding toilet training, so stool accumulates and hardens and passing it becomes more painful, in a vicious cycle.

The reassuring news for parents is that most childhood constipation is mild and treatable by adjusting diet and habits and does not indicate a serious disease. Treatment relies on softening the stool, breaking the pain-and-withholding cycle, increasing fiber and water, and encouraging the child to use the bathroom regularly without pressure. In some cases the doctor prescribes child-safe laxatives for a set period. Watching for warning signs — such as constipation from birth, weight loss, or blood — remains important to rule out rare organic causes.

## Causes

- Low fiber and fluids in the diet.

- Stool withholding out of fear of pain or distraction.

- Diet changes, weaning, or toilet training.

- Low activity and changes in routine.

## Symptoms

- Bowel movements less often than usual and large hard stools.

- Pain and difficulty passing stool and sometimes a little blood on it.

- Abdominal pain, bloating, and loss of appetite.

- Involuntary stool soiling in chronic cases.

## Risk factors

Low fibre or fluid intake and limited physical activity increase susceptibility to childhood constipation. Certain medicines and neurological or anorectal disorders may contribute and should be considered when symptoms recur. Fear after a painful bowel movement can lead to withholding and a continuing cycle; this should not be treated as wilful behaviour deserving punishment.

## Diagnosis

- Usually a clinical diagnosis from history and examination.

- Assessment of diet and bowel habits.

- Additional tests only when there are warning signs of organic causes.

## Treatment

### Adjusting diet and habits

- Increasing fiber (vegetables, fruit, whole grains) and water.

- Encouraging regular bathroom visits after meals without pressure or punishment.

- More movement and play.

### Medication

- Child-safe laxatives by the doctor's prescription for a period to break the constipation cycle.

This content is educational and does not replace consulting a pediatrician; medications are at doctor-determined doses.

## Complications

- Anal fissures and recurrent pain.

- Involuntary stool soiling that affects the child psychologically.

- Worsening of the withholding-and-pain cycle if neglected.

## Prevention

- A fiber-rich diet and adequate water intake.

- A regular bathroom routine and gentle, non-forced training.

- Regular physical activity.

## When to seek care

See a pediatrician if constipation persists despite diet changes, or there is severe pain, blood, weight loss, constipation from birth, or marked abdominal bloating. **Book an appointment with a pediatrician on ClinicsJo**.

## Are laxatives safe for children?

Doctor-prescribed laxatives at appropriate doses are safe and effective to break the constipation cycle and do not cause dependence when used correctly.

## Does cow's milk affect constipation?

Excess cow's milk may worsen constipation in some children; a balanced diet with more fiber and water helps.
