Pemphigus Vulgaris

## Summary

Pemphigus vulgaris is a non-contagious autoimmune disease causing blisters and erosions of the skin or mucous membranes, especially the mouth. Specialist treatment controls immune activity while protecting the skin and nutrition.

## Overview

Persistent mouth sores may precede skin blisters. Pemphigus vulgaris is one form of a group of blistering diseases; most mouth ulcers are not pemphigus. Treatment can control the disease and achieve remission, with continued monitoring for relapse and treatment effects.

## Causes

Antibodies attack proteins that hold skin cells together, allowing blisters to form. The trigger is not always known; genetic susceptibility or certain medicines may contribute. The condition is not spread by touching someone or sharing their surroundings.

## Symptoms

Fragile blisters can break quickly and leave painful, raw skin. Mouth erosions may interfere with eating and toothbrushing. Other mucosal surfaces, including the nose or genitals, may be affected. Painful swallowing, spreading sores and infection signs need attention.

## Risk factors

The disease often begins in adulthood, and genetic susceptibility varies between populations. A family history does not make it inevitable. Give your clinician a complete medication list when symptoms follow a new medicine, without stopping essential treatment without advice.

## Diagnosis

A dermatologist examines skin and mouth lesions. A biopsy may assess tissue changes and antibody deposits, while blood antibody testing can support diagnosis and monitoring. Similar-looking blistering disorders mean that a photograph alone cannot reliably establish the diagnosis.

## Treatment

Treatment may include corticosteroids and immune-directed medicines such as rituximab or other agents, chosen according to severity and circumstances. Infection risk and adverse effects require monitoring. Wound care, pain relief and nutrition also matter. Do not stop or taper prescribed medicines yourself.

## Complications

Open skin may become infected or lose fluid. Mouth involvement can make eating difficult and cause nutritional problems. Some complications result from immune-suppressing medicines, so follow-up includes appropriate testing and prevention of medication-related harm.

## Prevention

There is no proven way to prevent its onset. After diagnosis, follow wound-care advice, reduce skin friction and avoid foods that aggravate mouth sores. Use a soft toothbrush. Discuss vaccination and infection precautions before and during immune-suppressing treatment.

## When to seek care

Arrange assessment for persistent mouth sores or blisters. Seek urgent care if you cannot drink, pass less urine, develop rapidly spreading erosions, or have fever with painful redness or pus. Confusion or breathing difficulty requires emergency care.
