## Summary

Pinworm infection is a very common intestinal infection, especially in children, caused by small white worms that live in the intestine and lay eggs around the anus at night, causing intense nighttime anal itching. It is contagious and spreads easily, treated with a simple medication for the child and family along with strict hygiene.

## Overview

Pinworm infection is among the most common intestinal worm infections in the world, especially among school- and nursery-age children. It is caused by a small, thin white worm resembling a thread that lives in the large intestine, and its female emerges at night to the anal area to lay thousands of eggs around the anal opening.

This nighttime behavior is the cause of the infection's most prominent symptom: intense nighttime anal itching that bothers the child and disturbs their sleep. The infection spreads very easily because the eggs are microscopic and sticky: when the child scratches the area, the eggs stick to their fingers and under the nails, then transfer to the mouth directly or through food, toys, surfaces, and clothing, and are swallowed, completing the infection cycle. It therefore spreads quickly among members of one family and children in crowded places.

Despite being widespread and bothersome, pinworms are usually not dangerous and do not cause serious diseases, and the good news is that they are easy to treat. Treatment relies on an anti-worm medication prescribed by the doctor, usually given as a dose repeated after two weeks to eliminate worms hatching from remaining eggs, and treating all family members at the same time is advised to prevent reinfection. Treatment is complemented by strict hygiene: frequent handwashing, nail trimming, washing clothes and bedding in hot water, and avoiding scratching the area. It is important to know that infection does not mean poor hygiene, as it is very common and spreads easily.

## Causes

- Infection with the pinworm (Enterobius).

- Swallowing microscopic eggs via hands, food, surfaces, and clothing.

- Easy transmission among children and family members.

## Symptoms

- Intense anal itching, especially at night.

- Disturbed sleep and irritability in the child.

- Sometimes seeing thin white worms around the anus or in the stool.

- Vaginal itching in girls sometimes.

## Risk factors

Pinworm infection is common in school-age children and close household, childcare or crowded-setting contacts. Eggs adhere to hands, nails and surfaces; scratching followed by hand-to-mouth contact can cause reinfection. Infestation does not imply personal neglect; management of contacts and hygiene measures should follow appropriate assessment.

## Diagnosis

- Clinical diagnosis from the characteristic nighttime itching.

- An adhesive-tape test around the anus in the morning to detect eggs.

- Sometimes seeing the worms directly.

## Treatment

### Anti-worm medication

- A doctor-prescribed medication given as a dose repeated after two weeks to eliminate hatching worms.

- Treating all family members at the same time to prevent reinfection.

### Complementary hygiene

- Frequent handwashing and nail trimming.

- Washing clothes and bedding in hot water and avoiding scratching the area.

This content is educational and does not replace medical advice.

## Complications

- Disturbed sleep and irritability from itching.

- Skin inflammation around the anus from scratching.

- Repeated reinfection if hygiene and family treatment are not followed.

## Prevention

- Washing hands after the toilet and before eating and trimming nails.

- Avoiding scratching the area and nail-biting.

- Washing underwear and bedding in hot water and changing underwear daily.

- Treating all family members together when infected.

## When to seek care

See a doctor for recurrent nighttime anal itching in your child or seeing thin white worms, or if the infection recurs despite treatment. **Book an appointment with a pediatrician on ClinicsJo** to prescribe the right medication and a reinfection-prevention plan.

Published questions

## Does pinworm infection mean poor hygiene?

No, it is very common and spreads very easily among children even with good hygiene; infection does not indicate neglect but needs treatment and strict hygiene.

## Why is the whole family treated?

Because the infection spreads easily among family members, and treating the infected person alone may lead to reinfection from them, so treating everyone together is advised.

## Why is the medication dose repeated after two weeks?

Because the medication kills the worms but not all eggs, so the dose is repeated to eliminate worms hatching from remaining eggs and prevent the infection's return.
