Plantar Fasciitis

## Summary

Plantar fasciitis is the most common cause of heel pain, resulting from irritation of the thick ligament running under the foot. Its characteristic sign is sharp heel pain with the first steps in the morning that eases with movement. Most of it improves with exercises, stretching, supportive shoes, and relative rest, and it rarely needs advanced interventions.

## Overview

Plantar fasciitis is the most common cause of heel pain. The plantar fascia is a thick ligament of fibrous tissue running along the sole of the foot from the heel bone to the front of the foot, acting like a tendon that supports the foot's arch and absorbs shocks during walking. When this ligament is exposed to repeated strain or excess stretch, irritations and small areas of damage occur in it near its attachment to the heel bone, producing pain.

The most characteristic sign of plantar fasciitis is sharp pain in the bottom of the heel that is most severe with the first steps in the morning on getting out of bed, or after long periods of sitting, then gradually eases with walking and movement, but may return after prolonged standing or walking. Among the most prominent factors that increase its risk are: standing or walking for long hours on hard surfaces, obesity and weight gain, wearing unsupportive or flat shoes, tightness in the Achilles tendon and calf muscles, and a sudden excess of exercise or running. It is common among those whose work requires prolonged standing, athletes, and the middle-aged.

The good news is that the vast majority of cases improve with conservative treatment without needing surgery, although recovery may take weeks to months and requires patience and commitment. Treatment rests on several axes: relative rest and reducing activities that increase pain, regular stretching exercises for the fascia, Achilles tendon, and calf muscles (especially in the morning), applying ice after activity, wearing supportive shoes with a comfortable sole and sometimes orthotic insoles to support the foot's arch, anti-inflammatory painkillers when needed, and weight loss if there is excess. Physical therapy is very effective. Resistant cases that do not improve after months may need additional interventions determined by the doctor, and surgery is very rare.

## Causes

- Repeated strain and excess stretch on the plantar fascia.

- Prolonged standing or walking on hard surfaces.

- Obesity, and unsupportive or flat shoes.

- Tightness of the Achilles tendon and calf muscles, and a sudden excess of exercise.

## Symptoms

- Sharp pain in the bottom of the heel.

- Most severe with the first steps in the morning or after prolonged sitting.

- Eases gradually with movement and may return after prolonged standing or walking.

## Risk factors

Repeated heel loading, excess body weight and prolonged standing or walking on hard surfaces increase plantar-fasciitis risk. Foot-arch shape, gait and worn or unsuitable footwear affect load distribution. The condition can occur without an obvious risk factor, and a heel spur does not by itself prove the source of pain.

## Diagnosis

- Diagnosis is often clinical from the pain pattern, location, and examination.

- No imaging is needed in most cases.

- X-ray or ultrasound when another cause is suspected or there is no improvement.

## Treatment

### Conservative treatment (sufficient for most cases)

- Relative rest and reducing painful activities.

- Regular stretching exercises for the fascia, Achilles tendon, and calf muscles (especially in the morning).

- Applying ice after activity, and supportive shoes and orthotic insoles for the foot's arch.

- Anti-inflammatory painkillers when needed, weight loss, and physical therapy.

### Resistant cases

- Additional interventions determined by the doctor, and surgery is very rare.

Recovery may take weeks to months. This content is educational and does not replace medical advice.

## Complications

- Pain becoming chronic when treatment is neglected.

- Changing the way of walking, which may cause pain in the knee, hip, or back.

- Impact on daily activity and work.

## Prevention

- Wearing comfortable supportive shoes and avoiding flat or worn ones.

- Maintaining a healthy weight and stretching calf muscles before exercise.

- Increasing exercise intensity gradually and avoiding prolonged standing on hard surfaces.

## When to seek care

See a doctor if heel pain persists despite rest, exercises, and supportive shoes for several weeks, or is severe and disrupts walking, or is accompanied by swelling, redness, or numbness. **For persistent cases, book with an orthopedic specialist or physiotherapist on ClinicsJo**.

Published questions

## Why is heel pain most severe in the morning?

Because the plantar fascia contracts during sleep, so with the first morning steps it suddenly stretches and the pain increases, then eases gradually with movement and warming up.

## Does plantar fasciitis need surgery?

Very rarely; the vast majority of cases improve with conservative treatment (stretching, supportive shoes, relative rest, and physical therapy), and surgery is reserved for very resistant cases.

## How long does recovery from plantar fasciitis take?

It may take weeks to months and requires patience and commitment to exercises and supportive shoes; most cases improve without surgery.
