Prader-Willi Syndrome

## Summary

Prader-Willi syndrome is a genetic disorder affecting development, hormones, behavior and appetite. Infants may have low muscle tone and feeding difficulty before persistent hunger develops later, requiring coordinated lifelong support.

## Overview

Needs change with age: early feeding difficulty differs from later hyperphagia. Safe, structured access to food is part of care rather than punishment or evidence of poor parenting.

## Causes

The condition involves absent activity of particular paternal genes in a region of chromosome 15 through different mechanisms. Most cases lack a family history. Genetic investigation clarifies the mechanism and recurrence risk.

## Symptoms

Features may include low muscle tone, developmental delay, short stature, sleep problems, learning or behavioral difficulties and later excessive hunger. Severity varies, and not every person has every feature.

## Risk factors

This genetic condition affects both sexes and is not caused by maternal diet or child behavior. Recurrence risk depends on the genetic mechanism; counseling helps families considering another pregnancy.

## Diagnosis

Clinical suspicion is confirmed through specialized genetic testing. Further assessment covers growth, hormones, sleep-related breathing and nutrition. Obesity alone cannot diagnose the syndrome.

## Treatment

Care individualizes nutrition, activity, learning and behavioral support. Growth hormone requires specialist assessment and monitoring, including breathing. Extended-release diazoxide choline has a US indication for hyperphagia in selected patients, with glucose and fluid-retention monitoring; this does not establish availability in Jordan.

## Complications

Potential complications include severe obesity, diabetes, sleep-disordered breathing, scoliosis and bone problems. Regular monitoring and psychological and social support help reduce the overall burden.

## Prevention

Diet cannot prevent the genetic alteration, but consistent food routines, adequate nutrition and coordination between home and school reduce risk. A fixed calorie prescription is inappropriate across ages; a dietitian personalizes requirements.

## When to seek care

Poor feeding or growth warrants early review. Vomiting, abdominal swelling or new pain needs urgent assessment even if apparently mild, because pain perception may differ. Choking or severe breathing difficulty is an emergency.
