Pyoderma Gangrenosum

## Summary

Pyoderma gangrenosum is a rare inflammatory disease causing painful, sometimes rapidly enlarging ulcers. Despite its name, it is not primarily a bacterial infection and needs specialist assessment.

## Overview

A small bump may become an ulcer with an inflamed or purple edge. Skin injury or procedures can worsen lesions, a reaction known as pathergy.

## Causes

Abnormal immune activity is involved. Associated conditions include inflammatory bowel disease, inflammatory arthritis or blood disorders, although no associated illness is found in some cases.

## Symptoms

Severe pain, enlarging ulceration and discharge may occur, commonly on legs or around wounds or stomas. Early pain may seem disproportionate to the lesion’s size.

## Risk factors

Inflammatory or blood disorders and ulcers worsening after trauma raise suspicion. The condition is not contagious and does not automatically indicate poor hygiene.

## Diagnosis

Diagnosis combines appearance, history, biopsy and appropriate tests to exclude infection, vasculitis or cancer. A wound swab or photograph alone cannot establish it.

## Treatment

Gentle wound care, pain management and anti-inflammatory or immunosuppressive treatment are selected by severity. Secondary infection is treated when present; antibiotics alone do not address the underlying inflammation.

## Complications

Scarring, secondary infection, persistent pain and impaired mobility may occur. Recurrences are possible, and treatment requires monitoring for adverse effects.

## Prevention

Avoid manipulating ulcers and inform clinicians before skin procedures. Wide surgical debridement can worsen active disease; specialists decide whether and when an intervention is appropriate.

## When to seek care

Rapid ulcer expansion, severe pain or fever needs urgent review. Do not self-start steroids because infection can look similar and requires different treatment.
