Spinal Muscular Atrophy

## Summary

Spinal muscular atrophy is a genetic disorder affecting motor neurons and causing variable muscle weakness. It can begin in infancy or later and needs early diagnosis, treatment planning and support for breathing, nutrition and mobility.

## Overview

Chromosome-5-associated SMA is the most common form; rarer forms have other causes. Age at onset and motor abilities help describe disease, but modern therapies have changed its course. A type label alone cannot predict a child’s future.

## Causes

The common form involves SMN1 gene changes and insufficient protein needed by motor neurons. SMN2 copy number influences severity without determining every outcome. Parents often carry the variant without symptoms.

## Symptoms

Signs include floppy or weak limbs, difficulty controlling the head or sitting, delayed movement or lost skills, and swallowing or breathing problems. Some forms begin later in life. Intelligence is generally unaffected by SMA itself.

## Risk factors

Family history and carriage of particular variants increase the likelihood of common inherited SMA. When both parents carry a causative recessive variant, inheritance probabilities apply to each pregnancy. Counselling should use the actual test results and disease form.

## Diagnosis

Clinical assessment and genetic testing establish the common form; inconclusive cases may require additional neurological investigations. Breathing, swallowing, nutrition and movement are assessed. Family or prenatal testing should include genetic counselling.

## Treatment

Disease-modifying treatments such as nusinersen, risdiplam and some gene therapies can improve outcomes in eligible patients. Choice depends on age, disease form, clinical status, eligibility and local availability. Early treatment matters but does not guarantee a complete cure or replace respiratory, nutritional and rehabilitation support.

## Complications

Complications include chest infections, respiratory failure, poor nutrition, joint contractures and spinal curvature. Monitoring remains necessary even when movement improves with treatment. Care also supports education, communication, independence and family wellbeing.

## Prevention

Exercise cannot prevent the genetic change. Genetic counselling explains reproductive and testing options. Following diagnosis, vaccination, respiratory care, positioning, nutrition and safe activity help limit complications while avoiding excessive muscle fatigue.

## When to seek care

Breathing difficulty, blue discoloration or severe choking is an emergency. Weak cough, repeated choking during feeds or loss of motor skills needs prompt assessment. Respiratory support must not wait for all diagnostic results.
