(Stenosis) — Spinal Stenosis

## Summary

Spinal stenosis is a narrowing of the canal that contains the spinal cord and nerves in the spine, often due to age-related changes, pressing on the nerves. It most often affects the neck and lower back. Its symptoms are pain, numbness, and weakness in the limbs that increase with walking or standing and ease with bending or sitting. It is managed with physical therapy and medications, and sometimes surgery.

## Overview

Spinal stenosis is a condition in which the bony passage running through the spine and containing the spinal cord and the nerve roots branching from it narrows. With this narrowing, pressure occurs on the spinal cord or nerves, causing neurological symptoms. It most often affects the lower back (lumbar stenosis) and the neck (cervical stenosis). In most cases the cause is degenerative changes related to aging, such as joint arthritis, ligament thickening, bone spurs, and disc herniation, so it is common among older adults.

The symptoms of spinal stenosis differ by its location. In lumbar stenosis (lower back), a characteristic sign is pain, numbness, tingling, or weakness in the legs and buttocks that increases with walking or standing for a while, and clearly eases when sitting or bending forward (so the patient tends to bend or lean on a shopping cart, for example), a phenomenon known as neurogenic claudication. Cervical stenosis (neck) may cause pain, numbness, and weakness in the arms and hands, and in advanced cases may affect balance, walking, and the precision of hand movements. The stenosis may be mild and symptomless and found incidentally.

Spinal stenosis is diagnosed by symptoms, neurological examination, and imaging (especially MRI). Treatment depends on the severity of symptoms and the degree of pressure on the nerves. Most cases begin with conservative treatment: physical therapy and strengthening and stretching exercises, painkillers and anti-inflammatories, activity modification, and sometimes local injections to relieve inflammation and pain. Many patients improve with these measures. Severe cases that do not improve, or that cause increasing weakness or an effect on balance or control of urine or stool (a serious sign), may need surgery to widen the canal and relieve pressure on the nerves. It is important to see a doctor for persistent neurological symptoms in the limbs, as early evaluation helps choose appropriate treatment and prevent symptom worsening.

## Causes

- Age-related degenerative changes (joint arthritis, ligament thickening, bone spurs).

- Disc herniation.

- Sometimes congenital causes or injuries.

## Symptoms

- Lumbar stenosis: pain, numbness, and weakness in the legs increasing with walking and standing and easing with sitting and bending.

- Cervical stenosis: pain, numbness, and weakness in the arms and hands, and affected balance in advanced cases.

- May be mild and symptomless.

## Risk factors

Age is a major factor: spinal stenosis is common after 50 alongside degenerative change. It can occur earlier with structural abnormalities or conditions affecting the spine. Narrowing on imaging alone does not determine severity; findings need correlation with pain, walking ability and neurological examination.

Published questions

## Why does spinal stenosis pain ease with sitting or bending?

Because bending forward or sitting slightly widens the spinal canal and relieves pressure on the nerves, while walking and standing narrow it so the pain increases; this is a characteristic pattern of lumbar stenosis.

## Does spinal stenosis always need surgery?

No, most cases begin with conservative treatment (physical therapy, medications, and activity modification) and many improve; surgery is reserved for severe cases that do not improve or cause increasing weakness.

## What signs require emergency care in spinal stenosis?

Rapidly increasing limb weakness, loss of control of urine or stool, or noticeably affected balance and walking — these are signs of serious nerve pressure requiring urgent evaluation.
