Vocal Fold Nodules

## Summary

Vocal-fold nodules are benign callus-like lesions often related to repeated vocal strain. They cause hoarseness and fatigue and require laryngeal examination for diagnosis. Voice therapy and habit changes are central for many patients.

## Overview

Nodules usually occur on both folds where repeated contact takes place. They differ from polyps, cysts and hemorrhage. Listening to the voice or knowing a person’s occupation cannot reliably distinguish these lesions.

## Causes

Repeated forceful voice use, including shouting or talking over noise, can injure the surface. Other irritants may contribute. Not every hoarse singer or teacher has nodules.

## Symptoms

Symptoms include rough or breathy voice, speaking fatigue and difficulty reaching familiar pitches. Repeated throat clearing may occur. The degree of hoarseness does not by itself measure lesion size.

## Risk factors

Teachers, singers and children who frequently raise their voices may experience higher vocal loads. Group membership does not establish diagnosis; training is adapted to individual demands and contributing factors.

## Diagnosis

A laryngeal specialist visualizes the folds and may assess their vibration with specialized imaging. A voice therapist then evaluates use and goals following medical diagnosis, so conditions needing different care are not missed.

## Treatment

Individual voice therapy reduces strain and addresses confirmed irritants. Voice rest does not mean permanent silence or forceful whispering. Surgery is reserved for selected situations after assessment and conservative-care response, not offered automatically.

## Complications

Communication difficulties can persist and affect work and confidence. Continued vocal strain can prolong symptoms or promote recurrence. Potential surgical scarring and voice changes must be discussed before choosing a procedure.

## Prevention

Pace speaking, take breaks and consider amplification rather than shouting. Avoid smoking and repeated vocal forcing, and maintain hydration. Follow professionally selected exercises rather than unknown routines that may increase tension.

## When to seek care

Seek assessment when hoarseness persists or affects work or school. Sudden voice loss after shouting may reflect hemorrhage rather than a nodule and needs review. Breathing or swallowing difficulty and a neck lump warrant prompt care.
