Eosinophilic Cellulitis (Wells Syndrome)

## Summary

Wells syndrome, or eosinophilic cellulitis, is a rare inflammatory skin disorder that can resemble bacterial infection. Assessment is needed before automatically prescribing antibiotics or steroids.

## Overview

Lesions may recur and leave temporary pigment changes. The name does not establish bacterial infection, although a genuine infection can mimic or coexist with it.

## Causes

Cases may be unexplained or associated with medicines, insect bites or another illness. A reported association does not prove an individual trigger.

## Symptoms

Red swollen areas may itch or burn and form plaques or blisters. Limbs and trunk can be affected, with episodes resolving and recurring.

## Risk factors

Different age groups can be affected. Universal cancer screening is not indicated for every case; history, examination and persistent findings guide targeted investigation.

## Diagnosis

Assessment reviews medicines and exposures and may include skin biopsy. Blood eosinophilia or particular microscopic patterns support evaluation but are not diagnostic alone.

## Treatment

Dermatology-directed care may include topical or systemic steroids and other options for recurrent disease. Treat confirmed infection and review triggers rather than repeating antibiotics without evidence.

## Complications

Recurrence, pigment change or occasional secondary infection may occur. Persistent marked eosinophilia or other-organ symptoms requires investigation for broader causes.

## Prevention

There is no guaranteed prevention. Avoid a confirmed trigger in consultation with the clinician, protect the skin and do not stop essential medication on suspicion alone.

## When to seek care

Rapidly spreading rash with fever, severe pain or extensive blistering needs urgent assessment. Breathlessness, facial swelling or marked deterioration requires emergency care.
