# Xanax (Alprazolam) — English

Xanax contains alprazolam, a short- to intermediate-acting benzodiazepine that strengthens the inhibitory action of gamma-aminobutyric acid (GABA) in the brain. It is used in adults for the acute treatment of generalized anxiety disorder and for panic disorder, with or without agoraphobia. It is a controlled medicine with boxed warnings about combined use with opioids, abuse and addiction, and dependence with withdrawal reactions.

## Overview

The active ingredient of Xanax is alprazolam, a 1,4 benzodiazepine (ATC code N05BA12). Benzodiazepines are central nervous system depressants with calming, anti-anxiety, muscle-relaxing and anticonvulsant properties. The catalog product is an oral tablet containing 0.5 mg of alprazolam, and the same active ingredient is also marketed in 0.25 mg, 1 mg and 2 mg tablets.

Alprazolam is approved in adults for the acute treatment of generalized anxiety disorder and for panic disorder with or without agoraphobia (fear of situations from which escape may be difficult). It is not approved for chronic insomnia or for everyday stress, and it is not licensed for children. For long-term management of anxiety disorders, antidepressants such as selective serotonin reuptake inhibitors and psychological therapy such as cognitive behavioural therapy are generally preferred. A benzodiazepine is therefore used for the shortest time that is clinically needed, often while those treatments take effect, and the need for continued treatment should be reassessed frequently.

Xanax is a prescription-only medicine and is a controlled medicine in Jordan, so it can be supplied only against a valid prescription and in the quantity the prescriber has set. The official US labelling carries a boxed warning that concomitant use with opioids may cause profound sedation, respiratory depression, coma and death. The boxed warning also states that benzodiazepines expose users to abuse, misuse and addiction, and that continued use may cause physical dependence with withdrawal reactions that can be life-threatening. The former US pregnancy category of alprazolam was D.

## Mechanism of Action

Alprazolam binds to the benzodiazepine site on the gamma-aminobutyric acid type A (GABA-A) receptor, a chloride channel on nerve cells. GABA is the main inhibitory neurotransmitter of the brain. Binding of alprazolam does not open the channel by itself but makes the receptor respond more strongly to GABA, so chloride entry increases and nerve cells become less excitable.

This enhanced inhibition in the brain circuits that regulate fear, arousal and the stress response reduces the intensity of anxiety and the frequency and severity of panic attacks. The same mechanism explains the sedation, impaired coordination and memory effects, and the respiratory depression that can occur when alprazolam is combined with other sedating substances. With repeated use the brain adapts to the continuous enhancement of GABA activity, which underlies tolerance, dependence and withdrawal when the dose is reduced or stopped abruptly.

## Pharmacokinetics

- **Absorption:** Alprazolam is well absorbed after oral administration, and plasma levels rise in proportion to the dose over the range of 0.5 to 3.0 mg.
- **Onset and peak effect:** The peak plasma concentration is reached 1 to 2 hours after a dose. Calming effects are therefore usually noticed within the first hour or so and are greatest around the time of peak levels.
- **Distribution:** About 80% of alprazolam in the blood is bound to serum proteins, mainly albumin. It crosses the placenta and passes into breast milk.
- **Metabolism:** Alprazolam is extensively metabolized in the liver, mainly by the enzyme cytochrome P450 3A4 (CYP3A4). Its two main active metabolites, 4-hydroxyalprazolam and alpha-hydroxyalprazolam, circulate at less than 4% of the level of the parent drug and contribute little to its effect.
- **Elimination half-life:** The mean elimination half-life is about 11.2 hours in healthy adults, with a range of 6.3 to 26.9 hours. It is longer in healthy older adults (about 16.3 hours), in obese patients (about 21.8 hours) and in patients with alcoholic liver disease (about 19.7 hours).
- **Excretion:** Alprazolam and its metabolites are excreted mainly in the urine.

## Indications

- **Generalized anxiety disorder (acute treatment):** Alprazolam is used in adults for the acute treatment of generalized anxiety disorder, a condition of persistent and excessive worry with physical tension. Treatment should be as short as possible and its continued need reviewed often.
- **Panic disorder with or without agoraphobia:** It is used in adults to reduce the frequency and severity of panic attacks and the avoidance behaviour that often follows. The duration of treatment needed in patients who respond is unknown, and a supervised gradual withdrawal may be attempted after a prolonged period without attacks.

## Dosage and Administration

The doses below are the usual doses in the official US labelling for adults. The prescriber sets the actual dose according to the diagnosis, response and tolerability, and the lowest effective dose for the shortest necessary time should be used.

- **Adults, generalized anxiety disorder:** The usual starting dose is 0.25 mg to 0.5 mg three times daily, which may be adjusted every 3 to 4 days according to response, up to a maximum of 4 mg daily in divided doses.
- **Adults, panic disorder:** The usual starting dose is 0.5 mg three times daily, increased at intervals of 3 to 4 days by no more than 1 mg per day. Controlled trials used 1 mg to 10 mg daily, with a mean of about 5 mg to 6 mg, and doses above 4 mg daily should be periodically reassessed with a view to reduction.
- **Older adults and debilitated patients:** A lower starting dose of 0.25 mg two or three times daily is usual, with gradual increases only if needed and tolerated.
- **Patients with advanced liver disease:** The usual starting dose is also 0.25 mg two or three times daily, increased cautiously.
- **Patients taking ritonavir:** The labelling gives a specific dosage recommendation for this combination, so the prescriber should follow the full prescribing information or the product leaflet.
- **Stopping or reducing treatment:** The dose should be reduced gradually by no more than 0.5 mg every 3 days, and some patients need an even slower reduction, with the taper paused or the previous dose resumed if withdrawal reactions appear.
- **Children and adolescents:** No dose is established because the safety and effectiveness of alprazolam in patients under 18 years have not been demonstrated.

**How to take it:**
- Xanax tablets are swallowed with water and may be taken with or without food, at regular times spread through the day when a daily regimen has been prescribed.
- The tablet should not be taken more often or in larger amounts than prescribed, because misuse of benzodiazepines is a recognised route to overdose and addiction.
- Treatment should not be stopped suddenly after regular use. The dose must be lowered step by step as directed by the prescriber.

**If you miss a dose:** A missed dose should be taken as soon as it is remembered unless it is almost time for the next dose, in which case the missed dose is skipped. The dose should never be doubled to make up for a missed one.

**Overdose:** Overdose causes central nervous system depression ranging from drowsiness, confusion, slurred speech and poor coordination to low muscle tone, respiratory depression and coma, and it may be fatal, particularly with alcohol or opioids. It is a medical emergency that requires immediate emergency care.

## Pregnancy, Breastfeeding and Special Groups

- **Pregnancy:** Published observational studies have not shown a clear link between benzodiazepines and major birth defects. Use late in pregnancy or during labour can cause sedation, breathing difficulty, low muscle tone, feeding problems and withdrawal symptoms in the newborn, who should be monitored. Alprazolam is used in pregnancy only when the benefit clearly outweighs the risk, and the patient should consult her doctor before stopping or continuing it.
- **Breastfeeding:** Alprazolam passes into breast milk and may cause sedation and feeding difficulty in the infant. The US labelling states that breastfeeding is not recommended during treatment.
- **Children:** Safety and effectiveness have not been established in patients under 18 years, so alprazolam is not recommended in this age group.
- **Older adults:** Older adults have higher blood levels because of reduced clearance, and their half-life is longer, so a lower dose is recommended. They are at greater risk of drowsiness, confusion, falls and fractures.
- **Kidney impairment:** The labelling gives no specific dose adjustment, but alprazolam and its metabolites are excreted mainly in the urine. Treatment should begin at a low dose with careful monitoring for excessive sedation.
- **Liver impairment:** Clearance is reduced in liver disease and the half-life is prolonged, so the starting dose in advanced liver disease is 0.25 mg two or three times daily. Some national product information lists severe hepatic insufficiency as a contraindication, so the prescriber should assess the individual patient.

## Contraindications

- Xanax must not be used in patients with known hypersensitivity to alprazolam or to other benzodiazepines, and angioedema (swelling of the face, lips or throat) has been reported.
- It must not be used with strong inhibitors of the enzyme CYP3A, such as ketoconazole and itraconazole, because they markedly raise alprazolam levels. Ritonavir is an exception, and its use with alprazolam is governed by specific dosing advice in the full labelling.
- In European and other national product information for benzodiazepines, it is also contraindicated in myasthenia gravis, severe respiratory insufficiency including severe sleep apnoea, and acute narrow-angle glaucoma.

## Warnings and Precautions

- **Boxed warning: combined use with opioids and other sedatives:** Taking alprazolam with opioids, including tramadol and codeine, may cause profound sedation, respiratory depression, coma and death. Such combined prescribing is reserved for patients without adequate alternatives, with the lowest doses and shortest duration and close monitoring, and alcohol and other sedating medicines add to the same risk.
- **Boxed warning: abuse, misuse and addiction:** Even at recommended doses, alprazolam carries a risk of abuse, misuse and addiction that can lead to overdose or death. The risk is assessed before and during treatment and is higher with a history of substance use disorder, so tablets must never be shared or used for another person.
- **Boxed warning: dependence and withdrawal reactions:** Continued use may lead to physical dependence, and the risk rises with longer treatment and higher daily doses. Abrupt discontinuation or rapid dose reduction may precipitate life-threatening withdrawal reactions, including seizures, so a gradual supervised taper is always required.
- **Driving and operating machinery:** Alprazolam impairs alertness, judgement and coordination. Patients should not drive or operate machinery until they know how it affects them, and should avoid alcohol and other central nervous system depressants.
- **Depression and suicidal thoughts:** Caution is needed in patients with signs of depression, because anxiety and depression often coexist and suicidal thinking may emerge or worsen. The smallest feasible number of tablets should be prescribed, and new or worsening low mood or suicidal thoughts should be reported urgently.
- **Respiratory impairment:** Patients with chronic lung disease or sleep-related breathing disorders are at greater risk of respiratory depression. Alprazolam should be used with great care in these patients and at low doses.
- **Mania, paradoxical reactions and falls:** In patients with depression it may activate mania or hypomania, and paradoxical reactions such as agitation, irritability or aggression may occur. Older adults are particularly liable to dizziness, confusion and falls.
- **Pregnancy and the newborn:** Use during pregnancy, particularly late in pregnancy, can cause neonatal sedation or a neonatal withdrawal syndrome. A woman who becomes pregnant, or plans to, should contact her doctor promptly rather than stopping abruptly.

## Side Effects

**Common side effects:**
- Drowsiness, tiredness and light-headedness are frequent, especially at the start of treatment or after an increase in dose, and usually lessen with time.
- Impaired coordination and unsteadiness occur and may cause falls, so caution is needed when walking or climbing stairs.
- Slurred speech (dysarthria) is among the most frequent effects in clinical trials.
- Low blood pressure may cause dizziness when standing up, and rising slowly helps.
- Memory impairment, difficulty concentrating and confusion can occur, particularly at higher doses.
- Dry mouth, constipation, changes in appetite and weight, and headache are reported.
- Changes in sexual interest, most often an increase in libido, have been reported.
- Irritability or blurred vision may occur, and persistent symptoms should be discussed with the doctor.

**Side effects that need urgent medical advice:**
- Respiratory depression with slow or shallow breathing, extreme drowsiness or loss of consciousness requires emergency medical care, particularly when alprazolam has been combined with alcohol or an opioid.
- Withdrawal reactions after dose reduction or sudden discontinuation include rebound anxiety, insomnia, tremor, sweating and, in severe cases, seizures and confusion, and urgent medical help is needed.
- Angioedema, with swelling of the face, lips, tongue or throat or difficulty breathing, is an allergic reaction for which the medicine must be stopped and emergency care sought.
- New or worsening depression or suicidal thoughts need immediate medical attention.
- Paradoxical reactions such as marked agitation, aggression, impulsivity or violent behaviour should be reported, and the medicine should not be continued without medical advice.
- Signs of abuse or addiction, such as taking more than prescribed or craving the tablets, should prompt a request for medical help.

## Drug Interactions

- **Opioids (morphine, codeine, tramadol, methadone, oxycodone, fentanyl and others):** Combined use increases the risk of profound sedation, respiratory depression, coma and death because the two classes act on different receptors that control breathing. It should be avoided where possible, and otherwise doses and duration must be minimised under close monitoring.
- **Alcohol and other central nervous system depressants:** Alcohol, sedating antihistamines, antipsychotics, antidepressants with sedative action, anticonvulsants, sleeping medicines and other benzodiazepines add to the sedative effect. Alcohol should be avoided entirely during treatment.
- **Strong CYP3A inhibitors (ketoconazole, itraconazole):** These antifungals sharply increase alprazolam levels and are contraindicated with it. Ritonavir is treated separately in the labelling, which gives specific dosing advice.
- **Other CYP3A4 inhibitors (fluvoxamine, nefazodone, cimetidine, diltiazem, oral contraceptives, grapefruit juice):** These can raise alprazolam levels and increase sedation, so a lower dose may be needed. Grapefruit juice should be avoided during treatment.
- **CYP3A inducers (carbamazepine, phenytoin, rifampicin, St John's wort):** They lower alprazolam levels and may reduce its effect. The prescriber may need to adjust treatment.
- **Digoxin:** Alprazolam can raise digoxin concentrations and increase the risk of digoxin toxicity. Blood levels and signs of toxicity should be monitored.
- **Selective serotonin reuptake inhibitors and other antidepressants:** Alprazolam is commonly given alongside antidepressants for a short period and there is no general prohibition. Fluvoxamine and nefazodone raise alprazolam levels, and additive sedation may occur.

## Food, Alcohol and Daily Life

- Alcohol must be avoided completely during treatment because it increases sedation and the risk of life-threatening respiratory depression.
- Driving, cycling and operating machinery should be avoided until the individual response is known, because alertness and reaction time are reduced. Grapefruit juice may raise the level of alprazolam in the blood and should be avoided, and heavy smoking may lower it.
- Regular sleep, physical activity, limiting caffeine and psychological techniques for anxiety help to reduce the need for medication.

## Storage

- Store the tablets at 20 to 25 degrees Celsius in a tight, light-resistant container, protected from moisture.
- Keep out of the sight and reach of children and store securely, because this is a controlled medicine. Unused tablets should be returned to a pharmacy for proper disposal rather than kept or shared.

## Advice for Patients

- Agree with the prescriber in advance how long the treatment will last, and attend the planned reviews instead of requesting repeated prescriptions.
- Take the tablets exactly as prescribed and never increase the dose because the effect seems weaker, since this may be a sign of tolerance.
- Tell the doctor and pharmacist about every other medicine used, particularly painkillers such as tramadol or codeine, sleeping tablets, antifungals and herbal products.
- Never stop suddenly after regular use. Ask for a written plan for gradual dose reduction, and seek help if withdrawal symptoms appear during the taper.
- Do not give the tablets to anyone else, even a person with similar symptoms, because the medicine may be dangerous for them.
- Report promptly any worsening of mood, thoughts of self-harm, unusual agitation or breathing difficulty.

## Frequently Asked Questions

### How long can Xanax be taken?

The labelling describes alprazolam for acute treatment, and the dose and duration should be the lowest and shortest that control symptoms. The risk of dependence increases with longer treatment and higher daily doses, so the need for continued use should be reassessed frequently by the prescriber.

### Is Xanax addictive?

Yes, it can be. Even at recommended doses it carries risks of abuse, misuse, addiction and physical dependence, and the risk is higher with higher doses, longer use, a history of substance use, or combined use with alcohol or other drugs.

### Can Xanax be stopped suddenly?

No. Stopping suddenly after regular use can cause withdrawal reactions, including seizures, that can be life-threatening. The dose is usually reduced by no more than 0.5 mg every 3 days, and a slower schedule may be needed.

### Can it be used for sleep problems?

Alprazolam is not approved for insomnia. Its use for sleep problems is discouraged because of tolerance, next-day sedation and dependence, and a doctor can recommend better-suited treatment.

### Is it safe to take Xanax with a painkiller such as tramadol?

This combination is dangerous and can slow breathing to a life-threatening degree. It should be used only under direct medical supervision in the rare cases where there is no alternative.

### Can Xanax be used during pregnancy or breastfeeding?

Use in late pregnancy may cause sedation or withdrawal symptoms in the newborn, and the labelling advises against breastfeeding during treatment. A woman who is pregnant, planning pregnancy or breastfeeding should consult her doctor, who may suggest another approach and a safe way to adjust treatment.
