# Clomid (Clomiphene) — English

Clomid contains clomiphene citrate, a selective estrogen receptor modulator that stimulates ovulation. It is prescribed to women who do not ovulate regularly and wish to become pregnant, most often those with polycystic ovary syndrome. It is taken as a short oral course of tablets once each menstrual cycle under specialist supervision.

## Overview

Clomid is a 50 mg oral tablet of clomiphene citrate, classified in the ATC system under G03GB02 as an ovulation stimulant. It is a non-steroidal compound that was introduced for the treatment of infertility in the 1960s and has long been a first-line oral option for ovulatory dysfunction. The product is intended only for women with demonstrated ovulatory dysfunction who are not pregnant, have normal liver function, and have no ovarian cysts or unexplained abnormal uterine bleeding.

Before treatment, other causes of infertility must be excluded or treated, including problems in the male partner, blocked fallopian tubes, thyroid disease and raised prolactin. The women most likely to respond are those with polycystic ovary syndrome, amenorrhea with galactorrhea, psychogenic amenorrhea, amenorrhea after stopping oral contraceptives, and some cases of secondary amenorrhea of unknown cause. The medicine is not effective in primary pituitary or ovarian failure, where there is no functioning follicle reserve to stimulate. A pelvic examination is needed before the first and each later course, and the plan of treatment should be set out in advance.

In current practice clomiphene remains widely used because it is taken by mouth, is inexpensive to administer and needs less intensive monitoring than injectable gonadotropins. In women with polycystic ovary syndrome, current international guidance often prefers letrozole as the first-line oral agent, so clomiphene is now frequently a second choice in that group. Clomid is a prescription-only medicine and is not a controlled substance in Jordan. It is contraindicated in pregnancy, although human data do not show an increased risk of birth defects when it is taken as indicated, and it was formerly classified in US pregnancy category X.

## Mechanism of Action

Clomiphene citrate is a mixture of two isomers, enclomiphene and zuclomiphene, which bind to estrogen receptors in the hypothalamus, pituitary gland, ovary, endometrium, vagina and cervix. In the hypothalamus it competes with the body's own estradiol and delays the replenishment of intracellular estrogen receptors. The brain therefore perceives a low estrogen level and removes the normal negative feedback on gonadotropin release.

As a result the pituitary increases its release of follicle-stimulating hormone and luteinizing hormone. These gonadotropins drive ovarian steroid production and follicle growth, so circulating estradiol rises, and this leads to a preovulatory surge of luteinizing hormone and rupture of the follicle. After ovulation, progesterone and estradiol rise and fall as in a normal cycle. The antiestrogenic action on the endometrium and cervical mucus can work against conception, which is one reason the number of cycles is limited.

## Pharmacokinetics

- **Absorption:** Clomiphene is readily absorbed after oral administration, and food is not required for absorption.
- **Onset of action:** Gonadotropin release increases within the course of treatment, and ovulation most often occurs 5 to 10 days after a course of tablets is completed.
- **Distribution and metabolism:** The drug is metabolized in the liver and undergoes enterohepatic circulation, which contributes to its prolonged presence in the body.
- **Elimination half-life:** The mean plasma half-life of clomiphene is about 5 days, and the zuclomiphene isomer is cleared much more slowly, so small amounts can persist for several weeks.
- **Excretion:** Elimination is mainly through the feces by way of bile, with a smaller part excreted in the urine.

## Indications

- **Ovulatory dysfunction in women desiring pregnancy:** This is the approved use, for women who do not ovulate or ovulate rarely and have been evaluated so that other barriers to pregnancy are excluded or treated. Polycystic ovary syndrome is the most common underlying cause, and weight management and lifestyle measures are advised alongside treatment.
- **Other causes of anovulation:** It is also used in amenorrhea with galactorrhea, psychogenic amenorrhea, amenorrhea after stopping oral contraceptives, and some cases of secondary amenorrhea of undetermined cause. Properly timed intercourse around the expected ovulation is important for these women.
- **Ovarian stimulation before intrauterine insemination (off-label):** Fertility specialists sometimes use clomiphene to stimulate one or a few follicles in cycles planned for intrauterine insemination. This use is outside the approved indication and is directed by the specialist.
- **Male infertility (off-label):** Clomiphene is used by specialists in selected men with low gonadotropin-driven testosterone or sperm production because it raises the body's own testosterone and sperm-stimulating hormones. This is not an approved indication and requires specialist assessment.

## Dosage and Administration

The doses below are the usual doses in official labelling, and the prescriber sets the actual dose and the timing of each course after clinical assessment. Treatment should be supervised by a physician experienced in gynecologic or endocrine disorders.

- **Adults — first course:** 50 mg (one tablet) once daily for 5 days, starting on or about the 5th day of the cycle, or at any time if there has been no recent uterine bleeding and pregnancy has been excluded.
- **Adults — if ovulation does not occur:** 100 mg (two 50 mg tablets) once daily for 5 days, which may start as early as 30 days after the previous course once pregnancy has been excluded.
- **Adults — maximum dose and duration:** The maximum is 100 mg daily for 5 days per course, and increasing the dose or duration beyond this is not recommended. If ovulation does not occur after three courses, or three ovulatory courses do not produce pregnancy, further treatment is not recommended, and long-term cyclic therapy should not exceed about six cycles in total.
- **Women who ovulate on 50 mg:** There is no advantage in increasing the dose in later cycles once ovulation has occurred at 50 mg, and a low dose or short course is preferred in women who may be unusually sensitive to gonadotropins, such as those with polycystic ovary syndrome.
- **Children and adolescents:** The medicine is not indicated in children, and there is no pediatric dose.

**How to take it:**
- Take the tablet by mouth once daily at about the same time each day for all five days of the course, with water, with or without food.
- Starting on or about day 5 of the cycle is the labelled schedule, and some specialists begin slightly earlier, so follow the exact days given by the treating doctor.
- Intercourse should be timed around the expected ovulation, which is usually 5 to 10 days after the last tablet, and ovulation can be confirmed by urine luteinizing hormone tests, a basal body temperature chart or ultrasound follicle monitoring.
- Pregnancy, ovarian enlargement and ovarian cysts should be excluded before each new course.

**If you miss a dose:** If a tablet is missed, take it as soon as it is remembered on the same day, and do not double the next dose without advice. If more than one day has been missed, contact the treating doctor to decide how to complete the course.

**Overdose:** Overdose may cause nausea, vomiting, flushing, blurred vision, spots or flashes, and ovarian enlargement with pelvic or abdominal pain, and no specific antidote is known. It is a medical emergency, and the person should be taken for urgent care where supportive treatment and gastrointestinal decontamination can be given.

## Pregnancy, Breastfeeding and Special Groups

- **Pregnancy:** Clomiphene is contraindicated in pregnancy because it offers no benefit, and pregnancy must be excluded before every course. Available human data do not suggest a higher rate of congenital anomalies than in the general population when it is used as indicated, but animal studies showed increased embryo-fetal loss and malformations, so a woman who conceives while taking it should be told of the potential risk and stop the drug.
- **Breastfeeding:** It is not known whether clomiphene passes into human milk, so caution is advised. In some women it may reduce milk production, and it has no role in a woman who is breastfeeding.
- **Children:** The medicine is not indicated in children, and its safety and effectiveness have not been established in this group.
- **Older adults:** The product is intended for women of reproductive age, and the question of dose adjustment in older adults does not arise. Women after menopause should not use it.
- **Kidney impairment:** No specific dose adjustment is established in the labelling, as clomiphene is eliminated mainly through the bile and feces. Women with significant kidney disease should be assessed by their doctor before treatment.
- **Liver impairment:** Clomiphene is contraindicated in women with liver disease or a history of liver dysfunction, because the drug is metabolized in the liver and its clearance may be reduced. Normal liver function should be confirmed before treatment.

## Contraindications

- Clomiphene must not be used in women with a known hypersensitivity or allergy to clomiphene citrate or to any ingredient of the tablet.
- It must not be used during pregnancy, and pregnancy must be excluded before each course.
- It must not be used in women with liver disease or a history of liver dysfunction.
- It must not be used in women with abnormal uterine bleeding of undetermined origin until the cause has been investigated and neoplastic lesions excluded.
- It must not be used in women with ovarian cysts or ovarian enlargement, except enlargement due to polycystic ovary syndrome.
- It must not be used in women with uncontrolled thyroid or adrenal dysfunction or an organic intracranial lesion such as a pituitary tumor, and it is not appropriate in primary ovarian or pituitary failure.

## Warnings and Precautions

- **Visual symptoms:** Blurred vision, spots, flashes and other visual disturbances can occur and become more frequent with higher total dose or longer treatment. They are usually reversible, but prolonged and possibly irreversible cases have been reported, so any visual symptom requires stopping the medicine and a prompt full eye examination. They may make driving or operating machinery more hazardous, particularly in variable lighting.
- **Ovarian hyperstimulation syndrome:** Ovarian hyperstimulation syndrome may progress within 24 hours to several days into a serious disorder, and it can occur with clomiphene alone or with gonadotropins. Severe abdominal pain, rapid abdominal swelling or weight gain, vomiting, breathlessness or reduced urine output require urgent medical review, and women with polycystic ovary syndrome should receive the lowest effective dose.
- **Ovarian enlargement and cysts:** Ovarian enlargement and cyst formation can occur, and further courses should not be started until these have resolved. A pelvic examination is required before every course.
- **Multiple pregnancy:** Treatment increases the chance of a multiple pregnancy, most often twins, which carries higher risks for the mother and babies. The patient should be informed of this before treatment starts.
- **Limit on the number of cycles:** Long-term cyclic therapy is not recommended beyond about six cycles in total. Epidemiologic data suggest that prolonged use may increase the risk of borderline or invasive ovarian tumors, and the antiestrogenic effect on the endometrium may also reduce the chance of conception.
- **Underlying conditions that must be treated:** Thyroid disease, raised prolactin and other causes of anovulation, as well as male factor and tubal problems, should be addressed before and during treatment. Failure to ovulate after three courses means the patient should be reevaluated rather than treated further.
- **Lack of menstruation after ovulation:** If menses does not occur after an ovulatory response, the patient should be reevaluated, and pregnancy and ovarian enlargement should be excluded before any further treatment.

## Side Effects

**Common side effects:**
- Ovarian enlargement is the most frequently recorded effect and is usually mild, resolving after the course is stopped.
- Hot flushes occur in about 1 in 10 women and are temporary, with no treatment usually needed.
- Abdominal or pelvic discomfort and bloating are common, and mild symptoms can be managed with rest and simple measures while they are monitored.
- Nausea and vomiting occur in a small proportion of women and may be reduced by taking the tablet with food.
- Breast tenderness or discomfort occurs and usually settles after the course.
- Headache is reported and can generally be treated with a simple analgesic after discussion with a doctor.
- Intermenstrual spotting or heavier menstrual bleeding can occur, and persistent abnormal bleeding should be reported.
- Mood changes, nervousness, insomnia, dizziness, fatigue, and hair thinning have been reported in fewer than 1 in 100 women.

**Side effects that need urgent medical advice:**
- New blurred vision, spots, flashes or other visual disturbance requires stopping the medicine and urgent eye assessment, since some cases persist after stopping.
- Severe abdominal pain with rapid swelling, weight gain, vomiting, breathlessness or reduced urination suggests ovarian hyperstimulation syndrome and needs emergency care.
- Sudden severe pelvic pain may indicate an ovarian cyst that has twisted or ruptured, and it should be assessed urgently.
- Allergic reactions such as rash, swelling of the face or lips and difficulty breathing require immediate medical attention, and the medicine must not be taken again.
- Severe depression or marked changes in mood should be reported to the doctor without delay.
- Prolonged use has been associated with a possible increase in the risk of ovarian tumors, which is a reason to avoid more than about six cycles.

## Drug Interactions

- **Gonadotropins and other ovulation-stimulating drugs:** Combining clomiphene with injectable gonadotropins or other stimulants increases the risk of ovarian hyperstimulation syndrome and multiple pregnancy. Such combinations should be used only in specialist-directed protocols with ultrasound monitoring.
- **Tamoxifen, raloxifene and other estrogen receptor modulators:** These drugs act on the same receptors and may oppose or duplicate the effects of clomiphene, so they should not be taken together without specialist advice.
- **Strong CYP2D6 inhibitors such as fluoxetine and paroxetine:** Clomiphene is converted to active metabolites by liver enzymes, so strong inhibitors of CYP2D6 could theoretically reduce its effect. The significance is not established, and the doctor should be told about all other medicines.
- **Estrogen-containing and other hormonal contraceptives:** Hormonal contraceptives suppress ovulation and defeat the purpose of treatment, so they must be stopped before a course is started.
- **Estrogen or progestin used to test the cycle:** A progestin may be given to induce bleeding before a course and is planned by the specialist. Other hormonal therapy during a course should be avoided unless prescribed.
- **Documented interactions:** The official labelling states that drug interactions with clomiphene have not been documented, so the routine tests used in treatment, such as early-cycle hormone levels, ultrasound follicle monitoring and sometimes a mid-luteal progesterone level, are the main companions of therapy rather than other drugs.

## Food, Alcohol and Daily Life

- No food or drink restrictions are established for clomiphene, but a healthy diet and regular physical activity improve ovulation and pregnancy chances in women with polycystic ovary syndrome, especially when weight is reduced in overweight women.
- Avoid driving or operating machinery if any visual disturbance occurs, especially in bright or changing light, and stop the medicine and seek an eye examination.
- Avoid smoking, which reduces fertility, and limit alcohol while trying to conceive.

## Storage

- Store the tablets at 20 to 25 °C, with brief excursions between 15 and 30 °C permitted, in the original container away from moisture and light.
- Keep the medicine out of the sight and reach of children.

## Advice for Patients

- Take the tablets on exactly the days prescribed, and do not increase the dose or add extra cycles on your own.
- Track ovulation with urine luteinizing hormone strips, a temperature chart or ultrasound, and plan intercourse every one to two days around the expected ovulation rather than relying on tablet dates alone.
- Do a pregnancy test and arrange the planned examination before beginning each new course.
- Report any visual change, severe abdominal pain, rapid abdominal swelling or breathlessness immediately and stop the tablets.
- Be prepared for hot flushes, bloating and mood changes, which are usually mild and short-lived.
- Discuss with the specialist the other options, such as letrozole, injectable gonadotropins or assisted reproduction, if pregnancy has not occurred after the recommended number of cycles.

## Frequently Asked Questions

### When does ovulation happen after taking Clomid?

Ovulation usually occurs 5 to 10 days after the last tablet of the course. Most women who are going to ovulate do so after the first course, so the doctor decides on the dose increase only when ovulation has not been confirmed.

### Why is treatment limited to about six cycles?

Labelling advises against long-term cyclic therapy beyond about six cycles in total, because prolonged use may raise the risk of ovarian tumors, and the antiestrogenic effect on the endometrium may reduce the chance of pregnancy. If pregnancy has not occurred, other treatments should be considered.

### Does Clomid cause twins?

The chance of a multiple pregnancy is higher than without treatment, and twins are the most common. Triplets and higher are rare, and ultrasound monitoring helps the doctor to identify a larger number of mature follicles.

### Can I take Clomid if I do not have periods?

Yes, in some women the course may be started at any time if there has been no recent uterine bleeding, but only after pregnancy has been excluded. Alternatively the doctor may first induce a withdrawal bleed with a progestin and then start on or about day 5.

### Is letrozole better than Clomid?

For women with polycystic ovary syndrome, current international guidance often prefers letrozole as the first-line oral option, as it is associated with higher live-birth rates in many studies. Clomiphene remains useful, and the choice depends on the diagnosis, local availability and the specialist's judgement.

### What should I do if I take Clomid and find I am pregnant?

Stop the tablets and inform your doctor at once. Available human data do not suggest an increased rate of congenital anomalies above the background risk when it is used as indicated, but you should receive advice and early pregnancy care.
