Designated smoking areas or ventilation systems do not provide complete protection from secondhand smoke. There is no safe level of exposure to secondhand tobacco smoke, and even brief exposure can cause immediate harm. The only way to fully protect non-smokers is the complete elimination of indoor smoking.

## Ongoing Risks of Secondhand Smoke

Commercial tobacco smoke contains thousands of chemicals, hundreds of which are toxic and about 70 of which can cause cancer. These harmful chemicals are present in the smoke that smokers inhale as well as in secondhand smoke [\[1\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S1>) [\[2\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S2>). Even low levels of secondhand smoke can be harmful and increase the risk of coronary heart disease, stroke, and lung cancer in non-smoking adults. Infants and young children are especially affected by health problems resulting from secondhand smoke because their bodies are still developing [\[1\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S1>).

## Why Are Ventilation and Designated Areas Insufficient?

Opening windows, using fans, ventilation systems, designating specific rooms for smoking at home, or setting specific smoking times does not eliminate exposure to secondhand smoke [\[2\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S2>). Tobacco smoke can travel through hallways, ventilation ducts, and heating and cooling systems, even in multi-unit buildings such as apartments [\[1\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S1>). This means that non-smokers, including children, may be exposed to secondhand smoke even if no one smokes directly in their residential unit [\[1\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S1>).

## Effective Protection Against Secondhand Smoke

The only way to ensure full protection for non-smokers from secondhand smoke is to implement comprehensive laws and policies that ban smoking in all indoor public spaces and workplaces, including restaurants and bars [\[1\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S1>). These laws ensure that everyone breathes smoke-free air [\[1\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S1>). On a personal level, the most effective steps include:

- Prohibiting smoking inside the home entirely [\[2\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S2>).

- Not allowing anyone to smoke inside the car, even with windows open [\[2\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S2>).

- Ensuring that places where children are cared for are completely tobacco-free [\[2\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S2>).

- Teaching children to avoid secondhand smoke [\[2\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S2>).

## E-cigarettes and Indirect Exposure

E-cigarettes produce indirect aerosol (not secondhand smoke). This aerosol also contains harmful and potentially harmful substances, such as nicotine, heavy metals, volatile organic compounds, and carcinogens [\[2\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S2>). Therefore, including e-cigarettes in smoking ban policies can protect people who do not use them from being exposed to their emissions [\[1\]](<https://clinicsjo.com/en/faq/can-designated-smoking-areas-or-ventilation-systems-protect-from-secondhand-smoke#source-S1>).
