An insurer’s name on a profile does not establish that every policy tier or service is covered. Confirm your policy category, provider network, planned service, copayment and any prior-authorization requirement with both the clinic and insurer.

Bring your insurance card and any required referral or authorization. If the network has changed or the insurer is not listed, call the clinic rather than assuming insurance is rejected or every service is automatically covered.
