Sleeve gastrectomy removes a large part of the stomach, leaving a smaller, tube-shaped stomach without rerouting the intestines. Gastric bypass creates a small stomach pouch connected to the intestine so that food bypasses part of the stomach and small intestine.

Both procedures affect food intake and appetite regulation; bypass also changes absorption. Vitamin deficiencies can occur after either operation, so follow-up and supplements are needed according to the clinical plan. Sleeve surgery may worsen reflux in some patients.
