Functional ovarian cysts are fluid-filled sacs that form on or within an ovary as a result of the natural ovulation process, and they are among the most common conditions in women of reproductive age. In most cases, these cysts do not cause any noticeable symptoms and are often discovered incidentally during a routine pelvic examination [\[1\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S1>). However, when cysts begin to cause symptoms, they are usually due to the size or location of the cyst.

## Common symptoms of functional ovarian cysts

The symptoms a woman may experience with a functional cyst vary, and they are usually mild to moderate and disappear on their own over time. The most notable common signs include:

- **Pressure sensation:** A woman may feel persistent pressure or heaviness in the pelvic area or lower abdomen.

- **Bloating and abdominal swelling:** Noting an increase in abdominal circumference or a feeling of fullness, which is a symptom associated with the size of the cyst.

- **Lower abdominal pain:** Pain is often concentrated on one side of the abdomen, specifically the side containing the ovary with the cyst [\[1\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S1>).

It is essential to understand the difference between normal and concerning conditions to know when it is standard and when it warrants worry, which we explain in our detailed article on [Functional Ovarian Cysts: When are they normal and when should you worry? (Arabic)](<https://clinicsjo.com/ar/articles/functional-ovarian-cysts-normal-vs-concerning>).

## Signs requiring immediate medical intervention

Although functional cysts are often benign and harmless, there are specific situations that require urgent medical consultation. If a cyst ruptures or causes internal bleeding, it is considered an emergency condition that requires immediate intervention [\[1\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S1>). Any sharp, sudden abdominal pain should not be ignored, as it can indicate complications requiring direct medical evaluation to determine the appropriate course of action.

## Diagnosis, monitoring, and prevention

When an ovarian cyst is discovered, doctors often recommend waiting for a period to monitor the condition and see if the cyst will resolve spontaneously. In some cases, birth control pills may be prescribed as a treatment method to help prevent the formation of new cysts in the future [\[1\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S1>). If the pain persists, the cyst is large, or if the woman is postmenopausal, the doctor may recommend surgical intervention. It is worth noting that the risk of these cysts becoming malignant (cancerous) is very rare, but this risk increases with age [\[1\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S1>).

## Ovarian cysts and Polycystic Ovary Syndrome (PCOS)

A clear distinction must be made between individual functional ovarian cysts and Polycystic Ovary Syndrome (PCOS). While functional cysts are natural sacs related to the ovulation cycle that resolve on their own, PCOS is a hormonal disorder that may cause irregular menstrual cycles, elevated levels of male hormones (which can be measured via tests such as the DHEA-sulfate test), and the presence of numerous small cysts in the ovaries [\[1\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S1>) [\[2\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S2>). Therefore, if symptoms are accompanied by other physical changes such as increased hair growth or skin problems, one should visit a doctor to perform the necessary hormonal tests for an accurate diagnosis and to exclude adrenal gland disorders or other hormonal causes [\[2\]](<https://clinicsjo.com/en/faq/symptoms-functional-ovarian-cysts#source-S2>).
