A sore throat is a common health issue, often caused by a viral infection, though in some cases, it may be bacterial. Understanding the difference is important to determine the appropriate treatment and avoid complications. While most viral sore throat cases resolve on their own, bacterial sore throat often requires antibiotic treatment.

## Viral Sore Throat

Viral sore throat is the most common type, caused by viruses such as those responsible for the common cold or flu. Symptoms are usually accompanied by other upper respiratory tract symptoms [\[3\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S3>).

- **Common symptoms:** In addition to throat pain, symptoms may include runny nose, cough, sneezing, hoarseness, and body aches. It may also be accompanied by a mild fever.

- **Treatment:** No antibiotics are effective against viruses. Treatment focuses on relieving symptoms, such as rest, drinking warm fluids, using over-the-counter pain relievers, and salt water gargles. Symptoms usually disappear within a week.

## Bacterial Sore Throat (Strep Throat)

Bacterial sore throat, most notably strep throat (Streptococcal pharyngitis), is caused by Streptococcus bacteria. This type is less common but can lead to serious complications if left untreated [\[3\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S3>).

- **Common symptoms:** Symptoms often start suddenly and include severe throat pain, difficulty swallowing, fever (which may be high), small red spots on the roof of the mouth (soft palate), and swollen tonsils with white patches or streaks of pus. You may experience nausea or vomiting, especially in children. Coughing or a runny nose is usually absent.

- **Diagnosis:** To confirm strep throat, the doctor performs a rapid throat swab test to detect the bacteria [\[3\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S3>). In some cases, the swab may be sent for a bacterial culture to confirm the diagnosis.

- **Treatment:** Strep throat requires antibiotic treatment prescribed by a doctor. It is essential to complete the full course of antibiotics, even if symptoms improve, to prevent complications such as rheumatic fever or kidney inflammation.

## When should you see a doctor?

You should consult a doctor if you experience severe or concerning symptoms, such as:

- Severe throat pain lasting for more than two days.

- Difficulty swallowing or breathing.

- High fever.

- Swollen lymph nodes in the neck.

- Appearance of white patches or pus on the tonsils.

- Skin rash.

In addition to viral and bacterial sore throat, other disorders can affect the throat and cause similar symptoms, such as Laryngopharyngeal Reflux (LPR), which you can learn more about, along with other common throat disorders, by visiting [Common Throat Disorders: From Tonsillitis to Laryngopharyngeal Reflux (Arabic)](<https://clinicsjo.com/ar/articles/common-throat-disorders-tonsillitis-lpr>).

## Barium Swallow Test

In some cases where the patient suffers from difficulty swallowing or suspected structural problems in the upper digestive tract, the doctor may recommend a [\[1\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S1>) Barium Swallow test. This imaging test uses barium to coat the internal organs, allowing them to be seen more clearly on X-rays, and reveals problems such as ulcers, gastroesophageal reflux disease (GERD), hiatal hernia, or tumors [\[1\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S1>). The test involves drinking a liquid containing barium while live X-rays (fluoroscopy) are taken to monitor the swallowing process and the movement of the barium through the throat, esophagus, and stomach [\[1\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S1>). Preparation for this test requires fasting for several hours before the procedure, and you may be asked to avoid smoking or chewing gum [\[1\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S1>).

Barium swallow results can reveal abnormalities in size, shape, or movement in the upper digestive tract, which helps in diagnosing conditions such as esophageal strictures or tumors [\[1\]](<https://clinicsjo.com/en/faq/viral-vs-bacterial-sore-throat-differences-symptoms-diagnosis#source-S1>). It is important to discuss any questions about the results with your doctor.
