### Look at the skin and household pattern

Describe location, rash, new soaps or medicines and whether others at home are affected. Scabies is not evidence of poor hygiene; confirmed cases may require contacts to receive treatment together under clinical advice.

### Reduce irritation safely

Use an unperfumed moisturiser and avoid hot water and irritating clothes. Do not cover the body with strong steroids or scabies treatments without a diagnosis. Sedating antihistamines may be unsuitable with some conditions or medicines.

### Pregnancy and prompt assessment

Severe itching during pregnancy, especially on palms, soles or at night, warrants prompt contact with the maternity team for possible cholestasis. Jaundice or weight loss also needs assessment. Tongue swelling or breathing difficulty is an emergency.
