أكثر ما يُشاهد عند الأطفال خرز وقطع إسفنج وبذور وأجزاء أعواد قطن وقطع أقلام، وعند الكبار قطن عالق أو حشرة دخلت أثناء النوم أو جزء من سمّاعة طبية. وبعض الأجسام تُعدّ حالة عاجلة لا تنتظر موعداً: البطارية الزرّية لأنها تُحدث تأكّلاً كيميائياً سريعاً في الجلد، والأجسام الحادّة، والحشرة الحيّة، وأي جسم يرافقه نزف أو ألم شديد أو نقص سمع مفاجئ.

تحذير مهم: محاولة الإخراج في المنزل بعود قطن أو ملقط أو دبوس شعر تدفع الجسم أعمق نحو الطبلة وتجرح جلد القناة وتُصعّب الإجراء لاحقاً، وهي من أشيع أسباب تحويل حالة بسيطة إلى حالة تحتاج غرفة عمليات. ولا يُغسل الأذن بالماء إذا كان الجسم عضوياً كبذرة أو حبّة عدس لأنه ينتفخ ويزيد الانحصار، ولا إذا كان هناك شكّ في ثقب طبلة أو وجود أنبوب تهوية.

حدود الإجراء: الاستخراج يزيل السبب فحسب، ولا يعالج نقص سمع كان موجوداً قبل الحادثة، ولا يشفي التهاباً مزمناً في الأذن، ولا يُصلح أذى وقع على الطبلة أثناء محاولات الإخراج المنزلية. وقد يبقى احتقان أو ألم أو نقص سمع خفيف لأيام حتى يتعافى جلد القناة. كذلك لا يُنجَز كل جسم في العيادة؛ فالجسم العميق أو الملتصق أو الطفل الصغير غير المتعاون قد يستلزم إخراجاً تحت تخدير عام حفاظاً على سلامة الأذن.

طريقة الإخراج ووقته يُحدَّدان بالفحص السريري وبالتنظير المباشر للأذن، لا بوصف هاتفي ولا بصورة، لأن نوع الجسم وشكله وموقعه بالنسبة للطبلة هو ما يحكم الخطة كلها.
Removal of a foreign body from the ear is a procedure to extract an object lodged in the external ear canal. It is performed in an ENT clinic under direct light and magnification using purpose-made instruments: fine forceps, an angled hook, gentle suction, or irrigation in carefully selected cases. The aim is to retrieve the object with the least possible trauma to the canal skin and the eardrum.

In children the objects most often seen are beads, pieces of foam, seeds, broken cotton-bud tips and fragments of pencils or toys. In adults it is more often retained cotton, an insect that entered during sleep, or part of a hearing aid. Some situations are urgent and should not wait for a routine appointment: a button battery, because it causes rapid chemical injury to the skin; sharp objects; a live insect; and any object accompanied by bleeding, severe pain or sudden hearing loss.

An important warning: trying to remove the object at home with a cotton bud, tweezers or a hairpin pushes it deeper towards the eardrum, grazes the canal skin and makes later removal harder. This is one of the commonest reasons a simple case ends up needing a general anaesthetic. The ear should also not be irrigated when the object is organic, such as a seed or a lentil, because it swells and becomes more tightly wedged, nor when a perforated eardrum or a ventilation tube is suspected.

What the procedure does not do: extraction removes the cause only. It does not treat hearing loss that existed before the incident, it does not cure chronic ear disease, and it does not repair damage inflicted on the eardrum during home attempts. Mild blockage, discomfort or slightly reduced hearing may persist for a few days while the canal skin heals. Not every object can be dealt with in the clinic either; a deeply wedged or adherent object, or a small child who cannot cooperate, may need removal under general anaesthesia.

The method and the timing are decided by clinical examination and direct otoscopy, not by a phone description or a photograph, because the nature, shape and position of the object relative to the eardrum govern the whole plan.

خرافتان شائعتان يجب تصحيحهما: أعواد القطن لا تُنظّف الأذن بل تدفع الشمع إلى العمق وتخدش الجلد وقد تثقب الطبلة، وهي من أشيع أسباب الانسداد الذي يُراجَع من أجله. وشموع الأذن المشتعلة لا أساس لفاعليتها، وتسبّب حروقاً في القناة والوجه وترك شمع مذاب داخل الأذن.

حدود الإجراء: إزالة الشمع تُصلح ما سبّبه الانسداد فقط. فإن استمرّ نقص السمع أو الطنين أو الدوار بعد تفريغ القناة، فالسبب في الأذن الوسطى أو في العصب السمعي ويحتاج تقييماً وقياس سمع لا تنظيفاً متكرّراً. كما أن الإزالة لا تمنع عودة التراكم لمن لديه ميل إليه، ولا تُعالج التهاب الأذن الخارجية إن كان موجوداً.

بعض الحالات لا يناسبها الغسل المائي إطلاقاً: ثقب الطبلة أو أنبوب تهوية أو جراحة أذن سابقة أو التهاب خارجي نشط أو أذن واحدة سامعة، وكذلك مريض السكري أو نقص المناعة. لهؤلاء يكون الشفط تحت الرؤية أأمن. الترشيح للطريقة المناسبة يُحدَّد بالفحص السريري لا بالوصف.
Ear wax removal is a procedure to clear impacted cerumen from the external ear canal when it causes symptoms or prevents examination. Three main methods are used: microsuction under an otoscope or microscope, manual removal with small instruments under direct vision, or irrigation with body-temperature water after the wax has been softened with drops for several days.

Ear wax is not dirt that needs constant cleaning. It is a normal secretion that protects and moisturises the canal skin and traps dust, and the ear expels it on its own through skin migration and jaw movement. Most people therefore need no routine cleaning at all. Removal is indicated when the wax is impacted and causes reduced hearing, tinnitus, a blocked or full sensation, pain, dizziness or a reflex cough, or when the eardrum needs to be seen, a hearing test performed, or an impression taken for a hearing aid.

Two widespread myths need correcting. Cotton buds do not clean the ear; they push wax deeper, abrade the skin and can perforate the eardrum, and they are among the commonest causes of the very blockage people attend for. Ear candling has no established benefit and can cause burns to the canal and face as well as leaving melted wax inside the ear.

What the procedure does not do: clearing wax corrects only what the blockage itself caused. If reduced hearing, tinnitus or dizziness persists once the canal is clear, the cause lies in the middle ear or the hearing nerve and needs assessment with an audiogram rather than repeated cleaning. Removal also does not stop wax from re-accumulating in people prone to it, and it does not treat coexisting infection of the outer ear.

Some ears should never be irrigated: a perforated eardrum, a ventilation tube, previous ear surgery, active otitis externa, or an only-hearing ear, and caution applies in diabetes or reduced immunity. For these, microsuction under vision is safer. The suitable method is decided by clinical examination, not by a description of the symptoms.

علاج التهاب الأذن يشمل حالتين مختلفتين يُخلط بينهما كثيراً: التهاب الأذن الخارجية الذي يصيب جلد القناة ويكون الألم فيه واضحاً عند لمس الأذن أو شدّها، والتهاب الأذن الوسطى الحادّ الذي يصيب الحيّز خلف الطبلة ويأتي غالباً بعد نزلة تنفّسية بألم عميق وحرارة ونقص سمع. التفريق بينهما يُحدَّد بتنظير الأذن، وعليه تُبنى الخطة كلها.

في التهاب الأذن الخارجية يكون الأساس تنظيف القناة تحت الرؤية وقطرات موضعية مناسبة، وقد يُوضَع فتيل ماصّ إن كانت القناة متورّمة لتصل القطرة إلى العمق، مع إبقاء الأذن جافّة تماماً. أمّا في التهاب الأذن الوسطى الحادّ فأولوية العلاج هي ضبط الألم والحرارة، ثم قرار المضاد الحيوي الذي يتّخذه الطبيب بحسب العمر وشدّة الحالة ومدّتها وإصابة أذن واحدة أو الأذنين، إذ إن جزءاً من الحالات فيروسي ويتحسّن دون مضاد حيوي تحت مراقبة قريبة.

أخطاء شائعة يجب تجنّبها: تقطير زيت أو ثوم أو أعشاب في الأذن، فهذا غير مثبت وخطير إن كانت الطبلة مثقوبة؛ واستخدام بقايا مضاد حيوي من مرّة سابقة أو من فرد آخر؛ ومحاولة تنظيف القناة المتورّمة بعود قطن. أمّا خروج إفراز من الأذن أثناء الالتهاب فقد يعني انفتاح ثقب في الطبلة، وهو يستوجب فحصاً لا تجاهلاً، ويمنع أي غسل مائي.

حدود العلاج: المضاد الحيوي لا يُفيد الالتهاب الفيروسي، ولا يُذيب السائل الذي يبقى خلف الطبلة بعد انطفاء الالتهاب، وهذا السائل قد يستمرّ أسابيع مع نقص سمع خفيف ويُتابَع بقياس سمع لا بكورسات متكرّرة. كذلك لا يمنع أي علاج تكرار الالتهاب عند من لديه استعداد، ولا يعني اختفاء الألم بعد يومين أن الشفاء تمّ؛ فالمتابعة هي ما يؤكّد عودة الطبلة إلى طبيعتها.

الوقاية عملية أكثر من الدواء: علاج انسداد الأنف والحساسية، الابتعاد عن دخان التبغ، عدم إرضاع الطفل مستلقياً على ظهره، وتجفيف الأذن جيداً بعد السباحة. والترشيح لأي خطوة، بما فيها أنبوب التهوية عند تكرار الالتهاب، يُحدَّد بالفحص السريري.
Ear infection treatment covers two conditions that are frequently confused. Otitis externa affects the skin of the ear canal and typically hurts when the ear is touched or pulled. Acute otitis media affects the space behind the eardrum, usually follows a respiratory infection, and presents with deep pain, fever and reduced hearing. Distinguishing the two requires otoscopy, and the entire plan follows from that distinction.

In otitis externa the mainstays are clearing the canal under vision and using appropriate topical drops, with an absorbent wick placed if the canal is too swollen for drops to reach the depth, while keeping the ear completely dry. In acute otitis media the first priority is controlling pain and fever; the decision about antibiotics is made by the clinician according to age, severity, duration and whether one or both ears are involved, since a proportion of cases are viral and settle without antibiotics under close review.

Common mistakes to avoid: instilling oil, garlic or herbal preparations into the ear, which is unproven and hazardous if the eardrum is perforated; using leftover antibiotics from a previous episode or from someone else; and trying to clean a swollen canal with a cotton bud. Discharge appearing during an infection may mean the eardrum has perforated; this needs examination rather than being ignored, and it rules out any irrigation.

What treatment does not do: antibiotics do not help viral infection, and they do not clear the fluid that often remains behind the eardrum after the acute episode settles. That fluid can persist for weeks with mild hearing loss and is followed with hearing testing rather than repeated antibiotic courses. No treatment prevents recurrence in a predisposed child, and pain settling after two days does not confirm resolution; follow-up examination is what confirms the eardrum has returned to normal.

Prevention is more practical than medication: treating nasal obstruction and allergy, avoiding tobacco smoke, not feeding an infant lying flat on the back, and drying the ears thoroughly after swimming. Suitability for any further step, including a ventilation tube for recurrent infection, is determined by clinical examination.

التهاب الجيوب الأنفية المزمن حالة التهاب مستمرّ في مخاطية الأنف والجيوب لمدّة اثني عشر أسبوعاً أو أكثر، ويُشخَّص بوجود أعراض مستمرّة مع علامة موضوعية تُرى بتنظير الأنف أو بالتصوير الطبقي المحوري. الأعراض الأساسية أربعة: انسداد الأنف، إفرازات أمامية أو خلفية، ثقل أو ألم في الوجه، ونقص أو فقد حاسة الشمّ. وجود الأعراض وحدها بلا علامة موضوعية لا يكفي للتشخيص.

العلاج يبدأ دائماً بالعلاج الطبي المنظّم، لا بالجراحة: غسل الأنف بمحلول ملحي بانتظام، وبخّاخ ستيروئيدي أنفي موضعي لمدّة كافية يحدّدها الطبيب، ومعالجة العوامل المساهمة وهي كثيرة ومهمّة: التهاب الأنف التحسّسي، والربو، والتدخين والشيشة، والارتجاع المعدي المريئي، والتهاب جذور الأضراس العلوية، وانحراف الحاجز الشديد، وأمراض المناعة. تُوصف المضادات الحيوية فقط عند دلائل جرثومية أو تفاقم حادّ وبقرار الطبيب، لا كعلاج دائم.

الجراحة الوظيفية بالمنظار خيار لمن لم يتحسّن بعد علاج طبي كافٍ ومُلتزَم به. ومفهومها الصحيح أنها توسّع فتحات التصريف الطبيعية وتُزيل النسيج المريض لتفتح مساراً للتهوية وللدواء الموضعي، وليست استئصالاً للجيب ولا تنظيفاً نهائياً له. ولذلك يستمرّ الغسل الملحي والبخّاخ بعد الجراحة، فهي تجعل العلاج الموضعي أكثر فاعلية لا تُلغيه.

حدود الإجراء بوضوح: جراحة الجيوب لا تشفي الحساسية ولا الربو، ولا تمنع نزلات البرد، ولا تضمن عدم رجوع اللحميات الأنفية عند من لديه التهاب شديد، وقد لا تعود حاسة الشمّ كاملة إن كان فقدها طويلاً. والصداع ليس دائماً من الجيوب؛ كثير من حالات الثقل الوجهي سببها صداع نصفي أو صداع توتّري ولا تتحسّن بجراحة. كذلك لا يُطلب التصوير الطبقي المحوري لكل زكام أو صداع، بل بضوابط سريرية.

خرافة شائعة: لا يوجد شيء اسمه شفط الجيوب أو تنظيفها نهائياً بجلسة واحدة. والغسل الملحي مفيد وآمن لكنه ليس بديلاً عن التقييم. أمّا بخّاخات مضادات الاحتقان التي تفتح الأنف فوراً فاستخدامها أكثر من أيام قليلة يسبّب احتقاناً ارتدادياً يزيد الانسداد. والترشيح لأي علاج أو جراحة يُحدَّد بالفحص السريري وتنظير الأنف لا بالقراءة.
Chronic sinusitis, more accurately chronic rhinosinusitis, is persistent inflammation of the lining of the nose and sinuses lasting twelve weeks or more. Diagnosis requires ongoing symptoms together with an objective finding seen on nasal endoscopy or on a CT scan. There are four cardinal symptoms: nasal obstruction, anterior or posterior discharge, facial pressure or pain, and reduced or absent sense of smell. Symptoms alone, without an objective finding, are not sufficient to make the diagnosis.

Treatment always begins with structured medical therapy rather than surgery: regular saline nasal irrigation, a topical intranasal steroid spray used for an adequate period determined by the clinician, and management of contributing factors, which matter a great deal. These include allergic rhinitis, asthma, cigarette and waterpipe smoking, gastro-oesophageal reflux, infected roots of the upper molars, a severely deviated septum and immune disorders. Antibiotics are reserved for evidence of bacterial infection or an acute exacerbation, at the clinician's decision, and are not a long-term treatment.

Functional endoscopic sinus surgery is an option for patients who have not improved after adequate, properly adhered-to medical treatment. Correctly understood, it widens the natural drainage pathways and removes diseased tissue to open a route for ventilation and for topical medication; it is not removal of the sinus, nor a definitive one-off cleaning. This is why saline irrigation and sprays continue afterwards: surgery makes topical treatment more effective rather than replacing it.

The limits should be stated plainly: sinus surgery does not cure allergy or asthma, it does not prevent colds, it does not guarantee that nasal polyps will not return in patients with severe inflammation, and the sense of smell may not fully recover if it has been lost for a long time. Headache is also not always sinus-related; much facial heaviness is due to migraine or tension-type headache and will not improve with surgery. A CT scan is likewise not indicated for every cold or headache, but according to clinical criteria.

A widespread myth needs correcting: there is no procedure that permanently cleans or suctions out the sinuses in a single session. Saline irrigation is useful and safe but is not a substitute for assessment. Decongestant sprays that open the nose immediately cause rebound congestion and worse obstruction if used for more than a few days. العلاج الأساسي في الحالات الفيروسية دعمي: راحة وسوائل كافية ومسكّنات يحدّدها الطبيب بحسب حالتك الصحية، وغرغرة بماء مالح فاتر، وتجنّب المخرّشات كالتدخين والشيشة. أمّا عند الشكّ بالعقديّة فيُوصف مضاد حيوي مناسب ويُكمَل كامل المدّة حتى مع تحسّن الأعراض سريعاً، لأن الهدف ليس تسكين الألم فقط بل تقليل خطر مضاعفات مثل الحمّى الرثوية والخرّاج حول اللوزة.

استئصال اللوزتين ليس علاجاً لأي التهاب، بل يُدرَس في حالات محدّدة: تكرّر التهابات موثّقة بعددها وتواريخها على مدى سنة أو أكثر، أو خرّاج حول اللوزة متكرّر، أو انسداد تنفّس نومي مرتبط بتضخّم اللوزتين، أو شكّ سريري بورم. وعند البالغ تكون فترة النقاهة أطول وأكثر إيلاماً من الطفل، عادة نحو عشرة إلى أربعة عشر يوماً، مع خطر نزف ثانوي يستوجب مراجعة فورية.

حدود العلاج بصراحة: استئصال اللوزتين لا يمنع نزلات البرد ولا التهاب البلعوم ولا التهابات الجيوب، ولا يُضعف المناعة كما يُشاع، ولا يعالج رائحة الفم إن كان سببها الأسنان أو الجيوب أو المعدة. والمضاد الحيوي لا يفيد الالتهاب الفيروسي ولا يقصّر مدّته، بل يعرّضك لأعراض جانبية ومقاومة جرثومية. كما أن الحصيات البيضاء الصغيرة في جيوب اللوزتين ظاهرة شائعة لا تعني التهاباً مزمناً بالضرورة.

تنبيه مهم عند الكبار: تضخّم لوزة واحدة دون الأخرى، أو تقرّح أو نزف عليها، أو ألم مستمرّ في الحلق أو الأذن مع بحّة أو نقص وزن أو كتلة رقبية، لا يُعالَج كالتهاب متكرّر بل يحتاج تقييماً بلا تأخير، خصوصاً عند المدخّنين. والترشيح لأي خطوة، دوائية أو جراحية، يُحدَّد بالفحص السريري لا بقراءة الأعراض.
Treating tonsillitis in adults begins with identifying the cause, not with prescribing an antibiotic. Tonsillitis may be viral, which is the commonest scenario, or bacterial, most importantly group A streptococcus. The clinician distinguishes them on clinical grounds: the appearance of the tonsils and any exudate, body temperature, tender enlarged neck lymph nodes, and the absence of cough and coryzal symptoms, supported where needed by a throat swab or a rapid test.

For viral episodes the mainstay is supportive care: rest, adequate fluids, analgesia chosen by the clinician according to your health status, gargling with warm salt water, and avoiding irritants such as cigarettes and waterpipe smoke. Where streptococcal infection is suspected, an appropriate antibiotic is prescribed and completed for its full course even if symptoms settle quickly, because the aim is not only pain relief but reducing the risk of complications such as rheumatic fever and peritonsillar abscess.

Tonsillectomy is not a treatment for any single episode. It is considered in defined situations: documented recurrent episodes recorded by number and date over a year or more, recurrent peritonsillar abscess, obstructive sleep-disordered breathing linked to tonsillar enlargement, or clinical suspicion of a tumour. In adults recovery is longer and more painful than in children, usually around ten to fourteen days, with a risk of secondary bleeding that requires immediate review.

The limits stated frankly: tonsillectomy does not prevent colds, pharyngitis or sinus infections, it does not weaken immunity as is commonly believed, and it does not resolve bad breath arising from the teeth, the sinuses or the stomach. حدود الإجراء صراحةً: جراحة الحنك واللهاة قد تُخفّض صوت الشخير لكنها لا تُعالج انقطاع النفس النومي بالضرورة، واختفاء الصوت لا يعني اختفاء الانقطاعات، ولذلك لا يُقاس النجاح بشهادة الشريك وحدها بل بإعادة دراسة النوم عند الحاجة. وجراحة الأنف لا تُلغي جهاز ضغط الهواء لمن يحتاجه، لكنها قد تُحسّن تحمّله للقناع. وخسارة الوزن تُحسّن الحالة لكنها لا تُغني عن التقييم إن كانت الأعراض شديدة.

Treating snoring starts with understanding where it comes from. Snoring is the sound produced when soft tissues in the upper airway vibrate during sleep as the airway narrows and airflow becomes turbulent. The site of narrowing differs between individuals: the nose, because of a deviated septum, enlarged turbinates or polyps; the soft palate and uvula; the base of the tongue; the tonsils; or a small, set-back lower jaw.

The most important step before any treatment is distinguishing simple snoring from obstructive sleep apnoea, because the latter is a medical condition affecting the heart, blood pressure and concentration, and its severity is not measured by how loud the noise is. Features that call for a sleep study include witnessed pauses in breathing, night-time gasping or choking, daytime sleepiness including dozing while sitting or driving, morning headache, blood pressure that is hard to control, and night-time heart rhythm disturbance. A sleep study is what turns an impression into a diagnosis.

Treatment is built on cause and severity. General measures include weight reduction where relevant, avoiding sleeping on the back, avoiding alcohol and sedatives before bed, regular sleep timing, and treating nasal obstruction and allergy. In moderate to severe sleep apnoea, continuous positive airway pressure is the primary treatment. Mandibular advancement devices help selected patients. Surgery such as septoplasty, turbinate reduction, palate and uvula procedures and tonsillectomy is considered according to the site of narrowing demonstrated on examination.

The limits stated plainly: palate surgery may reduce the loudness of snoring but is not a guaranteed treatment for sleep apnoea, and silence does not mean the breathing pauses have gone. Success is therefore not judged by a bed partner's report alone but by repeating the sleep study where indicated. Nasal surgery does not replace positive airway pressure for someone who needs it, although it may improve tolerance of the mask. Weight loss improves the condition but does not remove the need for assessment when symptoms are significant.

العلاج يتبع السبب: غسل الأنف بمحلول ملحي وبخّاخ ستيروئيدي موضعي لالتهاب الأنف المزمن، ومضادات هستامين وتقليل التعرّض للمسبّبات في الحساسية، ومعالجة الجيوب أو اللحميات إن وُجدت، وتصغير المحاريم بالترددات الراديوية أو غيرها عند تضخّم مقاوم للدواء، وتقويم الحاجز عند انحراف مُثبَت يفسّر الأعراض، واستئصال اللحمية خلف الأنف عند الأطفال بحسب الحالة.

حدود العلاج بصراحة: تقويم الحاجز يوسّع مجرى الهواء ولا يعالج الحساسية ولا يمنع نزلات البرد، ولذلك يبقى العلاج الموضعي مطلوباً بعده عند المصاب بالتهاب أنف تحسّسي. وبخّاخ مضاد الاحتقان يفتح الأنف في دقائق لكن استخدامه أكثر من أيام قليلة يُحدث احتقاناً ارتدادياً يجعل الأنف معتمداً عليه ويزيد الانسداد. كما أن بعض المرضى يشعرون بانسداد مع مجرى مفتوح فعلاً، وهي حالة حسّية وظيفية لا تُصلحها جراحة إضافية.

علامات تستوجب تقييماً بلا تأخير لأنها لا تُعالَج كانسداد عادي: انسداد في جهة واحدة مستمرّ عند بالغ، رعاف متكرّر من الجهة نفسها، إفراز دموي أو صديدي أحادي، ألم وجهي مع تنميل أو ازدواج رؤية أو جحوظ، وفقد شمّ مستمرّ. وعند الطفل: انسداد أحادي كريه الرائحة يوجّه إلى جسم غريب، وعند حديث الولادة صعوبة الرضاعة مع التنفّس تُعدّ حالة عاجلة. الترشيح لأي علاج أو جراحة يُحدَّد بالفحص السريري وتنظير الأنف.
Treating nasal obstruction aims to identify and address the cause of reduced airflow through the nose, rather than simply opening the nose temporarily. Obstruction is a symptom, not a disease, and it has several causes that can coexist in the same patient: allergic or non-allergic rhinitis, a deviated nasal septum, enlarged turbinates, nasal polyps, adenoid enlargement behind the nose in children, rebound congestion from overuse of decongestant sprays, changes during pregnancy, and less common causes such as masses and tumours.

Assessment begins with a careful history: whether the blockage is one-sided or alternates between sides, whether it worsens at night or on lying down, and whether it is accompanied by sneezing, itching and watery eyes, which points towards allergy; by purulent discharge and facial heaviness, which points towards the sinuses; or by nosebleeds and one-sided pain, which calls for greater caution. Nasal endoscopy then visualises the septum, turbinates, drainage openings, polyps and the nasopharynx, and allergy testing or imaging may be requested depending on the findings.

Treatment follows the cause: saline irrigation and a topical intranasal steroid for chronic rhinitis; antihistamines and reduced allergen exposure in allergic rhinitis; treatment of sinus disease or polyps where present; turbinate reduction, by radiofrequency or another technique, for enlargement that resists medical therapy; septoplasty for a demonstrated deviation that explains the symptoms; and adenoidectomy in children when appropriate.

The limits stated frankly: septoplasty widens the airway but does not treat allergy and does not prevent colds, which is why topical treatment continues afterwards in patients with allergic rhinitis. A decongestant spray opens the nose within minutes, but using it for more than a few days causes rebound congestion, leaving the nose dependent on it and more blocked than before. In a child, one-sided foul-smelling obstruction suggests a foreign body, and in a newborn difficulty feeding and breathing is an emergency. سوائل الأذن الوسطى، أو التهاب الأذن الوسطى المُصلي، هي تجمّع سائل في الحيّز خلف طبلة أذن سليمة غير مثقوبة ودون علامات التهاب حادّ من ألم وحرارة. وسببها خلل في تهوية الأذن الوسطى عبر قناة أوستاش التي تصل الأذن بالبلعوم الأنفي، وهي شائعة جداً عند الأطفال بعد النزلات التنفّسية أو بعد التهاب أذن حادّ، وترتبط عندهم بتضخّم اللحمية خلف الأنف والحساسية والتعرّض لدخان التبغ.

المشكلة الأساسية ليست الألم بل السمع: السائل يمنع اهتزاز الطبلة بحرّية فيُحدث نقص سمع توصيلياً خفيفاً إلى متوسّطاً، وغالباً بلا شكوى واضحة من الطفل. لذلك تُلاحَظ الحالة عبر علامات غير مباشرة: رفع صوت التلفاز، طلب تكرار الكلام، تأخّر أو تراجع في نطق بعض الأصوات، شرود وضعف تركيز في الصف، انزعاج في الأذن أو شدّها، وتوازن أقلّ ثباتاً. وليس الطفل عنيداً أو غير منتبه كما يُظنّ.

التشخيص يقوم على تنظير الأذن مع قياس المطاوعة الذي يُظهر ضعف حركة الطبلة، ويُكمَّل بقياس سمع مناسب لعمر الطفل، مع فحص الأنف والبلعوم الأنفي لتحديد السبب المساهم. أمّا الخطة فتبدأ عادة بالمراقبة، لأن نسبة كبيرة من الحالات تزول تلقائياً خلال أسابيع إلى نحو ثلاثة أشهر، مع معالجة الحساسية وانسداد الأنف وإبعاد الطفل عن دخان التبغ.

إن استمرّ السائل في الأذنين مع نقص سمع يؤثّر على الكلام أو التعلّم، يُدرَس وضع أنبوب تهوية صغير في الطبلة، وقد يُضاف استئصال اللحمية خلف الأنف بحسب الحالة. وحدود هذا العلاج يجب أن تكون واضحة: مضادات الهستامين ومضادات الاحتقان لا تُنزّل السائل ولا تُعجّل زواله، وكورسات المضادات الحيوية المتكرّرة ليست حلاً لهذه الحالة، وأنبوب التهوية يعيد السمع طالما هو في مكانه ويسقط تلقائياً عادة خلال أشهر، وقد يعود السائل بعد سقوطه فيلزم تقييم جديد.

تنبيه مهم عند البالغين: سائل في أذن واحدة بلا سبب واضح لا يُعالَج كحالة أطفال، بل يستوجب فحص البلعوم الأنفي بالمنظار لاستبعاد كتلة تسدّ فتحة قناة أوستاش، خصوصاً مع نقص سمع أحادي أو انسداد أنف أو رعاف أو كتلة في الرقبة. والترشيح لأي خطوة يُحدَّد بالفحص السريري وقياس السمع لا بمدّة الأعراض وحدها.
Middle ear fluid, or otitis media with effusion, is a collection of fluid in the space behind an intact eardrum without the acute features of pain and fever. It results from impaired ventilation of the middle ear through the Eustachian tube, which links the ear to the nasopharynx. It is very common in children after respiratory infections or following acute otitis media, and in them it is associated with adenoid enlargement, allergy and exposure to tobacco smoke.

The main problem is not pain but hearing. Fluid prevents the eardrum from vibrating freely and produces a mild to moderate conductive hearing loss, usually without any clear complaint from the child. The condition is therefore noticed through indirect signs: turning the television up, asking for words to be repeated, delayed or regressed pronunciation of certain sounds, inattention and poor concentration in class, ear discomfort or tugging, and slightly less steady balance. The child is not being stubborn or inattentive, as is often assumed.

Diagnosis rests on otoscopy together with tympanometry, which demonstrates reduced eardrum mobility, completed by age-appropriate hearing testing and examination of the nose and nasopharynx to identify contributing causes. Management usually begins with observation, because a substantial proportion of cases resolve on their own within weeks to around three months, alongside treatment of allergy and nasal obstruction and keeping the child away from tobacco smoke.

If fluid persists in both ears with hearing loss that affects speech or learning, insertion of a small ventilation tube into the eardrum is considered, with adenoidectomy added in selected cases. The limits of treatment should be clear: antihistamines and decongestants do not drain the fluid or hasten its clearance, and repeated antibiotic courses are not a solution for this condition. A ventilation tube restores hearing while it remains in place, usually extrudes on its own within months, and fluid may return afterwards, requiring fresh assessment.

An important caution in adults: fluid in one ear without an obvious cause must not be managed as if it were a childhood problem. It calls for endoscopic examination of the nasopharynx to exclude a mass obstructing the Eustachian tube opening, particularly alongside one-sided hearing loss, nasal obstruction, nosebleeds or a neck lump. Suitability for any step is determined by clinical examination and hearing testing, not by symptom duration alone.

لحمية الأنف بالمعنى الطبي هي البوليبات الأنفية: نموّات لحمية طرية شفّافة أو رمادية اللون، غير مؤلمة عند اللمس، تنشأ من الغشاء المخاطي الملتهب في الأنف والجيوب وتتدلّى داخل مجرى الهواء. وهي مظهر لالتهاب مزمن في المخاطية لا مجرّد زائدة يمكن قصّها، ولذلك يُعالَج الالتهاب نفسه إلى جانب إزالة النموّ.

توضيح مهم يمنع خلطاً شائعاً في الأردن وغيره: كلمة اللحمية تُستخدم عند الناس لشيئين مختلفين تماماً. الأول البوليبات الأنفية عند الكبار غالباً وهي أنسجة التهابية، والثاني اللحمية خلف الأنف عند الأطفال أي اللوزة الثالثة وهي نسيج لمفي طبيعي يتضخّم فيسبّب شخيراً وتنفّساً فموياً وسوائل في الأذن. العلاج مختلف تماماً بين الحالتين، ولذلك لا يصحّ نقل معلومة من إحداهما إلى الأخرى.

الأعراض النمطية للبوليبات: انسداد أنفي مستمرّ لا يتبدّل بين الجهتين، نقص أو فقد لحاسة الشمّ وهو من أبرز ما يشكو منه المريض، إفرازات أنفية أمامية أو خلفية، وثقل في الوجه. وترتبط البوليبات كثيراً بالربو وبحساسية الأسبرين وبالتهاب الجيوب المزمن، ولذلك يُقيَّم المريض ككلّ لا الأنف وحده. والتشخيص يُبنى على تنظير الأنف، ويُكمَل بتصوير طبقي محوري عند التخطيط للجراحة.

العلاج يبدأ طبياً ويستمرّ طبياً: بخّاخ ستيروئيدي أنفي موضعي بانتظام وغسل ملحي، وكورس ستيروئيد فموي قصير في حالات مختارة بقرار الطبيب، وضبط الربو والحساسية. وتُجرى الجراحة بالمنظار عند انسداد شديد أو فقد شمّ أو فشل العلاج الدوائي، وتوجد علاجات حيوية حديثة تُخصَّص لحالات محدّدة وتُقرَّر مع طبيب مختص. لا تُستخدم قطرات فتح الأنف كعلاج للبوليبات، فهي لا تُذيبها وتسبّب احتقاناً ارتدادياً.

حدود العلاج بصراحة: الجراحة تُزيل البوليبات ولا تُوقف الالتهاب الذي أنتجها، والرجوع شائع خصوصاً إن توقّف العلاج الموضعي بعدها، ولذلك تُوصف الجراحة كخطوة تفتح الطريق للدواء لا كخاتمة. وقد تتحسّن حاسة الشمّ جزئياً فقط أو لا تعود إن كان فقدها طويلاً. وعلامات تستوجب تقييماً عاجلاً لأنها قد لا تكون بوليباً بسيطاً: \*\*نموّ في جهة واحدة\*\*، أو رعاف متكرّر، أو ألم وجهي شديد، أو تنميل الخدّ أو ازدواج رؤية أو جحوظ العين، أو تسرّب سائل مائي صافٍ من الأنف. الترشيح لأي علاج يُحدَّد بالفحص السريري وتنظير الأنف.
Nasal polyps are soft, pale or grey fleshy growths that are painless to touch and arise from chronically inflamed mucosa in the nose and sinuses, hanging down into the airway. They are a manifestation of chronic mucosal inflammation rather than a simple lump that can just be trimmed away, which is why the underlying inflammation is treated alongside removal of the growth itself.

An important clarification prevents a very common confusion: in everyday Arabic the same word is used for two entirely different things. The first is nasal polyps, mostly in adults, which are inflammatory tissue. The second is the adenoid behind the nose in children, normal lymphoid tissue that enlarges and causes snoring, mouth breathing and middle ear fluid. Treatment differs completely between the two, so information about one must not be applied to the other.

Typical symptoms of polyps include persistent nasal obstruction that does not alternate between sides, reduced or absent sense of smell, which is often the patient's most prominent complaint, anterior or posterior nasal discharge, and facial heaviness. Polyps are frequently associated with asthma, aspirin sensitivity and chronic rhinosinusitis, so the patient is assessed as a whole rather than the nose in isolation. Diagnosis rests on nasal endoscopy, completed by CT imaging when surgery is being planned.

Treatment begins medically and continues medically: a regular topical intranasal steroid spray with saline irrigation, a short oral steroid course in selected cases at the clinician's decision, and control of asthma and allergy. Endoscopic surgery is performed for severe obstruction, loss of smell or failure of medical treatment, and modern biologic therapies exist for specific cases and are decided with a specialist. Decongestant drops are not a treatment for polyps; they do not shrink them and they cause rebound congestion.

The limits stated frankly: surgery removes polyps but does not stop the inflammation that produced them, and recurrence is common, particularly if topical treatment is stopped afterwards. Surgery is therefore described as a step that opens the route for medication rather than an endpoint. The sense of smell may improve only partially, or not at all if it has been absent for a long time. Features requiring urgent assessment because they may not be a simple polyp include a growth on one side only, repeated nosebleeds, severe facial pain, cheek numbness, double vision or a protruding eye, and a clear watery leak from the nose. Suitability for any treatment is determined by clinical examination and nasal endoscopy.
