إصلاح التمزق في الكتف إجراء جراحي يُعيد تثبيت وتر ممزّق من أوتار الكفة المدوّرة إلى موضعه على رأس عظم العضد باستخدام مراسٍ صغيرة وخيوط جراحية. والتسمية الشائعة «تمزق عضلي» تعني في معظم الحالات تمزقاً في الوتر لا في العضلة نفسها، وأكثر الأوتار إصابةً هو وتر العضلة فوق الشوكة. يُجرى الإصلاح غالباً بالمنظار عبر شقوق صغيرة، وقد يُجرى بشقّ صغير مفتوح حسب حجم التمزق وخبرة الفريق.

يُطرح الإصلاح عادةً عند التمزق الكامل الحادث بعد إصابة واضحة، أو عند تمزق مزمن يرافقه ألم ليلي يمنع النوم وضعف في رفع الذراع لم يتحسّن بعد فترة كافية من العلاج الطبيعي الموجَّه. أما التمزقات الجزئية الصغيرة فتُدار في الغالب بلا جراحة: برنامج تقوية لعضلات الكتف ولوح الكتف، وتعديل مؤقّت للأنشطة فوق مستوى الرأس، ومعالجة أسباب الألم المرافقة.

من المهم معرفة ما لا تفعله هذه الجراحة: هي تخيط الوتر إلى العظم ولا تُعيد بناء عضلة ضمرت أو تحوّلت إلى نسيج دهني بعد سنوات من التمزق. وفي التمزقات المزمنة الواسعة قد يتعذّر تقريب طرف الوتر إلى موضعه، فيُناقش الطبيب بدائل مثل الإصلاح الجزئي أو نقل وتر أو استبدال المفصل العكسي. كما أنها لا تعالج خشونة مفصل الكتف المصاحبة، ولا تُلغي الحاجة إلى تأهيل طويل: التحام الوتر بالعظم يستغرق أسابيع، واستعادة القوة تمتد شهوراً.

تتأثر النتيجة بحجم التمزق ومدّته وجودة النسيج والعمر والتدخين والسكري، وقبل ذلك كله بالالتزام ببرنامج التأهيل. ولا يمكن الحكم على حاجتك للجراحة من قراءة صفحة أو تقرير تصوير: \*\*الترشيح يُحدَّد بالفحص السريري\*\* لقوة الكتف ومدى حركته مع مراجعة الصور، وقد يكون القرار الصحيح تأجيل الجراحة أو الاستغناء عنها.
Rotator cuff repair is surgery that reattaches a torn shoulder tendon to its footprint on the humeral head using small anchors and sutures. Although patients commonly describe the problem as a torn shoulder muscle, the tear is almost always in the tendon rather than in the muscle belly, and the supraspinatus tendon is the one most often involved. The repair is usually performed arthroscopically through keyhole incisions, and sometimes through a small open incision depending on tear size and the surgical team's judgement.

Repair is generally considered for a full-thickness tear after a clear injury, or for a chronic tear with night pain that prevents sleep and weakness on lifting the arm that has not improved after an adequate trial of guided physiotherapy. Most small partial tears are managed without surgery, using a supervised strengthening programme for the cuff and shoulder blade muscles, temporary modification of overhead activity, and treatment of any coexisting causes of pain.

It is just as important to know what the operation cannot do. It stitches tendon back to bone; it does not rebuild a muscle that has wasted or turned fatty after years of tearing. In large chronic tears the tendon end may not reach its footprint at all, and your surgeon may discuss partial repair, tendon transfer or reverse shoulder replacement instead. The operation does not treat coexisting shoulder arthritis, and it never removes the need for prolonged rehabilitation: tendon-to-bone healing takes weeks, and regaining useful strength takes months.

Results depend on the size and age of the tear, tissue quality, your age, smoking and diabetes, and above all on adherence to the rehabilitation plan. No web page or imaging report can tell you whether you need surgery. Candidacy is decided by clinical examination of shoulder strength and range of motion alongside the images, and for some patients the right decision is to delay surgery or avoid it altogether.

الرباط الصليبي الأمامي أحد الأربطة الداخلية للركبة، ووظيفته منع انزلاق عظم الساق للأمام ومنع الدوران الزائد، فهو حجر الاستقرار عند تغيير الاتجاه والقفز والهبوط. تنقطع أليافه عادة في إصابة رياضية بحركة التواء مفاجئة أو هبوط خاطئ، ويشكو المصاب من صوت طقّة وتورّم سريع ثم إحساس بعدم ثبات الركبة وأنها «تخون» عند الالتفاف.

الرباط المقطوع لا يلتحم من تلقاء نفسه، لذلك تُجرى \*\*إعادة بناء\*\* له لا مجرّد خياطة: يُؤخذ وتر من المريض نفسه — من أوتار الفخذ الخلفية أو الرباط الرَّضفي عادة — ويُثبَّت في أنفاق عظمية في الفخذ والساق ليقوم بوظيفة الرباط الأصلي، ويُجرى ذلك بالمنظار عبر ثقوب صغيرة. يُقيَّم الغضروف الهلالي والغضاريف المفصلية في الجلسة نفسها ويُعالَج ما يلزم.

قرار الجراحة ليس آلياً لكل قطع. من كان نشاطه لا يتضمّن تغيير اتجاه ولا رياضة محورية وركبته مستقرّة قد يُدار بالتأهيل وتقوية العضلات. أما الرياضيون أو من يعاني عدم ثبات متكرّر أو لديه قطع مصحوب بتمزّق هلالي يحتاج إصلاحاً، فالجراحة أنسب لحماية المفصل من التآكل مستقبلاً.

الحقيقة الأهم التي تُغيّر النتيجة: \*\*التأهيل هو نصف العلاج\*\*. النتيجة النهائية تعتمد على برنامج علاج طبيعي منظّم يمتدّ شهوراً — لا على العملية وحدها — والعودة للرياضة التلامسية تُقرَّر بمعايير قوة واتزان لا بمرور الوقت.
The anterior cruciate ligament is one of the knee’s internal ligaments; it prevents the shin bone sliding forward and limits excess rotation, making it the anchor of stability during cutting, jumping and landing. Its fibres usually tear in a sporting injury with a sudden twisting movement or bad landing, and the injured person reports a pop, rapid swelling, then a sense that the knee is unstable and “gives way” on turning.

A torn ligament does not heal on its own, so the operation is a \*\*reconstruction\*\* rather than a simple repair: a tendon graft from the patient — usually the hamstring tendons or the patellar tendon — is fixed in bone tunnels in the thigh and shin to take over the original ligament’s function, performed arthroscopically through small portals. The meniscus and joint cartilage are assessed in the same session and treated as needed.

The decision to operate is not automatic for every tear. Someone whose activities involve no cutting or pivoting sport and whose knee is stable may be managed with rehabilitation and muscle strengthening. Athletes, those with recurrent instability, or those whose tear is combined with a meniscal tear needing repair are better served by surgery to protect the joint from future wear.

The fact that most changes the outcome: \*\*rehabilitation is half the treatment\*\*. The final result depends on a structured physiotherapy programme running for months — not on the operation alone — and return to contact sport is decided by strength and balance criteria, not by elapsed time.

تنظير مفصل الحوض إجراء جراحي يُدخل فيه منظار رقيق وأدوات دقيقة عبر شقوق صغيرة إلى داخل مفصل الورك — وهو المفصل المقصود بهذه التسمية — لرؤية المفصل من الداخل ومعالجة ما يُكتشف فيه دون فتح واسع، ويُستعان بجهاز الأشعة أثناء الإجراء لضبط مسار الأدوات.

أكثر ما يُعالَج به: تمزق الغضروف الحُقّي، وهو الحلقة الغضروفية المحيطة بتجويف الورك وتعمل كحاشية مانعة للتسرّب تُحسّن ثبات المفصل؛ وانحشار الورك الفخذي الحُقّي حيث يوجد نتوء عظمي على رقبة الفخذ أو زيادة تغطية على حرف الحُقّ فيصطدم المفصل عند الثني واللف؛ وإزالة أجسام حرة داخل المفصل؛ ومعالجة التهاب الغشاء الزليلي أو إزالة نسيج مؤلم.

الأعراض النموذجية ألم في مقدمة الورك أو المغبن يزداد بالجلوس الطويل والقيادة والانحناء وارتداء الجوارب، وقد يرافقه إحساس بطقّة أو انحشار أو عجز عن إكمال حركة معيّنة. ولأن آلام الورك يُقلّدها كثير من الحالات، لا بدّ من استبعاد مصادر أخرى: الفقرات القطنية، والتهاب الأوتار على الجانب الخارجي، وألم مفصل العانة، والفتق الرياضي، وآلام الحوض النسائية.

حدود الإجراء واضحة ويجب قولها صراحة: هو لا يُصلح خشونة الورك المتقدّمة؛ فإذا كانت المسافة المفصلية ضيّقة والغضروف مستهلكاً، فقلّما يفيد التنظير ويكون النقاش حول استبدال المفصل. وهو لا يعالج ألماً منشأه خارج المفصل، ولا يُصحّح خللاً في شكل العظم يحتاج جراحة أوسع، ولا يُغني عن تأهيل يمتد أشهراً لاستعادة قوة عضلات الورك ونمط المشي.

\*\*الترشيح يُحدَّد بالفحص السريري\*\* لحركات الورك واختبارات الانحشار مع مراجعة الصور، وقد يُدعَم بحقنة مخدّر تشخيصية داخل المفصل: إن خفّ الألم مؤقتاً بعدها فذلك يرجّح أن مصدر الألم داخل المفصل فعلاً.
Hip arthroscopy is keyhole surgery in which a slim scope and fine instruments are passed through small incisions into the hip joint so the surgeon can inspect it from the inside and treat what is found without a large open exposure. An X-ray image intensifier is normally used during the procedure to guide instrument placement safely.

The conditions treated most often are a torn acetabular labrum, the ring of cartilage around the socket that acts as a seal and improves joint stability; femoroacetabular impingement, where extra bone on the femoral neck or over-coverage at the socket rim causes the joint to collide during flexion and rotation; loose bodies inside the joint; and inflamed synovium or other painful tissue that needs debridement.

Typical symptoms are pain in the front of the hip or the groin that worsens with prolonged sitting, driving, bending and putting on socks, sometimes with clicking, catching or an inability to complete a particular movement. Because many other conditions mimic hip pain, other sources must be excluded first: the lumbar spine, tendon problems on the outer hip, pubic joint pain, sports hernia and gynaecological pelvic pain.

The limits of the procedure need to be stated plainly. It does not fix advanced hip arthritis: when the joint space is narrowed and the cartilage worn, arthroscopy rarely helps and the discussion becomes one about hip replacement. It does not treat pain arising outside the joint, it does not correct a bony deformity that requires larger reconstructive surgery, and it never replaces a rehabilitation programme lasting months to restore hip muscle strength and normal gait.

Candidacy is decided by clinical examination of hip motion and impingement tests together with imaging, and may be supported by a diagnostic local anaesthetic injection into the joint: temporary relief after that injection makes an intra-articular source of pain much more likely.

أشهر ما يُنجَز خلاله: إصلاح الغضروف الهلالي أو تقليم الجزء الممزّق منه، وإزالة جسم حر يسبّب قفل الركبة، ومعالجة رقعة غضروف متضرّرة، واستئصال طيّة زليلية مؤلمة أو تحرير أنسجة متيبّسة، وأخذ عيّنة من الغشاء الزليلي عند الشكّ في مرض التهابي، وغسل المفصل وسحب القيح عند العدوى الحادة — وهذه حالة إسعافية لا تحتمل التأخير.

من الضروري الوضوح فيما لا يفعله المنظار: «تنظيف الركبة» أو غسلها بالمنظار ليس علاجاً لخشونة الركبة. لا يوقف تقدّم الخشونة ولا يُنمّي غضروفاً جديداً ولا يُصحّح تشوّه محور الساق، وقد يمنح راحة قصيرة ثم تعود الأعراض. الأساس في الخشونة إنقاص الوزن وتمارين التقوية المنتظمة وإدارة الألم، ثم استبدال المفصل عند الحالات المتقدّمة.

يصبح المنظار منطقياً عندما توجد علامة ميكانيكية حقيقية: قفل أو انحشار متكرّر للركبة، جسم حر مؤكّد، تمزق هلالي مؤلم لدى مريض مناسب، أو شكّ في عدوى أو مرض زليلي. أما الألم المنتشر مع تغيّرات تنكسية واسعة فهو نادراً ما يستفيد.

\*\*الترشيح يُحدَّد بالفحص السريري\*\* ومراجعة الصور معاً، لا بتقرير رنين مغناطيسي بمعزل عن الأعراض؛ فتغيّرات الرنين المفصلية شائعة جداً عند أشخاص لا يشكون من شيء، وعلاج تقرير لا مريض هو أكثر أسباب خيبة الأمل بعد الجراحة.
Knee arthroscopy is surgery in which a pencil-thin scope is passed through a small incision so the inside of the knee can be seen on a monitor, while a second incision admits fine instruments to treat what is found. It is used to diagnose a problem that imaging has not settled and to treat specific mechanical problems inside the joint, and it is usually performed as day-case surgery.

The commonest tasks are repairing the meniscus or trimming its torn portion, removing a loose body that locks the knee, addressing a damaged patch of articular cartilage, excising a painful synovial fold or releasing stiff tissue, taking a synovial biopsy when an inflammatory disease is suspected, and washing out the joint with drainage of pus in acute infection, which is an emergency that must not be delayed.

It is essential to be clear about what arthroscopy does not do. A knee clean-out or washout is not a treatment for knee osteoarthritis. It does not halt the progression of arthritis, it does not grow new cartilage, it does not correct malalignment of the leg, and any relief it gives may be short lived before symptoms return. The foundations of arthritis care are weight reduction, regular strengthening exercise and pain management, with joint replacement reserved for advanced disease.

Arthroscopy makes sense when there is a genuine mechanical sign: true locking or repeated catching, a confirmed loose body, a painful meniscal tear in a suitable patient, or suspicion of infection or synovial disease. Diffuse pain with widespread degenerative change rarely benefits.

Candidacy is decided by clinical examination together with the images, not by an MRI report read in isolation from symptoms. Degenerative MRI findings are extremely common in people with no complaints at all, and treating a report rather than a patient is the most frequent reason for disappointment after surgery.

يحدث التمزق نمطين: تمزق إصابي عند الشباب والرياضيين بعد لفّ الركبة والقدم مثبّتة على الأرض، وتمزق تنكسي يظهر تدريجياً عند متوسطي العمر وما بعدهم مع تغيّرات الخشونة، وقد يبدأ دون حادث واضح. الأعراض المعتادة ألم على خطّ المفصل، وتورم يظهر بعد ساعات، وصعوبة في القرفصاء والدوران، وأحياناً إحساس بالانحشار أو قفل حقيقي يمنع مدّ الركبة.

اختيار المسار يعتمد على موقع التمزق وشكله وعمر المريض وحالة الغضروف المحيط. التمزقات في المنطقة الخارجية الغنية بالتروية الدموية هي الأقدر على الالتحام، فتُفضَّل خياطتها عند المريض المناسب لأن الحفاظ على النسيج يحمي المفصل مستقبلاً. أما التمزقات التنكسية بلا انحشار ميكانيكي فتُدار في الغالب بالعلاج الطبيعي وتقوية عضلات الفخذ وإنقاص الوزن، والأدلة الحالية تُظهر أن نتائج ذلك تقارب نتائج التقليم في هذه الفئة.

ما لا يفعله العلاج يجب أن يكون واضحاً: لا الخياطة ولا التقليم يعيد الغضروف الهلالي إلى حالته الأصلية، والتقليم يزيل نسيجاً لا يُستعاد ويرفع احتمال الخشونة على المدى الطويل، ولا يعالج أيّ منهما خشونة موجودة أصلاً في المفصل، ولا يُغني عن التأهيل. كما أن الإصلاح يفرض قيوداً أطول على التحميل والثني من التقليم.

\*\*الترشيح يُحدَّد بالفحص السريري\*\* ومراجعة الصور معاً؛ فوجود تمزق في تقرير الرنين المغناطيسي لا يعني بذاته حاجةً للجراحة، إذ توجد تمزقات صامتة كثيرة بلا أعراض.
The meniscus consists of two crescent-shaped pads of fibrocartilage that sit between the thigh bone and the shin bone in each knee, acting as shock absorbers, load distributors and secondary stabilisers of the joint. Treating a meniscal tear is not one single operation but a decision between three routes: non-surgical care, suture repair of the tear, or trimming only the torn fragment when repair is not feasible.

Tears occur in two patterns. Traumatic tears affect younger and athletic patients after twisting the knee with the foot planted, while degenerative tears develop gradually in middle-aged and older people alongside arthritic change and may begin with no clear injury at all. Usual symptoms are joint-line pain, swelling that appears over hours rather than instantly, difficulty squatting and pivoting, and sometimes catching or true locking that prevents the knee straightening.

The choice of route depends on the location and pattern of the tear, the patient's age and the state of the surrounding cartilage. Tears in the outer, better-vascularised zone have the best healing potential, so repair is preferred in suitable patients because preserving meniscal tissue protects the joint in the long term. Degenerative tears without mechanical catching are usually managed with physiotherapy, quadriceps strengthening and weight reduction, and current evidence shows outcomes in this group are broadly comparable to those after arthroscopic trimming.

What treatment cannot do must be equally clear. Neither repair nor trimming restores the meniscus to its original state; trimming removes tissue that never comes back and increases the long-term risk of arthritis; neither procedure treats arthritis that is already present; and neither removes the need for rehabilitation. A repair also imposes longer restrictions on weight-bearing and deep bending than a trim does.

Candidacy is decided by clinical examination together with imaging. A tear reported on MRI does not by itself mean surgery is needed, because silent asymptomatic tears are common.

عملية بانكارت بالمنظار إجراء جراحي يُعيد تثبيت الحلقة الغضروفية الأمامية السفلية للكتف مع المحفظة والرباط المرتبط بها إلى حرف تجويف الكتف، باستخدام مراسٍ صغيرة وخيوط تُمرَّر بالمنظار عبر شقوق صغيرة. تُجرى لمعالجة عدم ثبات الكتف الأمامي بعد خلع متكرّر.

عند الخلع الأول للكتف تتمزّق هذه الحلقة عن حرف التجويف في معظم الحالات، فيفقد المفصل حاجزه الأمامي ويصبح عرضة للخلع مرة بعد أخرى بحركات أقل عنفاً، حتى يصل بعض المرضى إلى خلع أثناء النوم أو عند لبس الملابس. يزيد احتمال التكرار كثيراً عند صغار السن والرياضيين وفي الرياضات التلاحمية، والتكرار نفسه يُتلف العظم والغضروف مع الوقت.

قرار الجراحة ليس آلياً بعد كل خلع؛ يُقيَّم عدد الخلعات وعمر المريض ونوع نشاطه ومدى التلف. ويجب قبل ذلك تصوير مقطعي أو تصوير مخصّص لقياس فقدان العظم في حرف التجويف، لأن هذا القياس هو الذي يوجّه الاختيار: فإذا كان الفقدان العظمي كبيراً أو كانت هناك فجوة عظمية متداخلة في رأس العضد، فإن الإصلاح اللين وحده يكون معدّل تكرار الخلع بعده أعلى، ويُفضَّل إجراء عظمي مثل نقل قطعة عظمية إلى حرف التجويف.

حدود الإجراء يجب أن تُقال صراحة: هو يُصلح حاجزاً ممزّقاً ولا يُعيد عظماً مفقوداً، ولا يُصلح خلعاً سببه رخاوة عامة في الأربطة أو خلعاً يستطيع المريض إحداثه بإرادته — تلك حالات تبدأ بالتأهيل لا بالمشرط. ولا يعيد الكتف فوراً إلى الرياضة: تبقى الحمّالة فترة، ثم تُستعاد الحركة تدريجياً، والعودة إلى الرياضات التلاحمية تحتاج عادة عدة أشهر وقراراً سريرياً لا رغبةً شخصية.

Arthroscopic Bankart repair is keyhole surgery that reattaches the torn antero-inferior labrum, together with its capsule and ligament, back onto the rim of the shoulder socket using small anchors and sutures passed through keyhole portals. It is performed to treat anterior shoulder instability after recurrent dislocation.

When a shoulder dislocates for the first time, this labral ring is torn off the socket rim in most cases. The joint loses its front-side bumper and becomes vulnerable to dislocating again with progressively less force, until some patients dislocate while asleep or while getting dressed. The risk of recurrence is much higher in young patients, athletes and contact sports, and each further dislocation adds damage to bone and cartilage over time.

Surgery is not automatic after every dislocation. The number of dislocations, the patient's age and activity, and the extent of damage are all weighed. Crucially, a CT scan or dedicated imaging is needed to measure bone loss at the socket rim, because that measurement drives the choice of operation: with significant glenoid bone loss, or an engaging bony defect in the humeral head, a soft-tissue repair alone carries a higher redislocation rate and a bone-block procedure that transfers bone to the socket rim is usually preferred.

The limits must be stated plainly. This operation repairs a torn bumper; it does not restore missing bone. It does not correct instability caused by generalised ligament laxity, nor dislocations a patient can produce voluntarily, both of which begin with rehabilitation rather than surgery. And it does not return a shoulder to sport immediately: a sling is worn for a period, motion is regained in stages, and return to contact sport usually takes several months and is a clinical decision rather than a personal preference.

Candidacy is decided by clinical examination with instability testing alongside imaging, not by the number of dislocations alone.
