الجراحة التحفظية لسرطان الثدي عملية يُستأصل فيها الورم مع هامش من النسيج السليم المحيط به، مع الإبقاء على باقي الثدي، وتُستكمل في معظم الحالات بعلاج إشعاعي. وهي خيار معتمد لمن تنطبق عليها معايير محدّدة، ويُقارن معها دائماً بديل الاستئصال الكامل، والقرار يُتّخذ مع الفريق المعالج لا بمقارنة تجربة قريبة أو حالة قرأت عنها.

لا تبدأ هذه الجراحة إلا بعد أن يكون هناك \*\*تقرير نسيجي واضح من خزعة\*\* تُحدّد نوع الخلايا ودرجتها ومستقبلاتها، مع تصوير كامل للثديين والإبط. فالخزعة قبل أي قرار ليست خطوة إدارية بل هي أساس الخطة كلّها: نوع الجراحة، والحاجة إلى علاج دوائي قبلها لتصغير الورم، والتعامل مع العقد اللمفاوية. ويُقيَّم الترشيح بحسب حجم الورم بالنسبة لحجم الثدي، وعدد بؤره، وموقعه، ونتيجة الفحص الجيني عند وجود دواعٍ، وإمكان تلقّي العلاج الإشعاعي، ورغبتك المُطلعة.

تُجرى العملية عادة بتخدير عام وتستغرق عادة من ساعة إلى ساعتين، وتطول إن رافقتها إعادة تشكيل للثدي. ويُصاحبها في الغالب تقييم العقد اللمفاوية بأخذ العقدة الخافرة لتحديد ما إذا كان الورم قد وصل إلى الإبط. بعدها يُفحص النسيج المستأصل بدقّة، وقد يُظهر التقرير النهائي أن الهامش غير كافٍ فيُحتاج إلى توسيع الاستئصال أو — في بعض الحالات — إلى استئصال كامل. هذا ليس فشلاً بل جزء معروف من مسار العلاج، ومن الأفضل أن تعرفيه قبل الجراحة لا بعدها.

حدود الجراحة بصراحة، وبلا وعود: هي \*\*لا تُنهي رحلة العلاج بمجرد انتهائها\*\*. العلاج الإشعاعي مكوّن أساسي في معظم الحالات، وقد تُوصف علاجات دوائية أو هرمونية لسنوات بحسب التقرير النسيجي، ولا يمكن لأحد أن يضمن عدم عودة الورم في الثدي نفسه أو في مكان آخر — ولهذا تبقى المتابعة والتصوير الدوري ضرورة دائمة. كما أن شكل الثدي وحجمه قد يتغيّران، ولا يمكن ضمان تماثل تام مع الثدي الآخر، وقد يبقى تنميل أو تيبّس أو تغيّر في الإحساس حول الجرح، ويؤثّر الإشعاع على قوام الجلد والنسيج.

الجانب الإنساني ليس هامشاً في هذا القرار: من حقّك وقت لتفهمي، ورأي ثانٍ، ومرافق في كل موعد، وشرح مكتوب بلغة تفهمينها. اسألي قبل الجراحة — لا بعدها — عن الحفاظ على الخصوبة إن كان العلاج الكيميائي محتملاً وكنتِ في سنّ الإنجاب، وعن أثر الخطة على العمل والحياة الأسرية، وعن إعادة التشكيل ومتى تُجرى. والدعم النفسي والمجموعات المتخصّصة جزء من العلاج لا رفاهية؛ اطلبيه صريحاً، فالخوف والحزن في هذه المرحلة طبيعيان تماماً. والترشيح النهائي يُحدَّد بالفحص السريري والتصوير والتقرير النسيجي وقرار فريق متعدّد التخصصات.
Breast-conserving surgery removes the tumour together with a margin of healthy surrounding tissue while keeping the rest of the breast, and in most cases is completed by radiotherapy. It is an established option for patients who meet defined criteria, is always weighed against the alternative of total mastectomy, and the decision is made with the treating team rather than by comparing a relative's experience or a case read about online.

This surgery does not begin until there is \*\*a clear pathology report from a biopsy\*\* defining the cell type, grade and receptors, along with complete imaging of both breasts and the axilla. Biopsy before any decision is not an administrative step; it is the foundation of the entire plan, including the type of operation, whether drug treatment is given first to shrink the tumour, and how the lymph nodes are handled. Suitability is assessed from tumour size relative to breast size, the number of foci, the location, genetic testing where indicated, the ability to receive radiotherapy, and your informed preference.

The operation is usually performed under general anaesthesia and typically takes one to two hours, longer if reshaping of the breast is combined with it. It is usually accompanied by assessment of the lymph nodes through sentinel node biopsy to determine whether the tumour has reached the axilla. The excised tissue is then examined in detail, and the final report may show that the margin is inadequate, requiring wider excision or, in some cases, mastectomy. That is not a failure but a recognised part of the pathway, and it is better to know it before surgery than after.

The limits, stated honestly and without promises: this operation \*\*does not end the treatment journey when it finishes\*\*. Radiotherapy is an essential component in most cases, drug or hormonal treatment may be prescribed for years depending on the pathology, and nobody can guarantee that the tumour will not return in the same breast or elsewhere, which is why ongoing follow-up and periodic imaging remain necessary. The shape and size of the breast may change, symmetry with the other side cannot be guaranteed, numbness, tightness or altered sensation around the scar may persist, and radiotherapy affects the texture of skin and tissue.

The human side of this decision is not a footnote. You are entitled to time to understand, to a second opinion, to a companion at every appointment, and to a written explanation in language you understand. Ask before surgery, not after, about fertility preservation if chemotherapy is possible and you are of childbearing age, about the effect of the plan on work and family life, and about reconstruction and its timing. Psychological support and specialised support groups are part of treatment rather than a luxury, so ask for them explicitly; fear and grief at this stage are entirely normal. Final eligibility is determined by clinical examination, imaging, the pathology report and a multidisciplinary team decision.

خزعة الثدي إجراء يُؤخذ فيه جزء صغير من نسيج الكتلة أو من المنطقة المشتبهة ليُفحص مجهرياً. وهي الخطوة التي تحدّد طبيعة النسيج بشكل قاطع؛ فالتصوير — ماموغرام أو موجات صوتية أو رنين — يرجّح ويصف، لكنه لا يحسم. ولهذا السبب لا يُتّخذ أي قرار علاجي في الثدي، ولا تُوضع خطة جراحة أو دواء، قبل الحصول على تقرير نسيجي واضح.

تُختار طريقة الخزعة بحسب حجم الكتلة وموقعها وشكلها في التصوير: خزعة بالإبرة الرفيعة لسحب سائل أو خلايا، والخزعة الأساسية بالإبرة الأغلظ التي تأخذ أسطوانة نسيجية وهي الأكثر استخداماً لأنها تعطي تشخيصاً بنيوياً كاملاً، والخزعة بالشفط بالتفريغ لتكتّلات دقيقة، وقد تُوجَّه بالموجات الصوتية أو بالماموغرام أو بالرنين بحسب ما يُظهر المنطقة أفضل. تُوضع في الغالب علامة معدنية دقيقة داخل مكان الخزعة لتحديد الموضع بدقّة إن احتيج لجراحة أو علاج لاحقاً، وهي علامة آمنة وصغيرة.

يُجرى الإجراء عادة بتخدير موضعي في العيادة أو وحدة الأشعة، ويستغرق عادة من 15 إلى 30 دقيقة مع مكوث قصير للضغط والتضميد، ثم تُرسل العيّنة إلى المختبر. من الطبيعي أن يكون انتظار النتيجة أثقل من الإجراء نفسه؛ والقلق في هذه الأيام ردّ فعل مفهوم لا ضعف. اطلب مرافقاً في موعد النتيجة، واسمع التقرير من طبيبك لا من ورقة تحاول تفسيرها وحدك، ولا تبحث عن مصيرك في مجموعات الإنترنت. الدعم النفسي في هذه المرحلة جزء من الرعاية وليس رفاهية، ومن حقّك أن تسأل عنه.

حدود الإجراء بصراحة: الخزعة \*\*تشخيص لا علاج\*\* — هي لا تُزيل الكتلة ولا تعالجها (إلا في حالات محدودة تُزال فيها كتلة صغيرة بالشفط)، ولا يمكن لأحد أن يَعِدك بنتيجة قبل صدور التقرير. وتصحيحاً لخرافة خطيرة ومنتشرة: \*\*الإبرة لا «تُنشر» السرطان ولا تُحرّكه\*\*، وتأجيل الخزعة خوفاً من هذا الاعتقاد يؤخّر التشخيص ويضرّ فعلاً. كذلك نتيجة سليمة لا تُطمئن تماماً إذا كان التصوير مشبوهاً بقوة، وفي هذه الحالة تُعاد الخزعة أو تُجرى جراحياً، وقد تحتاج العيّنة فحوصاً إضافية على النسيج تستغرق أياماً إضافية.

أخيراً، لا يُقاس قرار الخزعة بتجربة قريبة أو صديقة؛ فالاختيار بين الطرق، وضرورة الخزعة من أصلها، يُحدَّدان بالفحص السريري وقراءة التصوير معاً. وإن كانت النتيجة تحتاج خطوة تالية، تُناقش الخطة عادة في فريق متعدّد التخصصات لا بقرار فردي.
A breast biopsy is a procedure in which a small amount of tissue is taken from a lump or a suspicious area so that it can be examined under the microscope. It is the step that establishes the nature of the tissue definitively; imaging — mammography, ultrasound or MRI — describes and estimates probability but does not settle the question. For that reason no treatment decision about the breast, and no plan for surgery or medication, is made before a clear pathology report is available.

The technique is chosen according to the size, position and imaging appearance of the lesion: fine-needle aspiration to draw fluid or cells; core biopsy with a wider needle that removes a cylinder of tissue, the most commonly used because it gives a full structural diagnosis; and vacuum-assisted biopsy for fine calcifications. Guidance may be by ultrasound, mammography or MRI, whichever shows the area best. A tiny metallic marker is usually left inside the biopsy site so the location can be identified precisely if surgery or further treatment is needed later; it is small and safe.

The procedure is usually done under local anaesthesia in a clinic or radiology unit, typically takes 15 to 30 minutes with a short stay for pressure and dressing, and the sample is then sent to the laboratory. It is normal for the wait for results to feel heavier than the procedure itself; anxiety during these days is an understandable reaction, not a weakness. Bring someone with you to the results appointment, hear the report from your doctor rather than trying to interpret a printout alone, and do not search for your prognosis in online groups. Psychological support at this stage is part of care rather than a luxury, and you are entitled to ask for it.

The limits, stated plainly: a biopsy is \*\*diagnostic, not therapeutic\*\*. It does not remove or treat the lump, except in limited situations where a small lesion is excised by vacuum device, and nobody can promise you a result before the report is issued. Correcting a dangerous and widespread myth: \*\*a needle does not spread cancer or stir it up\*\*, and postponing a biopsy because of that belief delays diagnosis and causes real harm. A benign result is also not fully reassuring when imaging is strongly suspicious; in that case the biopsy is repeated or performed surgically. The specimen may need additional tissue tests that take further days.

Finally, the decision to biopsy is not measured against a relative's or friend's experience. Both the choice of technique and the need for a biopsy at all are determined by clinical examination together with the imaging. If the result requires a next step, the plan is usually discussed by a multidisciplinary team rather than decided by one person.
