Brain tumour surgery is a neurosurgical procedure in which a limited window is opened in the skull so the surgeon can reach the tumour and remove as much of it as can be removed safely, while protecting healthy brain tissue and the pathways that control movement, speech and vision. The tumour may be primary, arising in the brain or its coverings, or secondary, having spread from another organ; it may be benign or malignant. Imaging alone cannot settle which of these it is. Only examination of the tumour tissue under the microscope can.

Tissue is the foundation of every decision that follows. The biopsy or the specimen taken during resection is sent for histopathology, and the tumour type and grade are what determine whether radiotherapy, drug treatment or imaging follow-up alone is appropriate afterwards. This is why some patients are offered a small biopsy before any major operation: treating without a tissue diagnosis is guesswork.

Understanding the limits of surgery matters as much as understanding its benefits. Resection does not repair neurological damage that occurred before the operation, it does not remove the need for long-term imaging follow-up, and it does not guarantee that certain tumour types will not return. In many cases removal is deliberately partial rather than incomplete by failure: when a tumour is interwoven with a critical centre or wrapped around a major artery, the surgeon may choose to leave part of it behind instead of risking permanent weakness or loss of speech, and treatment is then continued by other means.

Care here is the work of a multidisciplinary team rather than a single doctor: the neurosurgeon, the diagnostic radiologist, the pathologist, the oncologist, the radiation oncologist, and afterwards the rehabilitation, physiotherapy and speech therapy team. The goals of the operation are defined in advance and explained to the patient: obtaining a tissue diagnosis, relieving pressure inside the skull, controlling seizures, and preserving or improving function as far as possible.

Suitability for surgery is decided by a neurological examination, imaging and team discussion, never by reading a web page. Age, general fitness, tumour location, the number of lesions, and heart or clotting conditions can change the plan entirely, and may make a non-surgical option the better choice.

Diagnosing a brain tumour is a stepwise medical process that begins with the patient's history and neurological examination, moves through imaging, and ends at the microscope with examination of tumour tissue. The aim is not simply to prove that a mass exists, but to establish its type, its grade, its exact location and its relationship to the centres for movement, speech and vision, because these details determine what treatment is appropriate and whether treatment is needed now at all rather than surveillance.

The neurological examination comes before any machine. The doctor assesses strength, sensation, balance, speech, memory, eye movement, visual acuity and visual fields, and examines the back of the eye for signs of raised pressure inside the skull. This examination guides the choice of imaging and detects things scans cannot show, such as a gradual decline in one specific skill or a change in behaviour that relatives notice before the patient does.

Contrast-enhanced MRI is the core investigation, supplemented when needed by vascular imaging, functional studies, or CT in emergency situations. Imaging has a limit that every patient should understand: a scan makes a diagnosis more or less likely but does not prove the tumour type, and inflammatory conditions, abscesses and post-treatment changes can all mimic a tumour on images. For this reason a major treatment decision is not built on a radiology report alone.

The definitive answer comes from biopsy and histopathology: a small sample taken under image guidance or during a resection operation, examined microscopically and completed with tissue and molecular tests that classify the tumour precisely. That classification is what separates a tumour needing only surveillance from one needing surgery, radiotherapy or drug treatment, and it is what prevents a patient being treated with a plan that does not fit their disease. In some critical locations the team may decide against biopsy and rely on monitoring instead, and that too is a team decision rather than an individual choice.

Diagnosis here is multidisciplinary work bringing together the neurologist, neurosurgeon, radiologist, pathologist and oncologist, with the case discussed jointly. The order and urgency of the tests are set by clinical examination and symptom severity, not by online reading: some cases are worked up over weeks, and some cannot safely wait an hour.

Meningiomas are tumours that arise from the membranes covering the brain and spinal cord rather than from brain tissue itself, so they grow from the outside and press on the brain instead of infiltrating it. Most are slow-growing and not malignant, and many are discovered incidentally on a scan ordered for another reason such as headache or a head injury. Symptoms depend on location: a tumour on the convex surface of the brain may cause a seizure or weakness of a limb, one near the optic nerve may cause declining vision, and one at the skull base may cause facial numbness or dizziness.

Not every meningioma needs treatment now, and this is one of the facts patients most often do not know. If the tumour is small, causing no symptoms and not pressing on a critical structure, the correct decision may be to monitor it with imaging at defined intervals. Surveillance is a considered treatment plan, not neglect and not procrastination, but it only works if the imaging appointments are kept exactly, because early growth produces no symptom at all.

When treatment is required, the main options are surgery, radiotherapy, or a combination, chosen according to location, size, symptoms, and the patient's age and general condition. Surgery aims to remove the tumour together with the involved membrane as far as the location allows, and it is often most suitable for accessible convexity tumours on the surface. For tumours adherent to large vessels or sensitive nerves at the skull base, deliberately partial removal followed by radiotherapy to the remnant may be the safer strategy.

The limits of treatment should be clear before it starts. Radiotherapy does not dissolve the tumour immediately; its aim is to stop growth, and the tumour may remain visible on scans for years while being inactive. Surgery does not remove the possibility of recurrence, particularly after partial removal or with a higher-grade tumour. Neither surgery nor radiation repairs neurological damage that was already established before treatment, such as a lost part of the visual field. Grade is never determined by imaging: it is determined by histopathology once tissue has been obtained, and it guides the decision on surveillance or additional treatment.

Decisions here are made by a multidisciplinary team of neurosurgeon, radiologist, radiation oncologist and pathologist, sometimes with an ophthalmologist or ENT surgeon depending on the site. Suitability for any option is decided by clinical examination, imaging and team discussion, not by comparing your case with someone else's.

Treating brain cancer means building an individual, combined plan rather than performing a single procedure. "Brain cancer" is an umbrella term covering malignant tumours that behave very differently: primary tumours arising from the brain's own cells, such as gliomas, and secondary tumours that have spread to the brain from another organ. The distinction is not a matter of wording; it changes the treatment plan completely, and it cannot be settled by imaging alone but only by examining tissue under the microscope.

Tissue is the starting point, not the conclusion. A sample is obtained by image-guided biopsy or during resection surgery, examined by a pathologist, and completed with molecular tests that define the tumour's type, grade and biological features. Every subsequent decision rests on that result: whether to begin with surgery or radiotherapy, whether drug treatment is added, and in what sequence. A plan made before the tissue result is known is guesswork, which is why patients are asked to wait days for the report. That wait is part of correct treatment, not a delay in it.

The available tools are usually combined. Surgery relieves pressure, provides tissue and removes what can be removed safely. Radiotherapy is used in its various forms, including precisely targeted radiation to a limited focus. Drug treatment, whether chemotherapy or targeted therapy, is chosen according to tumour type and molecular features. Supportive care matters just as much: controlling seizures, reducing swelling around the tumour, treating nausea and pain, physiotherapy and speech therapy, and psychological support for the patient and family.

The limits are stated openly. Surgery may be deliberately partial when the tumour is interwoven with a critical centre, because leaving part of it behind is better than permanent paralysis. Radiation does not dissolve a tumour immediately; its aim is to control growth, and post-radiation scans can show changes that resemble tumour growth but are not. That is why images are compared over time and no decision is based on one scan. No treatment erases neurological damage that was already established before treatment began. Herbal and alternative remedies do not replace treatment and do not stop a tumour, and relying on them instead of the medical plan wastes time that cannot be recovered.

This care is by necessity multidisciplinary: neurosurgeon, oncologist, radiation oncologist, radiologist, pathologist and rehabilitation team. Suitability for each step is determined by clinical examination, general condition, the tissue result and team discussion, not by comparing your case with another patient's or by what you read online.

An acoustic neuroma, also called a vestibular schwannoma, is a usually benign and slow-growing tumour arising from the sheath of the eighth cranial nerve, which carries hearing and balance signals from the inner ear to the brain. It sits in a narrow canal between the ear and the skull cavity, and because it grows in a tight space its symptoms begin early and quietly: gradual hearing loss in one ear, ringing in that same ear, and a sense of unsteadiness or dizziness on rapid movement. As it enlarges it may press on neighbouring nerves and cause facial numbness or weakness of facial movement.

The most neglected symptom is hearing loss in one ear. Many patients attribute it to wax, a cold or ageing and postpone assessment for years. The practical rule is that any hearing loss or ringing in one ear but not the other deserves a hearing test and medical assessment, because a difference between the two ears is not a coincidence. Diagnosis rests on audiometry together with dedicated MRI of the inner ear region and the angle at the base of the skull, which is the study that reveals small tumours invisible on ordinary CT.

There are three management pathways, and choosing between them is not a matter of personal preference: surveillance with repeated imaging, microsurgical removal, or targeted stereotactic radiotherapy delivering a concentrated dose aimed at stopping growth. Small tumours without troublesome symptoms may be monitored for years, because some never grow or grow extremely slowly; surveillance is a considered plan that commits you to precise imaging dates, not simply waiting. Large tumours, those pressing on the brainstem, and those causing deteriorating balance may need intervention.

The limits of treatment must be understood before deciding. Neither surgery nor radiation restores hearing that has already been lost; the goal is to protect what remains and to halt tumour growth, not to recover the past. Tinnitus may persist after treatment and may not improve, and unsteadiness needs balance rehabilitation and weeks of exercises rather than time alone. There is a risk to the adjacent facial nerve that varies with tumour size, location and treatment type, and it is discussed openly before consent. The tumour type is only finally confirmed by histopathology when surgery is performed.

Decisions here are made by a team of neurosurgeon, ENT surgeon, audiologist, radiation oncologist and radiologist. Suitability for any pathway is determined by clinical examination, audiometry, imaging, and the patient's age and general condition, not by online reading or by another patient's experience.
