استئصال الزائدة الدودية عملية جراحية تُزال فيها الزائدة الدودية، وهي أنبوب صغير مغلق الطرف يتصل ببداية الأمعاء الغليظة في الجهة السفلية اليمنى من البطن، وذلك عندما تلتهب وتسبّب ألماً حاداً. التهاب الزائدة الحاد يُعامل كحالة طارئة، لأن استمرار الالتهاب قد يؤدي إلى انثقاب الزائدة وانتشار العدوى في تجويف البطن.

الصورة النموذجية تبدأ بألم غامض حول السرّة ينتقل خلال ساعات إلى أسفل يمين البطن ويزداد مع الحركة والسعال، مع فقدان الشهية والغثيان وربما حرارة خفيفة. لكن الصورة قد تكون غير نمطية عند الأطفال والحوامل وكبار السنّ. التشخيص سريري بالأساس ويُدعم بتحاليل الدم والتصوير بالأمواج فوق الصوتية أو الطبقي المحوري، وقرار الجراحة يُحدَّد بالفحص السريري والمراقبة داخل المستشفى، لا بقراءة صفحة ولا بنتيجة تحليل واحدة.

تُجرى العملية في معظم الحالات بالمنظار عبر ثلاث فتحات صغيرة: يُفصل مجرى الدم الصغير المغذّي للزائدة، وتُغلق قاعدتها، ثم تُخرَج داخل كيس. وقد يقرّر الجراح تحويل العملية إلى شقّ صغير مفتوح إذا كانت الزائدة منثقبة أو الالتصاقات كثيفة أو الرؤية غير آمنة، وهذا قرار سلامة لا يُعدّ فشلاً. التخدير عام في العادة.

في حالات مختارة من الالتهاب البسيط غير المعقّد يُطرح علاج بالمضادات الحيوية دون جراحة، لكنه لا يناسب كل مريض ويحمل احتمال عودة الالتهاب لاحقاً، والقرار يُتخذ داخل المستشفى بعد تقييم كامل ومتابعة قريبة.

حدود الإجراء: استئصال الزائدة يعالج التهاب الزائدة فقط. لا يعالج القولون العصبي، ولا آلام البطن المزمنة، ولا مشاكل المرارة أو المبيض، ولذلك لا تُستأصل الزائدة كحلّ لألم بطن غير مفسّر ولا وقائياً بلا التهاب. ومن الخرافات الخطيرة تناول مسكّن قويّ أو مليّن أو استخدام حقنة شرجية عند الشك بالتهاب الزائدة، لأن ذلك يخفي الأعراض ويزيد خطر الانثقاب: امتنع عن الأكل والشرب واتّجه إلى الطوارئ.
An appendectomy is an operation to remove the appendix, a small blind-ended tube attached to the beginning of the large intestine in the lower right side of the abdomen, when it becomes inflamed and painful. Acute appendicitis is managed as an emergency, because continuing inflammation can perforate the appendix and spread infection through the abdominal cavity.

The classic picture begins as vague pain around the navel that shifts within hours to the lower right abdomen and worsens with movement or coughing, together with loss of appetite, nausea and sometimes a low-grade fever. The presentation can be atypical in children, in pregnancy and in older adults. Diagnosis is mainly clinical, supported by blood tests and ultrasound or CT imaging; the decision to operate is made through clinical examination and in-hospital observation, not by reading a web page or a single test result.

Most appendectomies are done laparoscopically through three small incisions: the small blood supply of the appendix is divided, its base is secured, and the specimen is removed inside a retrieval bag. The surgeon may convert to a small open incision when the appendix is perforated, when adhesions are dense, or when the view is not safe. That is a safety decision, not a failure. General anaesthesia is used in most cases.

In selected patients with simple, uncomplicated inflammation, antibiotic treatment without surgery may be discussed. It does not suit everyone, it carries a risk that the inflammation returns later, and the choice is made in hospital after a full assessment with close follow-up.

Limits of the procedure: an appendectomy treats appendicitis and nothing else. It does not treat irritable bowel syndrome, chronic abdominal pain, gallbladder disease or ovarian problems, so the appendix is not removed as a solution for unexplained abdominal pain, nor preventively when it is not inflamed. One dangerous myth is taking a strong painkiller, a laxative or an enema when appendicitis is suspected: this masks the signs and raises the risk of perforation. Stop eating and drinking and go to the emergency department.

استئصال المرارة عملية تُزال فيها المرارة، وهي كيس صغير يقع أسفل الكبد ويخزّن العصارة الصفراوية ويفرغها في الأمعاء بعد الطعام الدسم. تُزال المرارة عندما تصبح مصدر أعراض متكرّرة أو مضاعفات، وأشهر السبب هو حصوات المرارة والتهابها.

الأعراض النموذجية ألم يبدأ أعلى يمين البطن أو تحت عظمة القصّ بعد وجبة دسمة، ثابت وضاغط لمدة تتراوح عادةً من عشرات الدقائق إلى بضع ساعات، وقد ينتشر إلى الظهر أو الكتف الأيمن مع غثيان وتقيّؤ. أما تكرار الألم الحاد، أو التهاب المرارة، أو انتقال حصوة إلى القناة الصفراوية مع اصفرار وبول غامق، أو التهاب البنكرياس الحصوي، فكلها مؤشّرات قويّة للجراحة. وجود حصوات صامتة بلا أي أعراض لا يعني بالضرورة عملية، والترشيح يُحدَّد بالفحص السريري والتصوير لا بالقراءة.

تُجرى العملية في معظم الحالات بالمنظار عبر أربع فتحات صغيرة، ويحرص الجرّاح على تحديد التشريح بوضوح قبل قطع أي بنية لتجنّب إصابة القناة الصفراوية. قد يُصوَّر مجرى الصفراء أثناء العملية عند الشك بوجود حصوة داخله، وقد يُحتاج إلى منظار داخلي للقناة الصفراوية قبل الجراحة أو بعدها. التحويل إلى شقّ مفتوح احتمال قائم في الحالات الملتهبة بشدّة، وهو قرار سلامة.

حدود الإجراء: استئصال المرارة يُزيل مصدر الحصوات، ولا يعالج القولون العصبي، ولا حرقة المعدة والجَزْر المعدي المريئي، ولا فتق الحجاب الحاجز، ولا التهاب المعدة. فإذا كانت شكواك انتفاخاً وحرقة وغازات أكثر من مغص صفراوي واضح، فقد لا تتحسّن بعد العملية، ويجب البحث عن السبب الحقيقي أولاً. ولا يوجد دواء أو عشبة تُذيب الحصوات بشكل موثوق، ولا يُعتمد على تفتيتها بالموجات كما في الكلى.

تصحيح خرافة مهمة: ما يُسمّى «تنظيف المرارة» بمزيج زيت الزيتون والليمون لا يُخرج حصوات، وما يظهر في البراز ليس حصى، وقد يدفع هذا المزيج حصوة حقيقية إلى القناة الصفراوية فيسبّب انسداداً أو التهاب بنكرياس. الحياة بعد إزالة المرارة طبيعية، وقد يحتاج بعض الناس أسابيع إلى شهور لتنظيم الدهون في الطعام، وقد يحدث إسهال مؤقت.
A cholecystectomy is an operation to remove the gallbladder, a small sac beneath the liver that stores bile and empties it into the intestine after a fatty meal. The gallbladder is removed when it becomes a source of repeated symptoms or complications, most often because of gallstones and inflammation.

The typical symptom is pain that starts in the upper right abdomen or below the breastbone after a rich meal, steady and pressing, usually lasting from tens of minutes to a few hours, and often radiating to the back or the right shoulder with nausea and vomiting. Repeated attacks, acute cholecystitis, a stone that has migrated into the bile duct causing jaundice and dark urine, or gallstone pancreatitis are all strong indications for surgery. Silent stones with no symptoms at all do not automatically mean an operation; suitability is decided by clinical examination and imaging, not by reading about it.

Most operations are performed laparoscopically through four small incisions, and the surgeon takes care to define the anatomy clearly before dividing any structure, so as to avoid injury to the bile duct. The bile duct may be imaged during surgery when a stone inside it is suspected, and an endoscopic procedure on the duct may be needed before or after the operation. Conversion to an open incision remains possible in severely inflamed cases and is a safety decision.

Limits of the procedure: removing the gallbladder removes the source of stones. It does not treat irritable bowel syndrome, heartburn and acid reflux, hiatal hernia or gastritis. If your main complaints are bloating, burning and wind rather than clear biliary colic, symptoms may persist after surgery, and the true cause should be sought first. No medicine or herb reliably dissolves gallstones, and shock-wave fragmentation is not used the way it is for kidney stones.

An important myth to correct: the so-called gallbladder flush with olive oil and lemon juice does not expel stones; what appears in the stool is not gallstone material, and the mixture can push a real stone into the bile duct, causing obstruction or pancreatitis. Life after gallbladder removal is normal, although some people need weeks to months to settle their fat intake, and temporary loose stools can occur.

إصلاح الفتق الإربي عملية جراحية تُعاد فيها محتويات البطن البارزة إلى مكانها، وتُقوّى نقطة الضعف في جدار البطن السفلي عند منطقة أصل الفخذ. والفتق الإربي نفسه هو بروز نسيج دهني أو حلقة من الأمعاء عبر هذه النقطة، فيظهر انتفاخ في المنطقة يزيد بالوقوف والسعال والحمل الثقيل ويقلّ أو يختفي عند الاستلقاء.

الأعراض المعتادة انتفاخ مع شدّ أو ثقل أو ألم خفيف يزداد آخر النهار، وقد يمتدّ الإحساس إلى الخصية عند الرجال. عوامل مثل السعال المزمن، الإجهاد عند التبوّل أو التبرّز، حمل الأوزان، والعمل الواقف الطويل تزيد الأعراض. أما الانتفاخ الصلب المؤلم الذي لا يعود إلى الداخل ويصحبه غثيان وتقيّؤ وتوقّف خروج الريح، فهو حالة طارئة تعني احتمال احتباس محتويات الفتق أو خنقها وتحتاج طوارئ فوراً.

الإصلاح يُجرى بطريقتين رئيسيتين: إصلاح مفتوح بشقّ في المنطقة الإربية، أو إصلاح بالمنظار عبر فتحات صغيرة. في معظم حالات الكبار تُستخدم شبكة صناعية لتدعيم الجدار وتوزيع الشدّ، وقد يُفضّل الإصلاح بالخياطة وحدها في حالات مختارة مثل الفتق الصغير جداً أو الأطفال. اختيار الطريقة ليس مسألة تفضيل عام، بل يعتمد على حجم الفتق وكونه ثنائياً أو ناكساً، وعمل المريض ووزنه وأمراضه، والترشيح يُحدَّد بالفحص السريري.

حدود الإجراء: الفتق لا يُشفى بالتمارين ولا بشدّ عضلات البطن ولا بالمساج ولا بالأعشاب، والحزام الطبي يخفّف الإحساس مؤقتاً فقط ولا يغلق الفتحة أبداً، وقد يعطي طمأنينة زائفة تؤخّر العلاج. ولا يوجد «إصلاح فتق بالليزر»: الليزر لا يبني جدار البطن، وأي طريقة تُسمّى بذلك تعني في الواقع إصلاحاً جراحياً بأدوات مختلفة.

كما أن العملية لا تعالج آلام أسفل الظهر ولا الشدّ العضلي الرياضي ولا مشاكل الخصية، ولا تمنع ظهور فتق جديد في موضع آخر أو رجوع الفتق نفسه، خصوصاً مع استمرار السعال المزمن أو زيادة الوزن أو رفع الأثقال بعد العملية بلا تدرّج. التنميل أو الألم المزمن في المنطقة احتمال معروف بعد أي إصلاح ويجب مناقشته مع الجرّاح قبل القرار.
Inguinal hernia repair is an operation that returns the protruding abdominal contents to their place and reinforces the weak point in the lower abdominal wall at the groin. The hernia itself is a bulge of fatty tissue or a loop of bowel through that weak point, appearing as a swelling in the groin that increases on standing, coughing or lifting and reduces or disappears when lying down.

Usual symptoms are a bulge with dragging, heaviness or mild ache that worsens towards the end of the day, and in men the sensation may extend into the scrotum. Chronic cough, straining to pass urine or stool, lifting weights and long hours of standing all make symptoms worse. A firm, painful bulge that will not go back in, accompanied by nausea, vomiting and no passage of gas, is an emergency: it may mean the hernia contents are trapped or strangulated, and it needs emergency care immediately.

Repair is carried out in two main ways: open repair through a groin incision, or laparoscopic repair through small ports. In most adults a synthetic mesh is used to support the wall and spread tension, while suture-only repair may be preferred in selected situations such as a very small defect or in children. The choice is not a matter of general preference; it depends on the size of the hernia, whether it is bilateral or recurrent, and on the patient's occupation, weight and medical conditions. Suitability is determined by clinical examination.

Limits of the procedure: a hernia is not cured by exercise, abdominal muscle work, massage or herbal remedies. A truss or support belt only eases the sensation temporarily and never closes the defect; it can also give false reassurance that delays treatment. There is no such thing as laser hernia repair: a laser does not rebuild the abdominal wall, and anything marketed under that name is in reality surgical repair with different instruments.

The operation also does not treat low back pain, sports-related muscle strain or testicular problems, and it cannot guarantee that no new hernia will appear elsewhere or that this one will never recur, particularly if a chronic cough continues, weight increases or heavy lifting resumes too soon. Numbness or persistent groin discomfort is a recognised possibility after any repair and should be discussed with your surgeon before deciding.

إصلاح الفتق السري عملية تُعاد فيها محتويات الفتق البارزة عند السرّة إلى داخل البطن، وتُغلق فتحة الضعف في جدار البطن وتُقوّى. والفتق السري نفسه هو بروز نسيج دهني أو غشاء أو حلقة من الأمعاء عبر نقطة ضعف طبيعية في مكان مرور الحبل السري، فيظهر انتفاخ حول السرّة أو يبرز شكل السرّة إلى الخارج، ويزيد مع الوقوف والسعال والشدّ وينقص عند الاستلقاء.

عند الكبار ترتبط الحالة بزيادة الوزن، وتكرار الحمل، والسعال المزمن، وحمل الأوزان، وتجمّع السوائل في البطن، والعمليات السابقة. الأعراض المعتادة انتفاخ مع شدّ أو ألم خفيف يزداد آخر اليوم أو بعد المجهود، وقد يبدو الجلد فوق الفتق مشدوداً. أما الانتفاخ الصلب المؤلم الذي لا يعود إلى الداخل مع غثيان أو تقيّؤ أو توقّف خروج الريح فهو حالة طارئة تعني احتمال احتباس أو خنق محتويات الفتق.

عند الرُّضّع والأطفال الصغار الوضع مختلف: معظم الفتوق السرّية الصغيرة تُغلق تلقائياً مع نمو الطفل خلال السنوات الأولى، ولذلك يكون التوجّه الأول عادةً المراقبة لا الجراحة، وتُطرح العملية عند بقاء الفتق بعد سنّ معيّنة أو كبر حجمه أو حدوث احتباس. والقرار في كل الأعمار يُحدَّد بالفحص السريري لا بالقراءة.

طريقة الإصلاح تعتمد على حجم الفتحة وحالة الأنسجة: الفتحات الصغيرة قد تُغلق بخياطة الطبقات، والفتحات الأكبر أو المتكرّرة تُدعَّم بشبكة صناعية، وقد يُجرى الإصلاح بشقّ صغير حول السرّة أو بالمنظار. لا توجد طريقة واحدة تناسب الجميع.

حدود الإجراء: إصلاح الفتق السري يغلق الفتحة ويعالج البروز، ولا يشدّ ترهّل جلد البطن، ولا يزيل الدهون، ولا يُنقص الوزن، ولا يعالج بالضرورة تباعد عضلات البطن الطولية الذي يشيع بعد الحمل، ولا يجعل شكل السرّة كما كان تماماً. ومن الخرافات الضارّة ربط قطعة نقود أو شدّ رباط على سرّة الطفل، فهي لا تُغلق الفتق وتسبّب تسلّخ الجلد والتهابه. كما أن استمرار زيادة الوزن أو السعال المزمن أو الرجوع السريع لرفع الأثقال يرفع احتمال عودة الفتق بعد أي إصلاح.
Umbilical hernia repair is an operation that returns the protruding contents at the navel into the abdomen, then closes and reinforces the defect in the abdominal wall. The hernia itself is a protrusion of fatty tissue, lining or a loop of bowel through the natural weak point where the umbilical cord once passed, producing a swelling around the navel or an outward-pointing belly button that increases with standing, coughing and straining and reduces when lying down.

In adults the condition is linked to excess weight, repeated pregnancies, chronic cough, lifting, fluid accumulation in the abdomen and previous operations. Usual symptoms are a bulge with dragging or mild pain that worsens at the end of the day or after exertion, and the skin over the bulge may look stretched. A firm painful bulge that will not reduce, with nausea, vomiting or no passage of gas, is an emergency, because the contents may be trapped or strangulated.

In infants and small children the picture is different: most small umbilical hernias close on their own as the child grows during the first years, so the usual first approach is observation rather than surgery. An operation is considered if the hernia persists beyond a certain age, becomes large, or a trapping episode occurs. At every age the decision is made by clinical examination, not by reading about it.

The repair technique depends on the size of the defect and the quality of the tissues: small defects may be closed by suturing the layers, while larger or recurrent defects are reinforced with a synthetic mesh, and the repair may be done through a small incision around the navel or laparoscopically. No single technique suits everyone.

Limits of the procedure: umbilical hernia repair closes the defect and treats the protrusion. It does not tighten loose abdominal skin, does not remove fat, does not reduce weight, does not necessarily treat the separation of the vertical abdominal muscles that is common after pregnancy, and does not restore the navel to exactly its former shape. A harmful myth is taping a coin or tying a band over a baby's navel: this does not close the hernia and it causes skin breakdown and infection. Continuing weight gain, an untreated chronic cough or an early return to heavy lifting all increase the chance of recurrence after any repair.

انسداد الأمعاء توقّف مرور محتوى الأمعاء من الطعام والسوائل والغازات إلى الأسفل، ويحدث لسبب ميكانيكي يسدّ المجرى، أو لتوقّف حركة الأمعاء نفسها بلا سدّ مادي. وعلاج الانسداد مسار تدبير طارئ يبدأ بتقييم سريع وإراحة الأمعاء وتصحيح السوائل، ثم جراحة عند وجود مؤشّراتها.

أشهر الأسباب الميكانيكية عند الكبار: التصاقات بعد عملية بطن أو حوض سابقة، فتق محتبس، ورم في القولون، انفتال أو التواء في حلقة معوية، وانحشار براز شديد. الأعراض المعتادة مغص متقطّع في وسط البطن، انتفاخ متزايد، غثيان وتقيّؤ قد يصبح ذا رائحة كريهة، وتوقّف خروج الريح والبراز. الجفاف واختلال الأملاح يتقدّمان بسرعة، ولذلك التوجّه للطوارئ لا يُؤجَّل.

التقييم يشمل الفحص السريري وفحص أماكن الفتوق والجرح القديم، وتحاليل الدم والأملاح، وتصويراً بالأشعة أو الطبقي المحوري بصبغة لتحديد موضع الانسداد وكونه جزئياً أو كاملاً ووجود علامات نقص تروية. أول الخطوات ليست الجراحة دائماً: صيام تام، سوائل ومحاليل وريدية، أنبوب أنفي معدي لتفريغ المعدة وتخفيف التقيّؤ والانتفاخ، ومراقبة جراحية متكرّرة. كثير من حالات الالتصاقات الجزئية تتحسّن بهذا التدبير المحافظ.

تُصبح الجراحة ضرورية عند الانسداد الكامل الذي لا يستجيب، أو علامات نقص تروية أو انثقاب، أو فتق محتبس، أو انفتال، أو ورم مسبّب. والعمل الجراحي يُحدَّد حسب السبب: فكّ التصاقات، إعادة فتق وإصلاحه، فكّ الانفتال، إزالة الكتلة المسدّة، استئصال جزء متضرّر من الأمعاء ووصل الطرفين، أو عمل فتحة تحويل مؤقتة على جدار البطن إذا لم يكن الوصل آمناً في تلك اللحظة.

حدود الإجراء: علاج الانسداد يفتح المجرى الحالي ولا يمنع تكوّن التصاقات جديدة، فمن أُجريت له جراحة بطن يبقى معرّضاً لتكرار الانسداد لاحقاً. والجراحة لا تعالج سبب الورم بمفردها، بل تُدرَج ضمن خطة كاملة. تصحيح خرافة خطيرة: المليّنات والحقن الشرجية والأعشاب والزيوت الفاتحة للأمعاء خطر حقيقي عند انسداد ميكانيكي، لأنها تزيد الضغط داخل حلقة مسدودة وقد تسبّب انثقاباً؛ ولا تُعالج الحالة بالانتظار في البيت ولا بمسكّن قوي يُخفي التدهور.
Bowel obstruction means that the passage of intestinal contents, food, fluid and gas has stopped, either because something mechanically blocks the lumen or because the bowel itself stops moving without a physical block. Treating obstruction is an emergency pathway: rapid assessment, resting the bowel and correcting fluids first, then surgery when there are clear indications for it.

The commonest mechanical causes in adults are adhesions after previous abdominal or pelvic surgery, a trapped hernia, a colonic tumour, a twisted or volvulated loop of bowel, and severe faecal impaction. Usual symptoms are cramping central abdominal pain, increasing distension, nausea and vomiting that may become foul, and no passage of gas or stool. Dehydration and electrolyte disturbance progress quickly, which is why going to the emergency department should never be delayed.

Assessment includes clinical examination with careful inspection of hernia sites and old scars, blood tests and electrolytes, and plain X-rays or contrast CT to locate the obstruction, determine whether it is partial or complete, and look for signs of poor blood supply. The first steps are not always surgical: complete fasting, intravenous fluids and electrolyte replacement, a nasogastric tube to empty the stomach and relieve vomiting and distension, and repeated surgical reassessment. Many partial adhesive obstructions settle with this conservative approach.

Surgery becomes necessary for a complete obstruction that does not respond, for signs of compromised blood supply or perforation, for a trapped hernia, for a volvulus, or when a tumour is the cause. What is done depends on the cause: dividing adhesions, reducing and repairing a hernia, untwisting a volvulus, removing the obstructing mass, resecting a damaged segment of bowel and joining the ends, or bringing out a temporary stoma on the abdominal wall if a join would not be safe at that moment.

Limits of the procedure: treating an obstruction opens the current blockage; it does not prevent new adhesions from forming, so anyone who has had abdominal surgery remains at risk of obstruction again later. Surgery alone does not treat an underlying tumour, which needs a complete plan of care. A dangerous myth to correct: laxatives, enemas, herbal preparations and purging oils are a genuine hazard in mechanical obstruction, because they raise pressure inside a closed loop and can cause perforation. This condition is not managed by waiting at home, nor by a strong painkiller that masks deterioration.

الورم الشحمي كتلة حميدة تتكوّن من خلايا دهنية ناضجة تنمو ببطء تحت الجلد، وتُحسّ عادةً كعقدة طرية مطاطية تتحرّك بسهولة تحت الأصابع وبلا ألم في معظم الحالات. وهو من أكثر الكتل الجلدية شيوعاً، ويظهر أكثر في الظهر والكتفين والرقبة والذراعين والفخذين، وقد يكون منفرداً أو متعدّداً.

ليس كل ورم شحمي يحتاج علاجاً. تُطرح إزالته عندما يكون مؤلماً أو ضاغطاً على عصب، أو ينمو بسرعة، أو يكبر حجمه ويعيق الحركة أو اللباس، أو يقع في موضع يتعرّض للاحتكاك والضغط، أو يزعج المريض شكلياً، أو عندما يكون التشخيص غير مؤكّد فيلزم فحص نسيجي. غير ذلك فالمراقبة خيار مقبول تماماً.

العلاج الأساسي هو الاستئصال الجراحي الكامل مع محفظة الورم، ويُجرى في معظم الحالات بتخدير موضعي في العيادة عبر شقّ بطول الكتلة تقريباً، وتُرسل الكتلة إلى الفحص النسيجي. الأورام الكبيرة أو العميقة أو القريبة من بنى مهمّة تُجرى في غرفة العمليات، وقد تحتاج تخديراً أعمق وتصريفاً مؤقتاً.

حدود الإجراء: لا يوجد كريم ولا دواء ولا حقنة تُذيب الورم الشحمي، والحمية ونزول الوزن لا تُزيله لأن خلاياه لا تستجيب كبقية الدهون. شفط الدهون قد يقلّل الحجم بشقّ أصغر لكنه يترك المحفظة والخلايا فيزيد احتمال الرجوع في الموضع نفسه، ولا يوفّر نسيجاً كاملاً للفحص. وإزالة ورم واحد لا تمنع ظهور أورام أخرى، خصوصاً عند من لديهم أورام متعدّدة أو استعداد عائلي. وأي إزالة تترك أثراً جراحياً، ويجب مناقشة موضع الشقّ وشكله قبل الموافقة.
A lipoma is a benign lump made of mature fat cells that grows slowly under the skin. It is usually felt as a soft, rubbery nodule that slides easily under the fingers and is painless in most cases. Lipomas are among the most common soft-tissue lumps, appearing most often on the back, shoulders, neck, arms and thighs, either singly or as multiple lumps.

Not every lipoma needs treatment. Removal is considered when the lump is painful or presses on a nerve, when it grows quickly, when its size restricts movement or clothing, when it sits where it is rubbed or pressed, when it bothers the patient cosmetically, or when the diagnosis is uncertain and tissue examination is required. Otherwise, observation is a perfectly acceptable option.

Assessment begins with clinical examination: size, consistency, mobility and the relationship of the lump to skin and muscle. Ultrasound may be requested for unclear lumps, and more detailed imaging for large or deep lumps inside muscle or close to important vessels and nerves before any removal. Features that call for deeper assessment before a simple excision is planned include a hard lump, one fixed and not mobile, rapid growth, pain, an unusually large size, or a lump lying deep beneath the muscle layer, because other soft-tissue masses can look like a lipoma while being entirely different in nature; imaging and histology are what separate them.

The mainstay of treatment is complete surgical excision together with the capsule. In most cases this is done under local anaesthesia in the clinic through an incision roughly matching the length of the lump, and the specimen is sent for histological examination. Large or deep lipomas, or those near important structures, are removed in the operating room and may need deeper anaesthesia and a temporary drain.

Limits of the procedure: no cream, tablet or injection dissolves a lipoma, and dieting or weight loss does not remove it, because its cells do not respond like ordinary body fat. Liposuction can reduce the bulk through a smaller incision, but it leaves the capsule and cells behind, so recurrence in the same site is more likely, and it does not provide a complete specimen for examination. Removing one lipoma does not prevent others from appearing, particularly in people with multiple lipomas or a family tendency. Any removal leaves a surgical scar, and the position and shape of the incision should be discussed before you consent.

علاج جروح القدم رعاية متخصّصة للجروح والتقرّحات في القدم التي لا تُشفى في المدّة الطبيعية المتوقّعة، وأكثرها شيوعاً قرحة القدم عند مريض السكري، ثم الجروح الناتجة عن ضعف التروية الشرياني، وقرح الضغط عند قليل الحركة، والجروح بعد الرضّ أو الاحتكاك بحذاء غير مناسب. الهدف الأساسي ليس تغطية الجرح فحسب، بل منع العدوى العميقة والحفاظ على القدم ووظيفتها.

الخطر الأكبر أن كثيراً من هذه الجروح لا تؤلم. اعتلال الأعصاب عند مريض السكري يُفقد الإحساس بالوخز والضغط والحرارة، فتتكوّن القرحة وتتوسّع بلا ألم واضح، ويتأخّر المريض في المراجعة حتى تصل العدوى إلى الأنسجة العميقة أو العظم. لذلك غياب الألم لا يعني أن الجرح بسيط، والفحص الدوري للقدم عند مريض السكري إجراء وقائي أساسي.

التقييم يشمل قياس الجرح وعمقه، وهل يوجد نسيج ميت أو نفق أو عظم مكشوف، وفحص نبض القدم وعلامات التروية، وتقييم الإحساس، ومراجعة ضبط السكري، وقد يلزم زرع من عمق الجرح وتصوير للعظم أو تقييم للأوعية الدموية. هذا التقييم هو ما يحدّد خطة العلاج، والترشيح لأي إجراء يُحدَّد بالفحص السريري لا بالقراءة.

Foot wound care is specialised management of wounds and ulcers on the foot that fail to heal within the expected time. The commonest is the diabetic foot ulcer, followed by wounds caused by poor arterial supply, pressure ulcers in people with limited mobility, and wounds after injury or friction from ill-fitting footwear. The aim is not simply to cover the wound, but to prevent deep infection and preserve the foot and its function.

The greatest danger is that many of these wounds do not hurt. Nerve damage in diabetes removes the sensation of pricking, pressure and heat, so an ulcer forms and enlarges without obvious pain and the patient presents late, once infection has reached deep tissue or bone. Absence of pain therefore does not mean the wound is minor, and regular foot checks for people with diabetes are a basic preventive measure.

Assessment covers the size and depth of the wound, whether there is dead tissue, tunnelling or exposed bone, the foot pulses and signs of perfusion, sensation testing, and a review of diabetes control. A deep tissue culture, bone imaging or vascular assessment may be needed. This assessment determines the treatment plan, and suitability for any procedure is decided by clinical examination, not by reading about it.

Treatment is a set of elements that work together: cleaning the wound and removing dead tissue in stages, choosing a dressing that matches the wound bed and the amount of exudate, offloading pressure from the ulcer site with special footwear, a cast or crutches, treating infection when present, improving blood supply with vascular surgery input if pulses are weak, and optimising blood sugar, nutrition and smoking cessation. Deep or infected wounds may require debridement in the operating room, negative-pressure therapy or skin grafting.

Limits of the procedure: no advanced dressing, cream or ointment will heal a wound on a foot with poor blood supply or under continuing pressure. Without offloading and improved perfusion the wound stays open, however expensive the dressing. Antibiotics treat infection; they do not close a wound and should not be used without indication. Healing is also not permanent safety: going back to the old shoes or walking barefoot brings the ulcer back in the same place. Several harmful and widespread myths also need correcting: do not use a blade, scissors, a sharp file or caustic corn plasters to remove dead skin and callus at home, do not soak the foot in hot water, do not apply herbs, toothpaste, ash or coloured preparations to the wound, and do not wrap the foot in a tight bandage yourself. These are repeated causes of deep infection and tissue loss, and coloured preparations hide important changes from the doctor's eye.

فتق الحجاب الحاجز صعود جزء من أعضاء البطن إلى تجويف الصدر عبر فتحة أو نقطة ضعف في الحجاب الحاجز، وهو العضلة التي تفصل الصدر عن البطن. وأشهر أنواعه عند الكبار هو الفتق عند فتحة المريء في الحجاب الحاجز، حيث يصعد أعلى المعدة إلى الصدر فيضعف الحاجز الطبيعي الذي يمنع رجوع الحمض. وهناك نوع خِلقي يظهر عند حديثي الولادة ويُدار كحالة طارئة في جراحة الأطفال، ونوع يحدث بعد رضّ قويّ على الصدر أو البطن.

الأعراض المعتادة عند الكبار حرقة وارتجاع حمضي، وصعود طعام أو سائل حامض إلى الحلق، وضغط أو ألم أعلى البطن أو خلف عظمة القصّ، وشبع سريع، وتجشّؤ متكرّر، وأحياناً سعال ليلي أو بحّة أو ألم عند البلع. الفتق الصغير قد يكون بلا أعراض ويُكتشف عرضاً في منظار أو تصوير. أما الفتق الكبير فقد يسبّب صعوبة بلع أو أنيميا بسبب نزف بسيط متكرّر أو ضيق نفس بعد الأكل.

التشخيص يعتمد على المنظار العلوي، وتصوير الجهاز الهضمي العلوي بالصبغة، وأحياناً قياس حركة المريء وحموضته قبل الجراحة. والقرار في العلاج يُحدَّد بالفحص السريري ونتائج هذه الفحوص لا بالأعراض وحدها ولا بالقراءة.

العلاج يبدأ غير جراحي في معظم الحالات: تعديل نمط الأكل بوجبات أصغر وتجنّب الاستلقاء بعد الطعام، تخفيف الوزن، رفع رأس السرير، الإقلاع عن التدخين، مع أدوية تخفيف الحموضة بوصف الطبيب ومتابعته. تُطرح الجراحة عند فشل هذا العلاج أو الاعتماد الدائم عليه بجرعات متزايدة، أو عند فتق كبير أو منزلق مع أعراض ضاغطة أو أنيميا أو خطر انحشار، أو عند مضاعفات في المريء. والجراحة تُجرى غالباً بالمنظار: تُعاد المعدة إلى البطن، ويُزال كيس الفتق، وتُضيَّق فتحة الحجاب الحاجز بخياطة وقد تُدعَّم عند اللزوم، ثم يُصنع صمّام من قمة المعدة يُلفّ حول أسفل المريء لمنع الجَزْر. الهدف استعادة الحاجز الميكانيكي بلا تضييق البلع.

A diaphragmatic hernia is the movement of part of the abdominal contents into the chest through an opening or weak point in the diaphragm, the muscle that separates chest from abdomen. The commonest form in adults is a hernia at the oesophageal opening of the diaphragm, where the upper stomach rises into the chest and weakens the natural barrier that prevents acid reflux. There is also a congenital form that presents in newborns and is managed as an emergency by paediatric surgery, and a form that follows severe chest or abdominal trauma.

Usual symptoms in adults are heartburn and acid reflux, food or sour fluid rising into the throat, pressure or pain in the upper abdomen or behind the breastbone, early fullness, frequent belching, and sometimes night-time cough, hoarseness or pain on swallowing. A small hernia may cause no symptoms and be found incidentally at endoscopy or on imaging. A large hernia can cause difficulty swallowing, anaemia from repeated minor bleeding, or breathlessness after meals.

Diagnosis relies on upper endoscopy, contrast imaging of the upper gastrointestinal tract, and sometimes oesophageal motility and acid studies before surgery. Treatment decisions are based on clinical assessment together with these results, not on symptoms alone and certainly not on reading about the condition.

Treatment starts non-surgically in most cases: smaller meals, avoiding lying down after eating, weight reduction, raising the head of the bed, stopping smoking, and acid-reducing medication prescribed and monitored by a doctor. Surgery is considered when this fails or requires permanent escalating treatment, when the hernia is large or sliding with pressure symptoms, anaemia or a risk of becoming trapped, or when complications develop in the oesophagus. Surgery is usually laparoscopic: the stomach is returned to the abdomen, the hernial sac is excised, the diaphragmatic opening is narrowed with sutures and reinforced if required, and a valve is fashioned from the top of the stomach and wrapped around the lower oesophagus to prevent reflux. The aim is to restore the mechanical barrier without narrowing swallowing.

Limits of the procedure: surgery corrects a mechanical defect. It does not cure oesophageal hypersensitivity, irritable bowel syndrome or chest pain of cardiac origin, which is why cardiac causes must be excluded before chest pain is attributed to the hernia. It is not a weight-loss operation and does not reduce stomach size for that purpose. Recurrence of the hernia or of symptoms is possible, particularly with weight gain, chronic cough or heavy lifting. Afterwards you may have temporary difficulty swallowing, bloating and an inability to belch or vomit; some patients need months to settle their eating, and a proportion continue on acid-reducing medication.

كيس الشعر، ويُسمّى أيضاً الجيب الشعري أو الناسور العصعصي، نفق أو تجويف صغير يتكوّن تحت الجلد في أعلى الشقّ بين الأردافين قرب العصعص. يبدأ عندما تنغرز شعرات مقطوعة في الجلد فتُحدث تفاعلاً التهابياً يبني تجويفاً يحتوي شعراً وبقايا جلدية، وقد يفتح فوهة أو أكثر على سطح الجلد تُفرز سائلاً.

الصورة الشائعة إمّا مزمنة: فوهة أو فوهات صغيرة تُفرز سائلاً أو دماً أو مادة كريهة الرائحة مع ألم عند الجلوس، أو حادّة: خرّاج مؤلم متورّم ساخن يجعل الجلوس والمشي صعباً ويحتاج تفريغاً عاجلاً. الحالة أكثر شيوعاً عند الشباب، وتزيد مع الجلوس الطويل، والشعر الكثيف الخشن في المنطقة، والتعرّق، والاحتكاك، وزيادة الوزن، وضعف العناية بالنظافة والتجفيف.

التشخيص سريري بالفحص المباشر لتحديد الفوهات وامتداد النفق وقربه من فتحة الشرج، وهذا مهمّ للتفريق بين كيس الشعر والناسور الشرجي لأن علاجهما مختلف تماماً. قد يُطلب سونار أو تصوير للامتدادات المعقّدة أو المتكرّرة، والترشيح لأي طريقة علاج يُحدَّد بالفحص السريري لا بالقراءة.

العلاج يعتمد على الحالة. في الخرّاج الحاد تكون الخطوة الأولى تفريغه لتخفيف الألم والعدوى، وهي خطوة إسعافية لا تُعدّ علاجاً نهائياً لأن النفق يبقى موجوداً. بعد هدوء الالتهاب يُطرح العلاج النهائي، وله عدّة مسارات: استئصال النفق مع إغلاق الجرح بطريقة تُبعد خطّ الخياطة عن منتصف الشقّ، أو الاستئصال وترك الجرح ليشفى بالتحبيب مع عناية يومية، أو تقنيات أقلّ توغّلاً مثل تنظير الجيب وتنظيفه من الداخل، أو سدّ المسار. الاختيار يعتمد على عدد الفوهات وامتداد النفق ووجود التهاب حاد وعدد المرّات التي عُولجت فيها الحالة سابقاً وطبيعة عمل المريض.

حدود الإجراء: أي عملية تُزيل النفق الموجود ولا تُلغي السبب — فإذا استمرّت العوامل نفسها فقد يتكوّن جيب جديد. لذلك إزالة الشعر المنتظمة عن المنطقة والنظافة والتجفيف الجيد وتقليل الجلوس المتواصل جزء من العلاج لا رفاهية إضافية. والمضاد الحيوي وحده لا يشفي الجيب ولا يُغلق النفق، وإن أخفى الأعراض مؤقتاً. وفتح الخرّاج في البيت أو الضغط عليه أو وضع مواد كاوية يوسّع العدوى ويعقّد العلاج النهائي. ونقطة ينبغي معرفتها قبل الموافقة: الجرح في هذه المنطقة يشفى ببطء بطبيعته لأنها معرّضة للحركة والضغط والرطوبة، وقد يحتاج أسابيع من العناية اليومية والالتزام بمواعيد المتابعة، والعودة إلى العمل تعتمد على نوعه ومقدار الجلوس فيه.
A pilonidal sinus is a small tunnel or cavity that forms under the skin at the top of the cleft between the buttocks, near the tailbone. It begins when broken hairs are driven into the skin and provoke an inflammatory reaction that builds a cavity containing hair and skin debris, often opening at one or more pits on the skin surface that discharge fluid.

The common presentation is either chronic, with one or more small pits discharging fluid, blood or foul-smelling material and pain on sitting, or acute, with a painful, swollen, hot abscess that makes sitting and walking difficult and needs urgent drainage. The condition is most frequent in young adults, and prolonged sitting, thick coarse hair in the area, sweating, friction, excess weight and inadequate cleaning and drying all increase the risk.

Diagnosis is clinical, by direct examination to map the pits and the extent of the tunnel and its distance from the anal opening. That distinction matters, because a pilonidal sinus and an anal fistula are treated in entirely different ways. Ultrasound or imaging may be requested for complex or recurrent extensions, and suitability for any technique is determined by clinical examination, not by reading about it.

Treatment depends on the situation. For an acute abscess the first step is drainage to relieve pain and infection; that is an emergency measure and not a definitive cure, because the tunnel remains. Once inflammation settles, definitive treatment is discussed and several paths exist: excision of the tract with closure designed to move the suture line away from the midline of the cleft, excision left open to heal by granulation with daily care, or less invasive techniques such as endoscopic inspection and cleaning of the sinus, or obliteration of the tract. The choice depends on the number of pits, the extent of the tunnel, the presence of acute inflammation, how many times the problem has been treated before, and the nature of the patient's work.

Limits of the procedure: any operation removes the existing tract but does not remove the cause, so if the same factors continue a new sinus can form. Regular hair removal from the area, cleanliness, thorough drying and breaking up long periods of sitting are therefore part of the treatment, not optional extras. Antibiotics alone do not heal the sinus or close the tract, even if they mask symptoms for a while. Opening an abscess at home, squeezing it, or applying caustic substances spreads infection and complicates definitive treatment. One point to understand before consenting: wounds in this area heal slowly by nature, because it is exposed to movement, pressure and moisture. Healing may require weeks of daily care and strict attendance at follow-up, and return to work depends on the type of job and how much sitting it involves.
