إبرة الظهر العلاجية حقنة تُوضع بدقّة في حيّز محدّد داخل العمود الفقري أو حوله — كالحيّز فوق الجافية أو محيط جذر عصبي — وتحتوي دواءً مضاداً للالتهاب من مجموعة الستيرويدات مع مخدّر موضعي، وتُجرى بتوجيه الأشعة أو الأمواج فوق الصوتية لضمان وصول الدواء إلى المكان المقصود بأقلّ كمية ممكنة.

الهدف منها تهدئة الالتهاب حول العصب المتهيّج، وهو الالتهاب الذي يصنع الألم الحارق المنتشر في الساق أو الذراع مع التنميل، كما يحدث في الانزلاق الغضروفي وتضيّق القناة الفقرية. عندما ينجح الحقن يتراجع الألم بما يكفي لأن يستأنف المريض الحركة والعلاج الطبيعي، وهذا هو دوره الحقيقي.

ولهذا يجب أن يكون واضحاً ما لا تفعله الإبرة: هي لا تُزيل الانزلاق الغضروفي ولا تُرجع الديسك إلى مكانه، ولا تُوسّع قناة ضيّقة، ولا تُصلح عدم ثبات فقري، ولا تعالج السبب البنيوي وحده. هي تفتح نافذة لتخفيف الألم تُمكِّن التأهيل ولا تُغني عنه؛ فمن يكتفي بالحقنة ولا يعمل على تقوية عضلات الجذع وتصحيح أنماط الحمل والحركة وإدارة الوزن، تعود شكواه في الغالب. وأثرها مؤقّت بطبعه ويتفاوت كثيراً بين مريض وآخر، وتكرارها محدود ولا يُعاد عندما لا تعطي فائدة واضحة.

هناك حالات لا محلّ فيها للحقنة أصلاً بل تستدعي تقييماً جراحياً عاجلاً: ضعف عضلي متزايد في الطرف، أو تخدير منطقة السرج، أو فقدان التحكّم بالبول أو البراز. كما تُؤجَّل عند وجود عدوى نشطة أو مشاكل تخثّر أو حين تكون مسيّلات الدم مستمرة.

A therapeutic spinal injection places medication precisely into a defined space in or around the spine, such as the epidural space or the area surrounding a nerve root. It contains an anti-inflammatory medicine of the corticosteroid class together with a local anaesthetic, and it is delivered under X-ray or ultrasound guidance so the drug reaches the intended target with the smallest necessary volume.

Its purpose is to calm the inflammation around an irritated nerve, the very inflammation that produces the burning pain radiating into the leg or arm along with numbness, as happens in disc herniation and spinal canal stenosis. When the injection works, pain settles enough for the patient to move again and engage properly with physiotherapy, and that is its genuine role.

For this reason it must be clear what the injection does not do. It does not remove a herniated disc or push it back into place, it does not widen a narrowed canal, it does not repair spinal instability, and it does not treat the underlying structural problem on its own. It opens a window of pain relief that enables rehabilitation rather than replacing it. Patients who rely on the injection alone, without strengthening the trunk muscles, correcting loading and movement patterns and managing weight, usually see symptoms return. Its effect is inherently temporary and varies widely between individuals, repetition is limited, and it is not repeated when it has produced no clear benefit.

Some situations have no place for an injection at all and call for urgent surgical assessment instead: progressive muscle weakness in a limb, numbness in the saddle area, or loss of bladder or bowel control. Injections are also postponed in active infection, clotting problems, or when blood thinners cannot be safely managed around the procedure.

Candidacy is decided by clinical examination and by matching symptoms to the level of pathology seen on imaging, because not every episode of back pain is a candidate.

الحقن بين الفقرات إجراء تداخلي يُوضع فيه مخدّر موضعي، وقد يُضاف إليه مضاد التهاب من مجموعة الستيرويدات، داخل المفاصل الصغيرة الواقعة بين الفقرات — وتُسمّى المفاصل الوجيهية — أو حول الأعصاب الصغيرة التي تُغذّيها والمعروفة بالفروع الإنسية. يُنجز بتوجيه الأشعة أو الأمواج فوق الصوتية، ويُستخدم للتشخيص وتخفيف الألم معاً.

هذه المفاصل مصدر شائع لألم الظهر أو الرقبة الميكانيكي: ألم موضعي يزداد بالوقوف الطويل ومدّ الظهر للخلف واللفّ، وينتشر إلى الأرداف أو الفخذ من الخلف دون أن يهبط إلى القدم، ويخفّ عند الانحناء للأمام أو الجلوس. ولا توجد صورة تُثبت وحدها أن المفصل هو المصدر، فتغيّرات الخشونة في المفاصل الوجيهية شائعة عند من لا يشكون من شيء، ولهذا صار للحقن دور تشخيصي مهم: إن خفّ الألم مؤقتاً بعد حقن مخدّر موجَّه، رجّح ذلك أن المصدر هو هذا المفصل، وفتح الباب لخيار كيّ الفرع الإنسي بالتردد الحراري لأثر أطول.

حدود الإجراء يجب أن تُقال بوضوح: هو لا يعالج الانزلاق الغضروفي ولا الألم الجذري الهابط إلى القدم، ولا يُوسّع قناة فقرية ضيّقة، ولا يُصلح انزلاقاً فقرياً أو عدم ثبات، ولا يُزيل خشونة موجودة في المفصل. وأثره في الغالب مؤقّت ويتفاوت، ولا يُعاد بلا فائدة موثّقة. الأهم أنه يُمكِّن التأهيل ولا يُغني عنه: تقوية عضلات الجذع، تصحيح الوضعيات وأنماط الحمل، تحسين مرونة الوركين، وإدارة الوزن — هذه ما يحمي المفصل على المدى الطويل، والحقنة نافذة زمنية للبدء بها لا بديلاً عنها.

A facet joint injection is an interventional procedure that places local anaesthetic, sometimes with an anti-inflammatory medicine of the corticosteroid class, either inside the small joints between the vertebrae, known as facet joints, or around the tiny nerves that supply them, known as medial branches. It is carried out under X-ray or ultrasound guidance and serves both diagnostic and pain-relieving purposes.

These joints are a common source of mechanical back or neck pain: localised pain that worsens on prolonged standing, arching backwards and twisting, that may spread to the buttock or back of the thigh without travelling down to the foot, and that eases on bending forward or sitting. No scan can prove on its own that the joint is the source, because degenerative facet changes are common in people with no symptoms at all. This is why the injection has an important diagnostic role: temporary relief after a targeted anaesthetic injection makes that joint the likely source and opens the option of radiofrequency treatment of the medial branch for longer-lasting effect.

The limits must be stated clearly. It does not treat a herniated disc or radicular pain running down to the foot, it does not widen a narrowed spinal canal, it does not repair spondylolisthesis or instability, and it does not remove existing arthritis in the joint. Its effect is usually temporary and variable, and it should not be repeated without documented benefit. Most importantly, it enables rehabilitation rather than substituting for it: trunk strengthening, correction of posture and loading patterns, improved hip mobility and weight management are what protect the joint long term, and the injection is a window in which to begin them.

Candidacy is decided by clinical examination and by matching the pain pattern to the suspected level, while excluding conditions that need urgent assessment such as progressive weakness or serious nerve compression symptoms.

علاج الديسك بالتردد الحراري إجراء تداخلي غير جراحي تُمرَّر فيه إبرة خاصة بتوجيه الأشعة إلى نقطة محدّدة قرب مصدر الألم، ثم يُطلَق عبر طرفها تيار عالي الترددّ يُنتج حرارة مضبوطة تُعطّل نقل الإشارة الأليمة في أعصاب حسّية صغيرة، أو تُحدث تغيّراً محدوداً في نسيج الحلقة الليفية للديسك.

يُستخدم بصورتين مختلفتين في الغرض: التردد الحراري المستمر الذي يرفع الحرارة إلى درجة تُقاطع نقل الإشارة، ويُستخدم كثيراً لأعصاب المفاصل الوجيهية (الفروع الإنسية) بعد أن يُثبت حقن تشخيصي أن المفصل هو المصدر؛ والتردد النابض الذي يُطلق الطاقة على دفعات بحرارة أقل، ويُستخدم بحذر قرب الأعصاب الجذرية حيث لا يُراد إتلافها. وحين يُطبَّق داخل الديسك نفسه فالهدف هو الحلقة الليفية والنهايات العصبية فيها، وليس إزالة النتوء الضاغط.

حدود الإجراء يجب أن تكون صريحة تماماً: التردد الحراري لا يُعيد الديسك المنفتق إلى مكانه ولا يُزيل الجزء الضاغط على العصب، ولا يُوسّع قناة فقرية ضيّقة، ولا يُصلح عدم ثبات فقري، ولا يُنمّي غضروفاً. وهو ليس علاجاً لحالة فيها ضعف عضلي متزايد أو أعراض ضغط شديد على الأعصاب — تلك حالات تُقيَّم جراحياً وبعضها إسعافي. كما أن الأدلة على تطبيقه داخل الديسك أقلّ رسوخاً من الأدلة على كيّ الفروع الإنسية لألم المفاصل الوجيهية، ولهذا يكون انتقاء المريض هو العامل الأهم في النتيجة.

الأثر ليس دائماً: عند كيّ الفرع الإنسي تعود الأعصاب الصغيرة إلى التعصيب مع الوقت فيرجع الألم عادة بعد فترة تتراوح من عدة أشهر إلى ما يقارب السنة، وقد يُعاد الإجراء. وفوق ذلك كله هو يُمكِّن التأهيل ولا يُغني عنه: بلا تقوية الجذع وتصحيح الحمل وإدارة الوزن يبقى السبب البنيوي قائماً.

Thermal radiofrequency treatment for disc-related pain is a non-surgical interventional procedure in which a special needle is advanced under X-ray guidance to a precise target near the source of pain. High-frequency current is then delivered through its tip, generating controlled heat that interrupts pain signal transmission in small sensory nerves or produces a limited change in the tissue of the disc's outer fibrous ring.

It is used in two distinct forms. Continuous radiofrequency raises the temperature enough to interrupt signal conduction and is widely used on the nerves supplying the facet joints, the medial branches, once a diagnostic block has shown the joint to be the pain source. Pulsed radiofrequency delivers energy in bursts at lower temperatures and is used cautiously near nerve roots that must not be damaged. When energy is applied inside the disc itself, the target is the annulus and its nerve endings, not the herniated fragment.

The limits need to be entirely explicit. Radiofrequency does not push a herniated disc back into place and does not remove the fragment pressing on a nerve. It does not widen a narrowed spinal canal, it does not repair spinal instability, and it does not regenerate cartilage. It is not the treatment for progressive muscle weakness or severe nerve compression, which require surgical assessment and are sometimes emergencies. Evidence for intradiscal application is also less established than evidence for medial branch treatment of facet joint pain, which makes careful patient selection the single most important factor in the outcome.

Relief is not permanent. After medial branch treatment the small nerves regenerate over time and pain typically returns after a period ranging from several months to around a year, at which point the procedure may be repeated. Above all it enables rehabilitation rather than replacing it: without trunk strengthening, load correction and weight management the underlying structural problem remains.

Candidacy is decided by clinical examination and by the result of a prior diagnostic block, not by an imaging report alone.
