The programme exists because diabetes damages peripheral nerves and narrows small arteries, so the skin becomes both less able to feel and less able to heal at the same time.

The real danger here is not pain but its absence. We are used to pain being what drives us to the doctor, but in the diabetic foot the absence of pain is false reassurance. You may walk all day on a nail, a pebble or an open wound without feeling it, and a wound may burrow beneath a layer of hardened skin and reach bone before any symptom appears. Many ulcers begin with a tight new shoe, with hot water whose temperature was not felt, with incorrect nail cutting, or with walking barefoot indoors.

What foot care does is clear, and what it does not do must be equally clear. The core tests are fasting blood glucose, the cumulative test that reflects average glucose over recent weeks, and a tolerance test performed after drinking a measured glucose solution, with a random measurement added when clear symptoms are present. The choice of test is not arbitrary: your doctor selects it according to your age, your condition and the reason for testing.

Testing is not only for people who feel ill. The most important group is those with no complaints at all: anyone over forty, anyone with a parent or sibling who has diabetes, anyone carrying extra weight especially around the abdomen, anyone with high blood pressure or abnormal lipids, a woman who had diabetes in pregnancy or delivered a large baby, and anyone with polycystic ovary syndrome. These people need periodic testing while feeling completely healthy, because type 2 diabetes can pass years without a single symptom while quietly causing damage.

The limits of testing must be stated openly. One normal result does not mean permanent safety; it means the test is repeated on schedule, because risk changes with weight and age. The cumulative test can be misleading in anaemia, kidney disease, some inherited blood disorders and in pregnancy, so no single number is read in isolation from your circumstances.
