Meningitis prevention is a set of medical measures aimed at preventing inflammation of the membranes that surround the brain and spinal cord. It combines vaccination, preventive antibiotics for the close contacts of a confirmed case, and simple steps that reduce the spread of respiratory droplets. Bacterial meningitis — most often caused by meningococcus, pneumococcus and Haemophilus influenzae type b — can deteriorate within hours, which is why prevention is built on acting early rather than waiting for symptoms.

The conjugate meningococcal vaccine, which covers several serogroups, is recommended for travellers to areas where the disease is more common, for Hajj and Umrah pilgrims because proof of vaccination is an entry requirement set by some authorities, for students moving into shared university accommodation, and for people without a functioning spleen or with immune or complement deficiencies. It should be given well before departure — usually no less than ten days — so that immunity has time to develop and the certificate is accepted. Pneumococcal and Hib vaccines are already part of the childhood immunisation schedule and are added for eligible adults.

It matters just as much to understand what the vaccine does not do. It does not prevent every form of meningitis. The commonly used conjugate vaccine does not cover all meningococcal serogroups — serogroup B needs a different vaccine that is not given routinely to everyone — and it offers no protection against viral, tuberculous or fungal meningitis. Protection also wanes over time, and a booster may be advised for high-risk groups. Holding a vaccination certificate is never a reason to ignore symptoms.

The bacteria spread through respiratory droplets and close, prolonged contact: living in the same household, sharing cups, cutlery, cigarettes and toothbrushes, and kissing. They are not carried by water or food, and simply passing near a patient does not transmit them. Not everyone who has seen a case needs preventive antibiotics; deciding who qualifies as a close contact is a clinical and public-health decision. Carrying the organism in the throat without illness is common, which is why respiratory hygiene and not sharing utensils genuinely complement vaccination rather than replace it.

Exactly what you need — which vaccine, whether extra or booster doses apply, and whether post-exposure prophylaxis is indicated — is decided by clinical assessment together with a review of your immunisation record, your destination and your immune status, not by reading about it.

Malaria prevention is a medical plan prepared before travelling to areas where the disease circulates. Its purpose is to stop the traveller acquiring the Plasmodium parasite, which is transmitted by the bite of the female Anopheles mosquito, usually between dusk and dawn. Jordan is not a malaria-transmission country, so cases diagnosed here are imported in people returning from endemic regions of sub-Saharan Africa, South Asia, South America and parts of South-East Asia.

The plan rests on four pillars that work together: knowing that the destination is endemic, preventing bites, taking preventive medication, and getting tested early for any fever. Bite prevention is not a minor detail. It means a reliable insect repellent on exposed skin reapplied as directed, long light-coloured clothing, an insecticide-treated net over the bed, a well-sealed or air-conditioned room, and particular vigilance at night, because the mosquito that carries malaria bites after dark.

Your doctor selects preventive medication according to the destination and the local pattern of parasite resistance, the length of your stay, your age, pregnancy, chronic conditions and current medicines. Sticking to the schedule before, during and after travel is inseparable from how well the plan works, because stopping the course too soon after returning home is one of the commonest reasons prevention fails.

What prevention does not do should be equally clear. No preventive medicine blocks malaria absolutely, and medication does not replace mosquito protection or the other way round. The vaccines introduced in some endemic countries are directed at children living in those communities and are not a substitute for traveller prophylaxis. This plan also gives no protection against the other mosquito-borne infections such as dengue, Zika and chikungunya, which are spread by daytime biters and therefore require protection around the clock.

The most dangerous mistake a returning traveller makes is treating a fever at home as influenza. Any fever, chills, headache or unusual fatigue in the weeks to months after return needs urgent medical assessment, and you must state your travel history explicitly, because falciparum malaria can deteriorate quickly. What suits you specifically is determined by clinical assessment and a review of your medicines and health, not by reading.

The influenza vaccine is a seasonal immunisation given each year to train the immune system to recognise the influenza viruses expected to circulate in the coming season. It is repeated annually for two reasons: the virus changes, and the immunity the vaccine produces fades over months. The commonly used version is an inactivated vaccine injected into the upper-arm muscle; it contains no live virus capable of causing the illness.

The people who need it most are those for whom influenza is a dangerous illness rather than an inconvenience: adults over sixty-five, young children from six months of age, pregnant women at any stage of pregnancy, people with heart, lung, kidney or liver disease, diabetes, blood disorders or reduced immunity, people with severe obesity, healthcare workers, residents of care homes, and Hajj and Umrah pilgrims, because crowding raises the chance of transmission. Vaccinating the household around an infant under six months protects that baby, who is too young to be vaccinated.

It is best taken at the start of the season, in autumn, before transmission picks up, because immunity takes about two weeks to build. If you are late, having it mid-season is still better than skipping it, since the season in Jordan usually runs into late winter and early spring.

There are limits worth knowing clearly. The vaccine does not prevent the common cold, COVID-19 or the other respiratory viruses, and you can still catch influenza after having it, though the illness is usually milder with fewer complications. The belief that the vaccine gives you flu is not true: a sore arm, tiredness and a low-grade temperature are a normal immune response that settles within a day or two, not an infection. The vaccine also does not replace hand washing, covering coughs and staying home while unwell.

Vaccination is postponed during an acute illness with a high fever, and it is not given to anyone with a confirmed severe allergic reaction to a previous dose or to a vaccine component, nor to infants under six months. Egg allergy usually does not prevent vaccination but it does influence where it is given and how long you are observed. The final decision — product, timing, and whether a child being vaccinated for the first time needs two doses — is made on clinical assessment and a review of your record, not from reading.
