كسل العين — واسمه الطبي الغَمَش — ضعف في حدّة الإبصار في عين واحدة أو في كلتيهما سببه أنّ الدماغ لم يتعلّم استخدام الصورة الواردة من تلك العين خلال سنوات نموّ البصر الأولى، وليس خللاً في بنية العين نفسها في معظم الحالات. ولذلك يكون العلاج موجّهاً إلى إعادة تدريب المسار البصري في الدماغ، لا إلى تقوية العين بجهاز أو قطرة أو فيتامين.

وللكسل ثلاثة مسارات رئيسية: حَول يجعل الدماغ يُلغي صورة العين المنحرفة ليتخلّص من الرؤية المزدوجة؛ وفرق في قوّة النظر بين العينين — أو ضعف انكساري كبير في كلتيهما — يجعل صورة إحداهما دائماً أقلّ وضوحاً؛ وحرمان بصري ناتج عن عائق يحجب الصورة مثل الساد الولادي أو تدلّي الجفن أو عتامة القرنية.

ويبدأ العلاج بمعالجة السبب: تصحيح الانكسار بنظارة تُلبس طول الوقت — وقد يكفي ذلك وحده لأشهر — ورفع أي عائق يحجب الصورة جراحياً إن وُجد. ثمّ يأتي تحفيز العين الضعيفة بتغطية العين الأقوى بلاصقة لعدد ساعات يحدّده الطبيب يومياً، أو بتغبيش رؤيتها بقطرة موصوفة كبديل عند من لا يتحمّل اللاصقة، مع علاج الحَول إن وُجد ومتابعة منتظمة تُعدّل الخطّة على النتيجة.

وأخطر ما يُقال للأهل إنّ الطفل سيتحسّن مع العمر — وهذا غير صحيح، والتأخير يقلّل فرص التحسّن. وحدود العلاج يجب أن تكون واضحة: التغطية تحسّن حدّة إبصار العين الضعيفة لكنها لا تُقوّم انحراف العين تجميلياً، فالحَول قد يحتاج جراحة منفصلة. والنظارة وحدها لا تكفي في كل الحالات. ولا يوجد تمرين منزلي أو جهاز أو مكمّل يُغني عن التشخيص والمتابعة. والالتزام الزائد بلا إشراف مضرّ: التغطية أكثر من الموصوف قد تُضعف العين المغطّاة نفسها.

والكسل غالباً لا يُلاحَظ بالنظر؛ فالطفل يرى بعين واحدة جيداً ويتصرّف طبيعياً تماماً ولا يشتكي. لذلك يبقى الفحص هو الوسيلة الوحيدة: عند الولادة، وفي زيارات متابعة الطفل السليم، وقبل دخول المدرسة، وفوراً عند وجود حَول أو ميل رأس أو تقريب الأشياء أو غلق عين في الضوء أو تاريخ عائلي لكسل العين أو الحَول أو نظارة قويّة مبكرة. والترشيح لكل خطوة يُحدَّد بالفحص السريري وقياس النظر بعد توسيع الحدقة، لا بالقراءة.
Lazy eye — medically called amblyopia — is reduced visual acuity in one eye, or less often in both, that arises because the brain never learned to use the image coming from that eye during the early years of visual development. In most cases the eye itself is structurally normal. Treatment is therefore aimed at retraining the visual pathway in the brain, not at strengthening the eye with a device, drops or vitamins.

Amblyopia follows three main routes. A squint makes the brain suppress the image from the deviating eye to avoid double vision. A difference in refractive power between the two eyes — or a large refractive error in both — leaves one image permanently less sharp. And visual deprivation occurs when something physically blocks the image, such as a congenital cataract, a drooping eyelid or a corneal opacity.

Treatment begins by addressing the cause: correcting the refractive error with glasses worn full time, which alone may be enough for several months, and surgically removing any obstruction blocking the image. Then the weaker eye is stimulated by patching the stronger eye for a number of hours each day set by the ophthalmologist, or by blurring it with prescribed drops as an alternative for children who cannot tolerate a patch, together with treatment of any squint and regular review that adjusts the plan according to progress.

The most damaging thing a family can be told is that the child will grow out of it. That is not true, and delay reduces the chance of improvement. The limits must also be clear: patching improves the acuity of the weaker eye but does not straighten a deviating eye cosmetically — a squint may need separate surgery. Glasses alone are not sufficient in every case. No home exercise, gadget or supplement replaces diagnosis and follow-up. And doing more than prescribed is harmful: patching beyond the recommended hours can weaken the covered eye itself.

Amblyopia usually cannot be seen. A child who sees well with one eye behaves entirely normally and never complains. Examination is therefore the only way to find it: at birth, at well-child visits, before starting school, and immediately if there is a squint, a head tilt, holding objects very close, closing one eye in bright light, or a family history of amblyopia, squint or strong glasses at an early age. Suitability for each step is determined by clinical examination and refraction after dilation, not by reading.

انسداد مجرى الدمع الولادي حالة شائعة عند الرُضّع يبقى فيها الطرف السفلي للقناة الدمعية الأنفية مغلقاً بغشاء رقيق لم ينفتح عند الولادة، فلا يتصرّف الدمع إلى الأنف كما ينبغي ويتراكم في العين. وفتح مجرى الدمع هو مجموعة الخطوات التي تُعيد هذا التصريف، وتبدأ بالأبسط وتتدرّج فقط إن لم تنجح.

ويظهر عادةً في الأسابيع الأولى من العمر: دمع مستمرّ يفيض على الخدّ من عين واحدة أو كلتيهما، ورموش ملتصقة وقشور عند الاستيقاظ، ومخاط أبيض أو أصفر متكرّر، واحمرار بسيط في جلد الجفن من كثرة المسح. والعين نفسها تبدو صافية وقرنيتها شفّافة، والطفل لا يُظهر ألماً ولا خوفاً من الضوء — وهذه التفاصيل هي ما يفرّقه عن حالات أخطر.

ومعظم الحالات تنفتح تلقائياً خلال السنة الأولى من العمر، ولذلك تكون البداية علاجاً محافظاً: تنظيف الجفن بلطف بماء معقّم أو محلول ملحي، وتدليك منطقة كيس الدمع بحركة نازلة ثابتة كما يُدرّبك الطبيب عليها، ومضاد حيوي موضعي فقط عند وجود التهاب صريح لا في كل مرّة يظهر مخاط. وإن استمرّت الأعراض بعد نحو السنة الأولى — أو أزعجت الطفل بالتهابات متكرّرة قبل ذلك — يُجرى تسبير القناة بمِسبار رقيق يفتح الغشاء، وغالباً تحت تخدير عام قصير. وإن تكرّر الانسداد فقد يُعاد التسبير مع توسيع بالبالون أو تركيب أنبوب سيليكون مؤقّت، وفي حالات قليلة جداً يُصار إلى إنشاء مجرى تصريف جديد.

وما لا تفعله كل خطوة يجب أن يُقال: التدليك ليس فتحاً للقناة، هو ضغط يساعد الغشاء على الانفتاح وقد لا ينجح مهما أُحسن أداؤه. والقطرات المضادّة تعالج الالتهاب المصاحب ولا تُزيل الانسداد. والتسبير لا يُصلح انسداداً سببه تشوّه عظمي أو غياب فتحة القناة أو انسداد عالٍ في النقاط الدمعية، وهذه تحتاج تخطيطاً مختلفاً. وقد يعود الدمع بعد إجراء ناجح، وقد يحتاج بعض الأطفال أكثر من خطوة.

والأهمّ ألّا يُنسب كل دمع عند الرضيع إلى القناة. فالتورّم الأحمر المؤلم أسفل الزاوية الداخلية للعين مع حرارة قد يكون التهاب كيس الدمع ويحتاج علاجاً عاجلاً؛ والكتلة المزرقّة عند حديث الولادة في المكان نفسه تحتاج تقييماً سريعاً؛ والدمع المصحوب بخوف شديد من الضوء أو تعتيم القرنية أو كِبَر قطرها قد يدلّ على زَرَق ولادي وهو حالة تهدّد البصر لا انسداداً دمعياً. والترشيح لأي خطوة يُحدَّد بالفحص السريري لعين الطفل كاملة، لا بالقراءة.
Congenital nasolacrimal duct obstruction is a common condition in infants in which the lower end of the tear duct remains closed by a thin membrane that did not open at birth, so tears cannot drain into the nose properly and pool in the eye instead. Opening the tear duct refers to the set of steps that restore this drainage, beginning with the simplest and escalating only if they do not work.

It usually shows itself in the first weeks of life: constant tearing overflowing onto the cheek from one or both eyes, lashes stuck together with crusting on waking, recurrent white or yellow discharge, and mild redness of the lid skin from repeated wiping. The eye itself looks clear with a transparent cornea, and the baby shows no pain and no aversion to light — and these details are precisely what distinguish it from more serious conditions.

Most cases open spontaneously during the first year, so management begins conservatively: gentle lid cleaning with sterile water or saline, massage over the tear sac with a firm downward stroke exactly as your doctor teaches you, and topical antibiotic only when there is genuine infection rather than every time discharge appears. If symptoms persist beyond roughly the first year — or cause repeated infections before then — the duct is probed with a fine instrument that opens the membrane, usually under a short general anaesthetic. If obstruction recurs, probing may be repeated with balloon dilation or the temporary placement of a silicone tube, and in a very small number of cases a new drainage passage is created surgically.

What each step cannot do should be said plainly. Massage does not open the duct; it applies pressure that helps the membrane give way, and it may not succeed however well it is performed. Antibiotic drops treat the accompanying infection and do not relieve the blockage. Probing does not correct obstruction caused by a bony abnormality, an absent duct opening or a high blockage at the punctum, and those need a different plan. Tearing can return after a successful procedure, and some children need more than one step.

Most important of all, not every tearing infant has a blocked duct. A painful red swelling below the inner corner of the eye with fever may be infection of the tear sac and needs urgent treatment; a bluish lump in the same place in a newborn needs prompt assessment; and tearing with marked light intolerance, a cloudy cornea or an enlarged corneal diameter may indicate congenital glaucoma, a sight-threatening condition rather than a drainage problem. Suitability for any step is determined by a full examination of the child's eye, not by reading.
