تقنية طومسون أسلوب من أساليب المناولة اليدوية للعمود الفقري (كايروبراكتيك) يُستخدَم فيه سرير خاص أقسامه تهبط هبوطاً قصيراً لحظة الضغط، فيقلّ مقدار القوة التي يحتاجها المعالج لتحريك المفصل. تُستخدَم لآلام الظهر والرقبة الميكانيكية عند مرضى مُنتقَين، والغرض منها تحسين مدى الحركة وتخفيف الألم — لا «إعادة فقرة إلى مكانها» كما يُقال شائعاً.

\*\*الحقيقة التي تُغيّر القرار\*\*: أغلب آلام الظهر تتحسّن مع الوقت والحركة والتمارين المتدرّجة، والمناولة اليدوية في أفضل الأحوال تُختصر بعض الطريق لبعض المرضى. وهي \*\*ليست علاجاً للانزلاق الغضروفي\*\* ولا لتضيّق القناة ولا للجَنَف، ولا تُغني عن تشخيص السبب.

\*\*ولا تُجرى المناولة قبل استبعاد ما يمنعها\*\*: هشاشة عظام أو كسر أو اشتباه كسر، ورم أو تاريخ سرطان، عدوى في العمود الفقري، رضّ حديث، اضطراب تخثّر أو مضادات تخثّر، وأي علامة عصبية. \*\*علامات الخطر التي تعني توقّفاً فورياً وتقييماً طبياً عاجلاً — لا جلسة\*\*: ضعف متزايد في الساق أو الذراع، أو فقدان التحكّم في البول أو البراز، أو تخدير في منطقة السرج بين الفخذين، أو حرارة مع ألم ظهر، أو ألم ليلي مستمرّ يوقظ من النوم، أو نقص وزن غير مبرَّر، أو ألم بدأ بعد حادث قوي.

The Thompson technique is one approach to manual spinal manipulation (chiropractic) that uses a special table whose sections drop a short distance at the moment of thrust, reducing the force the practitioner needs to move the joint. It is used for mechanical back and neck pain in selected patients, with the aim of improving range of movement and easing pain — not of "putting a vertebra back in place", as is commonly claimed.

\*\*The fact that changes the decision\*\*: most back pain improves with time, movement and graded exercise, and manual manipulation at best shortens part of that road for some patients. It is \*\*not a treatment for disc herniation\*\*, spinal stenosis or scoliosis, and it does not replace diagnosing the cause.

\*\*Manipulation is not performed before excluding what forbids it\*\*: osteoporosis, a fracture or suspected fracture, tumour or a cancer history, spinal infection, recent trauma, a bleeding disorder or anticoagulants, and any neurological sign. \*\*Warning signs that mean stopping immediately and seeking urgent medical assessment — not a session\*\*: increasing weakness in a leg or arm, loss of bladder or bowel control, numbness in the saddle area between the thighs, fever with back pain, persistent night pain that wakes you, unexplained weight loss, or pain that began after a significant accident.

In neck manipulation specifically there is a \*\*very rare but grave\*\* complication: vertebral artery dissection, which can cause a stroke. This possibility must therefore be discussed with you explicitly before you consent; you must tell the practitioner about any unusual headache, dizziness or new severe neck pain, \*\*and call emergency services at once if after a session you develop a sudden severe headache, dizziness with double vision, difficulty speaking or one-sided weakness\*\*. A qualified practitioner explains this unprompted; one who does not has not earned your spine.
