العلاج باليود المشع علاج يُعطى بالفم على شكل كبسولة أو سائل يحتوي يوداً مشعاً. خلايا الغدة الدرقية هي الخلايا الوحيدة في الجسم التي تسحب اليود وتخزّنه، ولذلك يتجمّع فيها العلاج ويعمل فيها موضعياً مع تعرّض محدود لبقية الأنسجة. يُستخدم في فرط نشاط الغدة الدرقية، سواء كان بسبب داء جريفز أو عقدة أو عقد نشطة، وكذلك بعد استئصال الغدة في بعض حالات سرطان الدرقية المتمايز للتخلّص من بقايا نسيج درقي.

قرار العلاج يُتخذ بعد تقييم كامل يشمل الفحص السريري وتحاليل وظائف الدرقية والتصوير، وتُختار الجرعة بأقلّ قدر يُتوقّع أن يكون فعّالاً في حالتك، ويوازن الطبيب بين الفائدة المرجوّة والتعرّض للإشعاع. هذا علاج مُبرّر ومنظّم في وحدات الطبّ النووي، لكنه ليس فحصاً روتينياً: له تعليمات ما قبل وما بعد يجب الالتزام بها حرفياً كي ينجح ويبقى آمناً لمن حولك.

\*\*أخبري الطبيب فوراً إن كنتِ حاملاً أو مُرضعة أو تشكّين في احتمال الحمل — الحمل والإرضاع مانعان للعلاج باليود المشع، ويُطلب عادة اختبار حمل قبل الإعطاء، وتُطلب المباعدة عن الحمل لفترة يحدّدها طبيبك بعده.\*\* كما يلزم إخبار الطبيب عن كل دواء أو مكمّل يحتوي يوداً، وعن أي صورة أشعة استُخدمت فيها صبغة مظلّلة حديثاً، لأنها تُشبع الغدة باليود وتُضعف نتيجة العلاج.

حدود العلاج واضحة: لا يعمل فوراً، فاستجابة الغدة تحتاج أسابيع إلى أشهر وقد يلزم إعطاء ثانٍ. وكثير من المرضى يُصابون بكسل درقي بعده ويحتاجون هرموناً بديلاً بانتظام، وهذه نتيجة متوقّعة يجري ضبطها بالمتابعة لا مفاجأة. ولا يفيد اليود المشع في أنواع سرطان الدرقية التي لا تسحب اليود، ولا يعالج تغيّرات العين المصاحبة لداء جريفز وقد يزيدها عند بعض المرضى، ولهذا يُقيَّم الأمر مسبقاً. الترشيح والجرعة يُحدَّدان بالتقييم الطبي لا بقراءة صفحة.
Radioactive iodine therapy is a treatment given by mouth as a capsule or a liquid containing radioactive iodine. Thyroid cells are the only cells in the body that actively take up and store iodine, so the treatment concentrates there and acts locally, with limited exposure to other tissues. It is used for an overactive thyroid, whether caused by Graves' disease or by one or more overactive nodules, and also after thyroid surgery in selected cases of differentiated thyroid cancer to clear remaining thyroid tissue.

The decision is taken after a full assessment that includes clinical examination, thyroid function tests and imaging. The activity given is chosen as the smallest amount expected to be effective for your particular situation, and your doctor balances the expected benefit against radiation exposure. This is a justified, tightly regulated treatment delivered by nuclear medicine units, but it is not a routine test: it comes with before and after instructions that must be followed exactly so that it works and remains safe for the people around you.

\*\*Tell your doctor immediately if you are pregnant, breastfeeding, or think you might be pregnant. Pregnancy and breastfeeding are contraindications to radioactive iodine, a pregnancy test is usually required before the dose is given, and you will be asked to avoid conceiving for a period your doctor defines afterwards.\*\* You must also declare every medicine or supplement containing iodine, and any recent scan that used contrast dye, because these saturate the gland with iodine and weaken the result.

The limits are clear. It does not work instantly: the gland responds over weeks to months and a second treatment is sometimes needed. Many patients later become underactive and need regular thyroid hormone replacement; this is an expected outcome managed by follow-up, not a surprise. Radioactive iodine does not help thyroid cancers that do not take up iodine, and it does not treat the eye changes of Graves' disease and may worsen them in some patients, which is why this is assessed in advance. تصوير العظام بالنظائر المشعة فحص من فحوص الطبّ النووي يُحقن فيه مقتفٍ مشع في الوريد، يرتبط بمناطق النشاط العظمي حيث يعيد العظم بناء نفسه أو يُصلح ضرراً، ثم تُصوّر بكاميرا جاما لتُظهر خريطة لهذا النشاط في الهيكل العظمي. يُجرى إما مسحاً كاملاً للجسم، أو ثلاثي المراحل يصوّر المنطقة مباشرة بعد الحقن ثم بعد فترة قصيرة ثم بعد ساعات، وهو النمط الذي يفيد في التمييز بين التهاب العظم والتهاب الأنسجة الرخوة حوله.

يُطلب الفحص عادة لألم عظمي لم تُفسّره الصور العادية، أو لكسر خفي أو إجهادي لا يظهر بالأشعة السينية، أو للاشتباه بعدوى في عظم أو حول مفصل صناعي، أو لتقييم ارتخاء مفصل صناعي، أو للبحث عن انتشار سرطاني في العظام ومتابعة استجابته. قوّته أنه يرى الهيكل كاملاً في فحص واحد ويكشف تغيّراً في نشاط العظم قبل أن يظهر تغيّر في شكله.

من ناحية الإشعاع: هذا \*\*فحص تشخيصي بجرعة منخفضة ومبرَّرة سريرياً\*\*، لا يُطلب إلا حين يُتوقّع أن تُغيّر نتيجته القرار العلاجي، والمقتفي يُطرح تدريجياً مع البول خلال ساعات. لا يجعلك الفحص خطراً على أسرتك بعد اتّباع التعليمات المعتادة، وليس فيه تخدير ولا صبغة مظلّلة كتلك المستخدمة في الأشعة المقطعية. \*\*أخبري الطبيب قبل الحقن إن كنتِ حاملاً أو تشكّين في الحمل أو مُرضعة، فقد يُؤجَّل الفحص أو يُستبدل، وقد يُطلب إيقاف الإرضاع مؤقتاً لمدّة يحدّدها الفريق.\*\*

حدود الفحص جوهرية لفهم نتيجته: هو يُظهر \*\*مكان\*\* النشاط العظمي ولا يقول \*\*سببه\*\*، فالكسر والالتهاب والعدوى والورم قد تُنتج جميعها بقعة نشطة متشابهة، ولذلك تُقرأ النتيجة مع القصّة السريرية والصور الأخرى وقد تُستكمل بالرنين أو المقطعي أو خزعة. كما أن الفحص السلبي لا ينفي كل مرض، وبعض الأورام قليلة النشاط العظمي قد لا تُظهر التقاطاً واضحاً. النتيجة تفسيرها للطبيب المعالج لا للمريض وحده.
A radionuclide bone scan is a nuclear medicine test in which a radioactive tracer is injected into a vein, binds to areas of active bone turnover where bone is rebuilding or repairing itself, and is then imaged with a gamma camera to produce a map of that activity across the skeleton. It may be performed as a whole-body scan, or as a three-phase study that images the region immediately after injection, again shortly afterwards, and once more some hours later, which is the pattern that helps separate infection of the bone from inflammation of the surrounding soft tissue.

The scan is usually requested for bone pain that ordinary imaging has not explained, for a hidden or stress fracture invisible on plain X-ray, for suspected infection in a bone or around a joint replacement, to assess a loose prosthesis, or to look for cancer spread to bone and follow its response. Its strength is that it surveys the whole skeleton in a single study and detects a change in bone activity before the shape of the bone changes.

On radiation: this is a \*\*diagnostic study using a low, clinically justified dose\*\*, requested only when the result is expected to change management, and the tracer is gradually cleared in the urine over hours. Understanding the limits is essential to reading the result. The scan shows \*\*where\*\* bone activity is increased but not \*\*why\*\*: a fracture, inflammation, infection and a tumour can all produce a similar hot spot, so the images are interpreted alongside the clinical story and other studies, and may be completed with MRI, CT or a biopsy. A negative scan does not exclude every disease, and some tumours with low bone turnover may not show clear uptake. Interpretation belongs with your treating doctor, not with the patient alone.

علاج سرطان البروستاتا بالراديوم علاج نويدات مشعّة يُعطى بالوريد، يستخدم الراديوم-223 الذي يتصرّف في الجسم كما يتصرّف الكالسيوم، فيتجمّع في مناطق بناء العظم النشط حيث توجد النقائل العظمية. هناك يُطلق إشعاع من نوع ألفا مداه قصير جداً — أجزاء من المليمتر — فيؤثّر في الخلايا الورمية المجاورة مع أذى محدود للنسيج السليم حولها ولنقيّ العظم البعيد.

يُطرح هذا العلاج في حالة محدّدة: سرطان بروستاتا مقاوم للإخصاء انتشر إلى العظام وسبّب أعراضاً، مع عدم وجود انتشار حشوي كبير في الكبد أو الرئة. القرار يُتخذ في فريق أورام، ويُبنى على التصوير وتعداد الدم ووظائف الكلى والكبد والعلاجات التي أخذتها سابقاً، ويُعطى على جلسات متكرّرة بفواصل منتظمة مع تعداد دم قبل كل جلسة.

من ناحية الإشعاع والاحتياطات: الجرعة محسوبة ومبرَّرة، ومدى إشعاع ألفا القصير يعني أن الخطر على من حولك محدود جداً، والاحتياطات المطلوبة عملية وبسيطة: نظافة صارمة في الحمّام لأن جزءاً من العلاج يُطرح مع الفضلات، وغسل اليدين، وغسل الملابس الملوّثة منفصلة، للمدّة التي يحدّدها فريق الطبّ النووي. \*\*أخبر الطبيب إن كان في بيتك أو مرافقيك امرأة حامل أو مُرضعة، والتزم بوسيلة وقاية جنسية للمدّة التي يحدّدها لك الفريق.\*\*

Radium therapy for prostate cancer is a radionuclide treatment given into a vein. It uses radium-223, which behaves in the body much as calcium does, so it concentrates in areas of active bone formation where bone metastases sit. There it emits alpha radiation with a very short range, a fraction of a millimetre, so it acts on nearby tumour cells with limited damage to the healthy tissue around them and to more distant bone marrow.

The treatment is considered in a specific situation: castration-resistant prostate cancer that has spread to bone and is causing symptoms, without significant visceral spread to the liver or lungs. The decision is made within an oncology team and is based on imaging, blood counts, kidney and liver function and the treatments you have already received. It is given as repeated sessions at regular intervals, with a blood count checked before each one.

On radiation and precautions: the activity given is calculated and justified, and the very short range of alpha radiation means the risk to people around you is minimal. It can lower blood cell and platelet counts, so regular laboratory monitoring is required, and it may cause diarrhoea, nausea or fatigue.
