تقطيب الجروح السطحية إجراء إسعافي يُغلق فيه الطبيب جرحاً في الجلد بخيوط جراحية دقيقة بعد تنظيفه، فتتلاصق حافّتا الجرح ويشفى أسرع وبأثر أقل. ويُقصد بالسطحية الجروح التي لا تتعدى الجلد والنسيج الدهني تحته، ولا تصل إلى عضلة أو وتر أو عصب أو تجويف مفصل.

ليس كل جرح يحتاج قطباً. الجرح القصير الذي تتقارب حافّتاه وحدهما قد يُغلق بشرائط لاصقة طبية أو بصمغ جراحي، أما الجرح الذي تفتح فوهته أو يتجاوز طوله سنتيمترات قليلة أو يقع فوق مفصل أو على الوجه أو ينزف بلا توقف فيُقيَّم للإغلاق بالخياطة. والوقت عامل حاسم: تُغلق معظم الجروح خلال الساعات الأولى من حدوثها، وكلما تأخّرت المراجعة ارتفع احتمال أن يُترك الجرح مفتوحاً عن قصد ليُشفى بالتضميد بدل الخياطة، تفادياً لحصر العدوى داخله. الترشيح للإجراء وطريقة الإغلاق يُحدَّدان بالفحص السريري لا بالقراءة.

ومن المهم معرفة حدود الإجراء: التقطيب يقارب حافّتي الجرح فقط. هو لا يُعقّم الجرح ولا يمنع العدوى بحد ذاته، ولا يُغني عن الغسل الغزير الذي هو أهم خطوة في العلاج، ولا يُصلح قطعاً في وتر أو عصب أو وعاء لم يُكتشف بالفحص، ولا يُلغي الحاجة إلى مراجعة التطعيم ضد الكزاز، ولا يمنع الندبة نهائياً — أقصى ما يفعله أن تكون الندبة أرفع وأنظم. وبعض الجروح لا تُخاط أصلاً: الملوّثة بالتراب أو الروث، والعضّات، والجروح الوخزية العميقة، وما تجاوز نافذته الزمنية.

وهناك حالات لا تنتظر عيادة ولا تحتمل بحثاً على الإنترنت: \*\*الجرح النافذ في الصدر أو البطن أو الرقبة أو العين، والنزيف الذي يقذف أو لا يتوقف بالضغط، والجرح المصحوب بفقدان إحساس أو عجز عن تحريك الطرف أو الأصابع، والجرح عند شخص مشوّش أو فاقد للوعي أو بعد سقوط عنيف أو حادث سير\*\*. في هذه الحالات اتصل بالإسعاف على 911 مباشرة واضغط على الجرح أثناء الانتظار.
Superficial wound suturing is an urgent-care procedure in which a clinician closes a skin wound with fine surgical sutures after cleaning it, bringing the wound edges together so that healing is faster and the resulting scar is finer. Superficial means the injury does not extend beyond the skin and the fatty layer beneath it, and does not involve muscle, tendon, nerve or a joint space.

Not every wound needs stitches. A short wound whose edges already sit together may be closed with adhesive skin strips or surgical glue, while a wound that gapes open, runs more than a few centimetres, crosses a joint, sits on the face, or keeps bleeding is assessed for suture closure. Timing is decisive: most wounds are closed within the first hours after injury, and the longer the delay the more likely the wound will be deliberately left open to heal with dressings instead, so that infection is not sealed inside it. Whether you are a candidate, and which closure method suits your wound, is determined by physical examination and not by reading online.

The limits of the procedure matter just as much. Suturing only approximates the wound edges. It does not sterilise the wound or prevent infection by itself, it does not replace copious irrigation which is the single most important step, it does not repair an unrecognised tendon, nerve or vessel injury, it does not remove the need to review tetanus vaccination, and it cannot abolish a scar; at best it makes the scar narrower and neater. Some wounds are deliberately not sutured at all, including heavily contaminated wounds, animal and human bites, deep puncture wounds, and wounds that present after the safe closure window.

Some situations must never wait for a clinic appointment or an internet search: a penetrating wound of the chest, abdomen, neck or eye; bleeding that spurts or does not stop with firm pressure; a wound with numbness or inability to move the limb or fingers; and any wound in a person who is confused, drowsy or unconscious, or who has had a violent fall or road traffic crash. In these cases call emergency services on 911 at once and keep pressing on the wound while you wait.

التسمم بأول أكسيد الكربون حالة إسعافية يستنشق فيها الشخص غازاً لا لون له ولا رائحة ولا طعم، ينتج عن الاحتراق الناقص للوقود: صوبات الغاز والكاز والحطب والفحم، ومسخّنات الماء، والمواقد والأفران، ومحرّك سيارة يعمل في مرآب أو مكان مغلق. يرتبط هذا الغاز بالهيموغلوبين ارتباطاً أقوى بكثير من الأكسجين، فيحجب وصول الأكسجين إلى الدماغ والقلب حتى مع تنفّس طبيعي في الظاهر.

الأعراض مخاتلة وتُشبه الإنفلونزا أو الإجهاد: صداع ودوخة وغثيان وضعف عام وتشوّش تركيز، ثم قد تتطوّر إلى قيء وفقدان وعي وتشنّج. وهناك مؤشران يفرّقان التسمم عن الإنفلونزا: أن يشتكي أكثر من فرد في البيت الأعراض في الوقت نفسه — بما في ذلك الحيوانات المنزلية — وأن تخفّ الأعراض عند الخروج إلى الهواء الطلق وتعود مع الرجوع إلى المكان. والحوامل والأطفال وأصحاب أمراض القلب والرئة أكثر تأثراً، والجنين أكثر تأثراً من أمه.

\*\*عند الاشتباه لا تنتظر ولا تبحث عن تفسير آخر: أخرج الجميع فوراً إلى هواء نقي، واتصل بالإسعاف على 911 من خارج المكان، ولا تعد إلى الداخل لإطفاء المصدر أو جمع الأغراض.\*\* العلاج الأساسي هو الأكسجين بتركيز عالٍ، وكلما بدأ أسرع قصرت مدة بقاء الغاز في الدم. ولا يوجد شراب ولا دواء منزلي يُبطل مفعول الغاز: فتح الشبّاك وشرب الحليب أو القهوة أو اللبن ليس علاجاً، والاعتماد على أن المصاب لا يزال واعياً ويتكلّم خطأ شائع ومكلف.

حدود العلاج يجب أن تكون واضحة: الأكسجين يُسرّع طرح الغاز لكنه لا يُصلح كل أذى وقع في الدماغ والقلب. كما أن جهاز قياس الأكسجين على الإصبع قد يعطي قراءة طبيعية مطمئنة كذباً في هذا التسمم، وقيمة تحليل الكربوكسي هيموغلوبين لا تتناسب دائماً مع شدة الحالة، فالقرار سريري بالأساس. وقد تظهر أعراض متأخرة بعد أيام إلى أسابيع مثل النسيان وتبدّل المزاج وبطء التفكير واضطراب الحركة، ولذلك تُطلب متابعة عصبية بعد التعرّض الشديد. أما الأكسجين المضغوط في الغرفة الضاغطة فيُدرَس في حالات مختارة كفقدان الوعي أو الحمل أو الأعراض العصبية أو القلبية، ويُحدَّد بقرار طبي عاجل لا برغبة المريض.

والوقاية هي العلاج الحقيقي: لا تُشغّل صوبة فحم أو غاز في غرفة نوم مغلقة، وأطفئ الصوبة قبل النوم، واترك تهوية دائمة ولو صغيرة، وافحص المدخنة وخرطوم الغاز ومسخّن الماء سنوياً عند فنّي مؤهل، ولا تُدفّئ السيارة داخل مرآب مغلق. وكاشف أول أكسيد الكربون المنزلي وسيلة غير مكلفة تُنبّه قبل ظهور الأعراض.
Carbon monoxide poisoning is an emergency caused by inhaling a gas that has no colour, smell or taste, produced by incomplete combustion of fuel: gas, kerosene, wood and charcoal heaters, water heaters, stoves and ovens, and car engines running in a garage or enclosed space. The gas binds to haemoglobin far more strongly than oxygen does, so it blocks oxygen delivery to the brain and heart even while breathing looks normal.

The symptoms are deceptive and mimic influenza or exhaustion: headache, dizziness, nausea, general weakness and poor concentration, progressing in more severe exposure to vomiting, loss of consciousness and seizures. Two clues separate poisoning from flu: several people in the same home feeling unwell at the same time, pets included, and symptoms that ease in fresh air and return on going back inside. Pregnant women, children and people with heart or lung disease are more vulnerable, and an unborn baby is affected more than the mother.

\*\*If you suspect it, do not wait and do not look for another explanation: get everyone out into fresh air, call emergency services on 911 from outside the building, and do not go back in to switch off the source or collect belongings.\*\* The mainstay of treatment is high-concentration oxygen, and the earlier it starts the sooner the gas clears from the blood. No home remedy neutralises the gas: opening a window and drinking milk, coffee or yoghurt is not treatment, and assuming that someone is safe because they are still awake and talking is a common and costly mistake.

The limits of treatment must be stated plainly. Oxygen speeds elimination of the gas but does not undo injury already sustained by the brain and heart. A fingertip pulse oximeter can give a falsely reassuring normal reading in this poisoning, and the measured carboxyhaemoglobin level does not always match the severity of the case, so decisions are primarily clinical. Delayed symptoms may appear days to weeks later, such as forgetfulness, mood change, slowed thinking and movement problems, which is why neurological follow-up is arranged after significant exposure. Hyperbaric oxygen is considered in selected cases such as loss of consciousness, pregnancy, or neurological or cardiac features, and that decision is an urgent medical one.

Prevention is the real treatment: never run a charcoal or gas heater in a closed bedroom, switch heaters off before sleeping, keep permanent ventilation even if small, have the chimney, gas hose and water heater checked yearly by a qualified technician, and never warm a car inside a closed garage. A household carbon monoxide alarm is an inexpensive device that warns before symptoms begin.

النزيف الحاد فقدان سريع للدم بكمية تُهدّد وصول الأكسجين إلى الأعضاء. قد يكون ظاهراً من جرح، أو خارجاً من فتحة طبيعية مثل نزيف أنف غزير أو قيء دموي أو براز أسود أو دموي أو نزيف رحمي شديد أو نزيف بعد الولادة أو بعد عملية، وقد يكون داخلياً مخفياً بعد رضّ أو تمزّق كيس أو حمل خارج الرحم. علاجه سباق مع الوقت وليس إجراءً يُؤجَّل.

خطورة النزيف لا تُقاس بكمية الدم على الأرض وحدها. راقب علامات نقص الدم في الدورة: شحوب وبرودة وتعرّق، تسارع النبض، دوخة عند الجلوس أو الوقوف، عطش شديد، تنفّس سريع، قلق ثم خمول وتشوّش. عند الأطفال وكبار السن ومن يتناولون أدوية للقلب قد تتأخر هذه العلامات ثم تتدهور الحالة بسرعة. \*\*لا تنتظر ظهورها لتتحرّك: اتصل بالإسعاف على 911 عند أي نزيف غزير أو لا يتوقف بالضغط، أو مع أي واحدة من هذه العلامات، أو بعد رضّ قوي حتى لو لم يظهر دم.\*\*

أول علاج هو الضغط المباشر القوي المتّصل على مكان النزيف بشاش أو قماش نظيف لعدة دقائق دون رفعه للتفحّص، مع رفع الطرف. وإن استمر نزيف شديد من طرف ولم يتوقف بالضغط فقد يُستخدم رباط ضاغط مخصّص أعلى الجرح ويُشدّ حتى يتوقف النزيف ويُسجّل وقت وضعه. لا تربط الطرف بحبل أو سلك أو خرطوم، ولا تُخرج جسماً منغرزاً في الجسم، ولا تنزع الشاش المُشبَّع بالدم بل أضف فوقه واستمر بالضغط. وفي نزيف الأنف: انحن للأمام واضغط على الجزء الطري من الأنف من 10 إلى 15 دقيقة متّصلة، ولا تُرجع رأسك للخلف.

في الطوارئ يسير العلاج على مسارين في الوقت نفسه: إيقاف المصدر، وتعويض ما فُقد. يُفتح وريد أو أكثر، وتُسحب فحوص مع تحديد زمرة الدم ومطابقته، وتُعطى سوائل ومشتقات دم بحسب الحالة، وقد يُعطى في النزيف الرضحي المبكر دواء يقلّل تحلّل الخثرة، ويُعدَّل أثر مميّعات الدم لمن يتناولها. أما إيقاف المصدر فقد يكون بالخياطة أو التضميد الضاغط، أو بالمنظار العلاجي في النزيف الهضمي، أو بالقسطرة التداخلية لإغلاق شريان، أو بعملية جراحية، أو بأدوية وإجراءات خاصة في نزيف ما بعد الولادة.

وحدود ما يُعمل خارج المستشفى صريحة: السيطرة على نزيف ظاهر لا تُوقف نزيفاً داخلياً، والرباط الضاغط إجراء مؤقت لإنقاذ الحياة لا علاج نهائي، وتوقّف الدم لا يعني أن الخطر انتهى فقد يعود النزيف بعد ساعات. ولا يوجد إسعاف منزلي للنزيف الداخلي؛ النقل السريع بالإسعاف هو العلاج. ولا تُستخدم على الجرح مواد كالتراب أو القهوة أو معجون الأسنان أو الأعشاب: فهي لا تُوقف نزيفاً، وتُسبّب عدوى، وتُعقّد تنظيف الجرح والجراحة لاحقاً.
Acute haemorrhage is rapid blood loss in a volume that threatens oxygen delivery to the organs. It may be visible from a wound, or come from a natural orifice as heavy nosebleed, vomited blood, black or bloody stool, heavy uterine bleeding, or bleeding after childbirth or surgery, and it may be hidden inside the body after trauma, a ruptured cyst or an ectopic pregnancy. Treating it is a race against time, not something to postpone.

Severity is not judged by the amount of blood on the floor alone. Watch for signs that the circulation is running short: pallor, cold clammy skin, sweating, a fast pulse, dizziness on sitting or standing, intense thirst, rapid breathing, restlessness and then drowsiness and confusion. In children, older adults and people on heart medication these signs may appear late and then deteriorate quickly. \*\*Do not wait for them before acting: call emergency services on 911 for any heavy bleeding, any bleeding that does not stop with pressure, any of these signs, or after a forceful injury even when no blood is visible.\*\*

The first treatment is firm, continuous direct pressure over the bleeding point with clean gauze or cloth for several minutes without lifting it to look, together with elevation of the limb. If severe limb bleeding continues despite pressure, a purpose-made tourniquet may be applied above the wound, tightened until bleeding stops, and the time of application recorded. Do not tie the limb with rope, wire or hose, do not pull out an impaled object, and do not remove blood-soaked gauze; add more on top and keep pressing. For nosebleeds, lean forward and pinch the soft part of the nose continuously for 10 to 15 minutes, and do not tilt the head back.

In the emergency department two tracks run at once: stopping the source and replacing what has been lost. One or more intravenous lines are placed, blood is drawn for tests and for blood grouping and cross-matching, fluids and blood products are given according to the situation, a medicine that reduces clot breakdown may be given early in traumatic bleeding, and the effect of blood thinners is reversed or adjusted in patients taking them. Stopping the source may involve suturing or a pressure dressing, therapeutic endoscopy for gastrointestinal bleeding, interventional catheter embolisation of an artery, an operation, or specific drugs and manoeuvres for postpartum haemorrhage.

The limits of what can be done outside hospital are explicit. Controlling visible bleeding does not stop internal bleeding; a tourniquet is a temporary life-saving measure and not definitive treatment; and bleeding that has stopped does not mean the danger has passed, because it can restart hours later. There is no home remedy for internal bleeding, and rapid ambulance transfer is the treatment. Never apply soil, coffee, toothpaste or herbal powders to a wound: they do not stop bleeding, they cause infection, and they make later wound cleaning and surgery harder.
