تحنيط العصب إجراء يُزال فيه الجزء الملتهب من لبّ السن الموجود داخل تاج السن، ويُترك اللبّ الحيّ داخل الجذور بعد تغطيته بمادّة دوائية تُهدّئه وتحفّز شفاءه، ثم يُغلق السن بحشوة نهائية. وهو الأكثر استخداماً في الأسنان اللبنية عند الأطفال حين يصل التسوّس إلى العصب، ويُستخدم أيضاً بصورة جزئية في الأسنان الدائمة الحديثة التي لم تكتمل جذورها أو بعد كسر رضحي حديث.

وهنا الفارق الجوهري الذي يُربك كثيراً من الأهل: تحنيط العصب ليس سحباً كاملاً للعصب. سحب العصب يُفرّغ اللبّ من التاج والجذور معاً ويحشو القنوات، أمّا التحنيط فيحافظ على لبّ الجذر حيّاً — وهذا مهمّ في السن اللبني لأنّه يترك جذره يتحلّل طبيعياً في وقته فيخرج السن الدائم في مساره الصحيح، ومهمّ في السن الدائم الفتيّ لأنّه يسمح للجذر بمواصلة نموّه وسماكته.

ولا يصلح التحنيط لكل سن مؤلم. يُستبعد إن كان اللبّ ميتاً أو التهابه غير قابل للعلاج، أو كان هناك خرّاج أو تقيّح أو ناسور أو تورّم، أو ارتشاف مَرضي في الجذر، أو فقدان كبير في العظم حول السن، أو حركة واضحة في السن، أو لم يبقَ من السن جدار سليم يحمل الحشوة. وفي هذه الحالات يكون الخيار علاج قناة جذرية كاملاً أو خلع السن مع حافظ مسافة. وحتى عند نجاحه، التحنيط لا يمنع تسوّس بقية الأسنان ولا يُغني عن التنظيف والفلورايد وتقليل السكّريات ومواعيد المتابعة.

والتغطية النهائية جزء من العلاج لا رفاهية: السن اللبني بعد التحنيط يكون أضعف وأقلّ تحمّلاً للعضّ، ولذلك يُفضّل غالباً تاج يغطّي السن كاملاً بدلاً من حشوة تعبئة فقط، لأنّ كسر الحشوة أو جدار السن يعني عودة التسرّب والالتهاب. ويحتاج السن المُحنّط متابعة سريرية وشعاعية دورية للتأكّد من بقاء لبّ الجذر سليماً، لأنّ بداية الفشل قد تكون بلا ألم.

والقرار — تحنيط أم علاج قناة أم خلع — يُحدَّد بالفحص السريري واختبارات حيوية اللبّ والصورة الشعاعية وعمر الطفل ومرحلة تبدّل الأسنان، لا بشدّة الألم وحدها ولا بالقراءة. ووجود سنّ لبني سيسقط لاحقاً ليس سبباً لإهماله؛ فالالتهاب المزمن يؤذي براعم الأسنان الدائمة تحته ويؤثّر على مضغ الطفل ونطقه ونموّ فكّه.
A pulpotomy is a procedure in which the inflamed portion of the dental pulp inside the crown of the tooth is removed while the living pulp inside the roots is preserved, covered with a medicated dressing that calms it and supports healing, and the tooth is then sealed with a definitive restoration. It is most often used in children's primary teeth when decay has reached the nerve, and is also performed as a partial pulpotomy in young permanent teeth whose roots are still immature or after a recent traumatic fracture.

Here lies the distinction that confuses many parents: a pulpotomy is not a full root canal. Root canal treatment empties the pulp from both the crown and the roots and fills the canals, whereas a pulpotomy keeps the root pulp alive. That matters in a primary tooth because it lets the root resorb naturally at the proper time so the permanent tooth erupts along its correct path, and it matters in a young permanent tooth because it allows the root to keep growing in length and thickness.

A pulpotomy is not suitable for every painful tooth. It is ruled out if the pulp is dead or irreversibly inflamed, if there is an abscess, pus, a sinus tract or swelling, if there is pathological root resorption, significant bone loss around the tooth, obvious mobility, or if too little sound tooth structure remains to hold a restoration. In those situations the choice becomes full root canal treatment or extraction with a space maintainer. Even when it succeeds, a pulpotomy does not prevent decay in the remaining teeth and does not replace brushing, fluoride, reducing sugars and keeping review appointments.

The final coverage is part of the treatment rather than a luxury. After a pulpotomy a primary tooth is weaker and less able to withstand biting forces, so a crown covering the whole tooth is often preferred over a simple filling, because a fractured filling or tooth wall means leakage and inflammation return. A treated tooth needs periodic clinical and radiographic review to confirm the root pulp remains healthy, since early failure can be entirely painless.

The decision — pulpotomy, root canal or extraction — is made on clinical examination, pulp vitality testing, radiographs, the child's age and the stage of tooth transition, not on the severity of pain alone and not from reading. A primary tooth that will fall out later is no reason for neglect: chronic infection harms the permanent tooth bud beneath it and affects the child's chewing, speech and jaw development.
